Abstract

Abstract

Patent ductus arteriosus (PDA) is a ductus arteriosus that cannot be closed spontaneously after birth due to failure of physiological closure of the fetal ductus. The initial pharmacological treatment for PDA is ibuprofen. The other drugs that can be given, such as paracetamol which has the same role as ibuprofen by inhibiting the synthesis of prostaglandins. Several studies have compared the effectiveness and side effects of the two drugs and obtained different results have shown that paracetamol cannot be used in all premature infants. This literature review was conducted to determine the comparison of paracetamol and ibuprofen in premature infants diagnosed with patent ductus arteriosus. From the analysis of 15 journals, the comparison of the effectiveness of paracetamol and ibuprofen in premature infants has the same effectiveness and there is no statistically significant difference. But the effectiveness of ibuprofen is much better in premature infants with very low birth weight (VLBW) and extremely low birth weight (ELBW) infants than paracetamol. The adverse effects that are often caused by ibuprofen are gastrointestinal bleeding, thrombocytopenia, increased BUN levels, increased serum creatinine levels, increased incidence of AKI, hyperbilirubinemia, and oliguria. Meanwhile, other side effects such as BPD, IVH, NEC, ROP, sepsis, SGOT and SGPT levels and death were not found to be statistically significant differences in the two drugs.

Abstract

Mental disorders have become a common problem globally. The World Health Organization shows that more than one in six people experienced mental health problems in 2016. Benzodiazepines, a drug that has been on the market since 1957, are often used to treat mental and behavior-related problems. Although effective for certain conditions, long-term use of benzodiazepines can cause significant side effects. Health regulations and guidelines suggest that treatment with benzodiazepines should be carried out rationally with reference to the Decree of the Minister of Health of the Republic of Indonesia in 2021 concerning the National Formulary. This literature aims to conduct a review to evaluate the rationality of the use of benzodiazepines by doctors in various countries over the past ten years. The method in this study is to search for articles through Google Scholar and Pubmed. The results obtained were that benzodiazepines are generally used to treat anxiety and sleep disorders. The maximum dose, which is according to the national formulary and the Food and Drug Administration, does not have a significant difference. Challenges that need to be addressed involve aspects of prescribing, such as ensuring the recipe is in accordance with the right indications, adhering to dosage restrictions, and paying attention to the maximum duration of use. Therefore, there needs to be clear rules to regulate this, supported by active socialization efforts to increase doctors' understanding and compliance with more rational prescribing practices.

Abstract

The use of gadgets in teenagers can’t be separated from the demands of education and entertainment, making teenagers the age group with the highest use of gadgets. Gadgets are also a risk factor that can affect sleep quality. Emission of blue light can disrupt the regulation of the work of the hormone melatonin in the human body, which plays an important role in the regulation and quality of human sleep. This study aims to determine the relationship between the duration of device use and sleep quality in adolescents. The analysis which used in this study is the Pittsburgh Sleep Quality Index (PSQI) questionnaire with data on the number of students who had poor sleep quality as many as 105 students (67.74%) and good sleep quality as many as 50 students (32.26%). The results of the cross table show data that students who have poor sleep quality with high duration of device use are 93 respondents (60%). Chi-Square analysis shows the p-value is 0.000 (p <0.05). So it can be concluded that there is a relationship between the length of use of devices and the quality of sleep among students at SMAN 112 Jakarta, Academic Year 2022 2023.

Abstract

Large epidemiological studies reveal high prevalence of vitamin D deficiency in women, antenatal and breastfeeding mothers. Recent studies found role for vitamin D in pregnancy disorders and complications. This research is a literature review. Literatures was retrieved from Google Scholar, PubMed and Biomed Central, a total of 620 journals, after passing inclusion and exclusion factors, 13 journals were found to be appropriate. Research shows the association of vitamin D deficiency in pregnant women with pre-eclampsia, IUGR, recurrent miscarriage, placental inflammation, gestational diabetes mellitus and bacterial vaginosis. Ali etal, found that total incidence of pre-eclampsia was lower through vitamin D supplementation at dose of 4000 IU/day during pregnancy. Zerofsky etal, found 2000 IU/day during pregnancy more effective for improving vitamin D status and associated with reduced risk of recurrent miscarriage and IUGR. Yin etal, found that vitamin D 600 IU/day during pregnancy significantly reduced risk of GDM. Taheri etal, found a 63.5% BV cure rate with two drops (2000 IU)/day of vitamin D3 oily solution in pregnant women. There is associations between vitamin D deficiency, abnormalities and complications of pregnancy. Giving 2000 IU to pregnant women was good for preventing the risk of pregnancy disorders and drops as the best preparation.
