<![CDATA[Introduction: Untreated dental caries can extend to the dentin and pulp, if the caries expanded and cause exposed dentine tubule, the microorganisms will easily enter and invade the pulp, in this condition, pulpitis is considered as irreversible. Case: A 38 year old female patient came to Poli Spesialis Gigi Rumah Sakit Udayana PSPDG UNUD with a complain that the mandibulary lower right molar had cavities and pain since ±2 months ago. The cavities were not treated until it dilated, pain is usually felt by the patient while doing activities, eating, and drinking, the patient has taken amoxicilin and paracetamol to relieve her complaints, the patients never checked her complaints to the dentist. The present ilness of patient, she still complaining the pain of her teeth. Root canal treatment is an appropriate treatment to restore teeth with diagnosis of irreversible pulpitis and metal onlay as the final restoration, because there is more that 1 cusp involved. Conclusion: The choice of crowning pressureless and step back treatment along with metal onlay restoration can preserve teeth, and restore chewing function, but the aesthetic aspect can be ruled out because in posterior teeth, aesthetics are not of much concern. Pendahuluan: Karies yang tidak ditangani dapat meluas sampai ke dentin atau ke pulpa, jika karies ini sudah meluas dan menyebabkan tubuli dentin terekspos, mikroorganisme akan mudah masuk atau mengivasi pulpa, pada kondisi ini, pulpitis dianggap sebagai ireversibel. Kasus: Pasien perempuan berusian 38 tahun datang ke Poli Spesialis Gigi Rumah Sakit Udayana PSPDG UNUD dengan keluhan gigi geraham belakang bawah kanan berlubang dan sakit sejak ±2 bulan yang lalu. Gigi yang berlubang berawal dari lubang kecil yang tidak dirawat hingga melebar, sakit biasanya dirasakan pasien saat sedang beraktivitas dan saat makan atau minum, pasien pernah mengkonsumsi antibiotik amoxicilin dan paracetamol untuk meredakan keluhannya, pasien belum pernah memeriksakan keluhannya ke dokter gigi. Saat ini pasien masih mengeluhkan sakit pada giginya tersebut. Perawatan saluran akar merupakan perawatan yang tepat merestorasi gigi dengan diagnosis pulpitis ireversibel dan onlay logam sebagai restorasi akhir karena terdapat lebih dari 1 cusp yang terlibat. Simpulan: Pilihan perawatan crowndown pressureless dan step back diserta restorasi onlay logam dapat mempertahankan gigi, dan mengembalikan fungsi kunyah, namun aspek estetik dapat dikesampingkan karena pada gigi posterior, estetik tidak terlalu diperhatikan.]]>
