Abstract

Since 2013, the government of Denpasar City has adopted smoke-free law (KTR). However, the implementation of this law faces several obstacles, one of which is the high social acceptability of smoking in the community. Cigarettes and smoking have been deeply embedded within social and cultural life of the community and become part of the hospitality. Hence, this study aims to explore the social norms of smoking and the potency of local wisdom as a social enforcement for smoking behavior particularly at worship places. This study was a qualitative study located in four sub-districts in Denpasar. Informants and participants were selected purposively. Data was collected through in-depth interviews and FGDs. The study succeeded in interviewing 14 informants and conducting 3 FGDs. The results show that smoking in public places remains common among male, particularly adolescents and the elderly. Smoking behavior is perceived as group identity, however also considered less beneficial due to moral value. Community leaders also noticed smoking in public places. Hence, those leaders have important role to control smoking behavior in public places. Moreover, public education, supervision and enforcement should be considered to engage cultural approach and establish local policy which could accelerate the social enforcement in the community.

Abstract

ABSTRAKTujuan dari penelitian ini adalah untuk memahami kinerja karyawan di salah satu rumah sakit swasta pada tahun 2016. Penelitian ini adalah penelitian kuantitatif deskriptif dengan desain cross sectional. Teknik pengambilan sampel menggunakan random sampling (probability sampling), dengan menggunakan teknik proportional stratified random sampling dengan jumlah sampel 86 orang. Data dianalisis dengan analisis bivariat menggunakan chi-square dengan ? = 0,05. Hasil penelitian ini mengungkap sekitar 51,16% (44 orang) memiliki kualitas kerja minor dari 86 responden. Responden dengan kualitas kerja yang baik adalah 51,16% (44 orang). Efektivitas minor responden adalah 65,12% (56 orang). 51,16% (44 orang) memiliki kebiasaan tepat waktu yang buruk, dan responden dengan independensi rendah adalah 62,79% (54 orang). Perbedaan proporsi hasil yang bermakna secara statistik status pekerjaan responden dengan kualitas karyawan adalah (p = 0,013). Ada perbedaan proporsi yang bermakna secara statistik antara latar belakang pendidikan responden dan efektivitas (p = 0,044), hasil uji analisis dalam proporsi yang berbeda antara latar belakang pendidikan responden dan tepat waktu adalah (p = 0,10). Hasil analisis uji coba pada proporsi umur responden yang berbeda dengan independensi kerja adalah (p = 0,047). Kesimpulan dari penelitian ini adalah setiap hasil penelitian ini terhadap kinerja sub-variabel, ini menunjukkan total dari hasil kinerja karyawan secara umum, yaitu 52,33% (45 orang) dari 86 responden yang berarti kinerja karyawan dapat dikategorikan baik. Beberapa saran yang disarankan dalam penelitian ini, mereka meningkatkan kualitas kerja dan menggunakan teknologi, dana, dan semua peralatan di rumah sakit diperlukan untuk menyelesaikan pekerjaan.Kata Kunci : Performa, karyawan, rumah sakit ABSTRACTThe purpose of this research is to understand the employees’ performance at private hospital in 2016. This research is a descriptive quantitative studies with cross sectional design. The sampling technique used random sampling (probability sampling), by using proportionate stratified random sampling technique with 86 persons as the samples. Data was analised with bivariate analysis using chi-square with ?=0.05. The result of this research reveal around 51.16% (44 persons) have a minor working quality out of 86 respondents. Respondents with good working quality are 51.16% (44 persons). The minor effectiveness of respondents is 65.12% (56 persons). 51.16% (44 persons) have a bad punctual habit, and respondents with low independence are 62.79% (54 persons). The different valuable proportion result statistically employment status of respondents with employees quality is (p=0.013). There is difference valuable proportion statistically between the education background of respondents and effectiveness in (p=0.044), the result of analysis trial in different proportion between education background of respondents and punctual is (p=0.10). And the result of trial analysis in different proportion of age of the respondents with working independence is (p=0.047) The conclusion of this research is every results of this research towards sub-variable performance, it shows the total from employees performance result generally, which is 52.33% (45persons) from 86 respondents that means the employees performance can be categorized as good. Some advices are suggested in this study, they are improving the quality of work and using the technology, funding, and all of the equipments in the hospital are needed to finisih a job.Keywords: performance; employee; private hospital

Abstract

ABSTRAK Kepatuhan mencuci tangan perawat merupakan salah satu faktor yang mempunyai pengaruh besar terhadap pencegahan terjadinya infeksi rumah sakit. Perawat memiliki andil yang sangat besar terhadap terjadinya penyebaran infeksi rumah sakit karena perawat berinteraksi langsung kepada pasien selama 24 jam. Kegagalan dalam melaksanakan cuci tangan sebelum kontak dengan pasien dan selama perawatan pasien, dapat menyebabkan terjadinya infeksi rumah sakit. Dalam penelitian ini bertujuan untuk mengetahui hubungan antara karakteristik, pengetahuan dan motivasi perawat dengan kepatuhan mencuci tangan. Penelitian ini adalah penelitian deskriptif dengan menggunakan desain cross - sectional. Responden yang digunakan sebanyak 97 responden dengan tehnik proportionated stratified random sampling. Hasil penelitian menunjukkan bahwa ada hubungan yang signifikan antara karakteristik (umur) dengan kepatuhan cuci tangan (p=0,007, PR= 3,68), tingkat pendidikan dengan kepatuhan mencuci tangan (p=0,021, PR= 0,33), pengetahuan dengan kepatuhan cuci tangan (p=0,003, PR= 4,16) dan motivasi dengan kepatuhan cuci tangan (p=0,013). Untuk variabel yang tidak berhubungan dengan kepatuhan petugas mencuci tangan yaitu masa kerja dengan kepatuhan cuci tangan (p=0,139, PR= 2,04) dan jenis kelamin dengan kepatuhan perawat mencuci tangan (p=0,069, PR= 0,34). Simpulan yang diperoleh bahwa pengetahuan dan motivasi perawat di RSU Surya Husadha sangat signifikan terhadap kepatuhan perawat melakukan cuci tangan. Saran peneliti agar edukasi, bimbingan dan pengawasan kepada perawat tentang pelaksanaan cuci tangan terus ditingkatkan agar kepatuhan cuci tangan semakin baik sehingga kejadian infeksi rumah sakit dapat dicegah. Kata kunci: Karakteristik, pengetahuan, motivasi, kepatuhan mencuci tangan. ABSTRACT Nurse compliance in nurses washing their hands is one of the factors that has a major influence on the prevention of hospital infections. Nurses have a very large contribution to the spread of hospital infections because nurses interact directly with patients for 24 hours. Failure to carry out hand washing prior to contact with patients and during patient care, can cause hospital infections. In this study aims to determine the relationship between characteristics, knowledge and motivation of nurses with compliance to wash hands. This research is a descriptive study using cross-sectional design. Respondents used were 97 respondents with proportionated stratified random sampling technique. The results showed that there was a significant relationship between characteristics (age) with hand washing compliance (p = 0.007, PR = 3.68), level of education with hand washing compliance (p = 0.021, PR = 0.33), knowledge with compliance hand washing (p = 0.003, PR = 4.16) and motivation with hand washing compliance (p = 0.013). For variables not related to compliance with hand washing officers, namely the length of service with compliance with washing hands (p = 0.139, PR = 2.04) and gender with compliance with nurses washing hands (p = 0.069, PR = 0.34). Conclusions obtained that the knowledge and motivation of nurses at Surya Husadha General Hospital are very significant to the compliance of nurses doing hand washing. Researchers suggest that education, guidance and supervision of nurses on the implementation of hand washing continue to be improved so that compliance with hand washing is better so that the incidence of hospital infections can be prevented. Abstract

ABSTRAKRSUD Kabupaten Klungkung merupakan salah satu rumah sakit yang belum dapat mencapai indikator mutu kelengkapan rekam medis 24 jam setelah selesai pelayanan. Kelengkapan, keakuratan, dan ketepatan rekam medis merupakan tanggungjawab utama dokter. Perilaku dokter dalam penyelesaian pengisian rekam medis salah satunya dapat dipengaruhi oleh kondisi kerja tempatnya bertugas. Penelitian ini bertujuan untuk mengetahui faktor-faktor kondisi kerja yang melatarbelakangi keterlambatan penyelesaian pengisian dokumen rekam medis oleh dokter di Instalasi Rawat Inap RSUD Kabupaten Klungkung. Penelitian ini menggunakan metode deskriptif kualitatif. Pengumpulan data menggunakan metode wawancara mendalam dan studi dokumentasi yang dilakukan di instalasi rawat inap RSUD Kabupaten Klungkung pada bulan April-Mei 2017. Informan pberjumlah delapan orang yang terdiri dari dokter spesialis dan manajemen rumah sakit. Hasil penelitian ini menunjukan bahwa beban kerja yang cukup tinggi, belum efektifnya supervisi, belum maksimal pengetahuan dokter terkait rekam medis, belum terbiasanya dokter menggunakan formulir rekam medis yang baru, serta belum efektifnya pelaksanaan sosialisasi melatarbelakangi keterlambatan penyelesaian pengisian rekam medis oleh dokter. Selain itu, faktor diluar kondisi kerja yang melatarbelakangi keterlambatan penyelesaian pengisian yaitu kebijakan hari libur dan kepulangan pasien diluar jam kerja dokter spesialis. Simpulan penelitian ini diketahui bahwa faktor kondisi kerja yang belum optimal di RSUD Kabupaten Klungkung melatarbelakangi terjadinya keterlambatan penyelesaian pengisian dokumen rekam medis oleh dokter spesialis di instalasi rawat inap. Maka perlu adanya perbaikan pada faktor kondisi kerja yang ada di RSUD Kabupaten Klungkung.Kata Kunci : Dokter, Kondisi Kerja, Rekam Medis. ABSTRACTKlungkung Public General Hospital has not able to achieve one of the quality indicators namely completeness of patients’ medical records after 24 hours of the patient care is finalized. The completeness and validity of the medical record is the responsibility of the physician. This study aimed at identifying factors related to the physicians’ performance in filling up the medical records. This study used a qualitative method with in-depth interviews conducted among eight specialized physician and administrative staff working at in-patient wards of Klungkung Public General Hospital from April to May 2017. The study results showed two groups of factors influencing the performance of physicians in filling up the medical records. The first was internal working condition factors consist of a high work-load, lack of socialization and supervision, lack of knowledge related to the medical records management, and problems with the new forms. The second was external factors such as off-day policy as well as the time difference between patients’ discharge and physician working hours. The study conclusion is that suboptimal working conditions influence the completeness of patients’ medical records. A human resources strategy to optimize working condition particularly for physicians, is needed.Keywords: Doctor, Work Condition, Medical Record

Abstract

Pengabdian masyarakat berupa pelatihan ini bertujuan untuk meningkatkan pengetahuan danketerampilan staf Dinkes dan Puskesmas di Gianyar dalam merencanakan kebutuhan SDM denganmetode WISN. Pelatihan WISN diselenggarakan pada tanggal 12 Juni 2015 di Gedung PSKM FKUnud. Sebelum pelatihan dimulai, dilakukan instalasi software WISN pada seluruh laptop peserta.Kegiatan pelatihan terdiri dari pemberian materi, diskusi, serta praktik penggunaan software WISN.Untuk evaluasi, seluruh peserta diminta untuk mengisi kuesioner pre dan post test. Nilai pre dan posttest dianalisis secara statistik dengan uji t dua sampel berpasangan untuk melihat ada tidaknyaperbedaan pengetahuan. Pelatihan dihadiri oleh 25 orang peserta (89,3%) dari Dinkes Gianyar dan13 Puskesmas di Kabupaten Gianyar. Saat pelatihan, seluruh peserta telah mendapatkan danmengetahui cara instalasi software WISN. Terdapat peningkatan pengetahuan peserta pelatihansebelum dan sesudah mengikuti pelatihan WISN sebesar 5,08 dengan nilai p <0.001. Seluruh pesertadapat melakukan analisis dengan software WISN dengan panduan tim pelaksana dan menyatakanakan menerapkan metode WISN di institusi mereka. Pelatihan telah berjalan dengan baik danberhasil meningkatkan pengetahuan dan keterampilan petugas Dinkes dan Puskesmas di Gianyardalam menggunakan software WISN. Bimbingan teknis dan follow up perlu dilakukan untukmendorong implementasi metode WISN untuk perencanaan kebutuhan SDM di Gianyar.

Abstract

ABSTRACT Midwifery as paramedic in a village plays an important role in primary health care in order to pursue Indonesia Sehat 2010. Midwifery should provide good services with adequate tariff for society. As one indicator of service, the use of unit cost method has become an important part in tariff calculation. It was found that knowledge of the local Midwifery about unit cost was quite low. The activity of community services was carried out at IKK-IKP department Faculty of Medicine, on 01st September 2007. This activity aimed to improve knowledge and ability of midwifery of how to use Unit Cost in tariff calculation. The method was practicing on calculate several health care services cost. The result of this activity was midwifery can create adequate tariff base on unit cost.

Abstract

Background and purpose: The application of patient safety in hospitals is influenced role of HR service providers. HR has the largest population of nearly 40% compared to other workers and have a period of direct contact with patients is a nurse. This study aims to determine the relationship between nurse motivation and commitment to work with the application of patient safety.Methods: This research is observational study with cross sectional approach. The sample consisted of 112 nurse selected by total sampling.Results: Based on the Chi-Square Tests, correlation nurse motivation and nurse commitment to work with implementation of patient safety obtained p value of 0.000 means there is a relationship between nurse motivation and nurse commitment to work with implementation of patient safety. Conclusion:Motivation and commitment to work and nurse characteristics (age, education, marital status, employment status, length of employment) is jointly associated with the implementation of patient safety. Factors working commitment of the most dominant influence on the application of patient safety compared with the motivational factors of nurses and nurse characteristics, so the need for efforts to increase the commitment of nurses through socialization associated with understanding the vision and mission of the hospital as well as the achievement of the performance indicators of good nurses by increasing the quality of nursing through education formal or informal ongoing basis, increased motivation of nurses, especially in terms of adjustment of incentives, leadership and commitment to create a situation conducive work. Abstract

Background: Needlestick and sharp injuries (NSIs) remain one of the main occupational hazards among health care workers (HCWs) that lead to blood-borne pathogen exposure. Purpose: This study examined the risk factors for NSIs among HCWs at a tertiary referral hospital in Bali. Methods: A case-control study was conducted at Sanglah General Hospital, Bali, involving 171 HCWs, divided into 81 cases and 90 control subjects. The sample was selected using a simple random sampling technique. Data were collected using questionnaires and analyzed using multiple logistic regression. Results: The majority of respondents had experienced NSIs once, in the morning shift, and had affected an individual’s hands. The most frequent activity leading to NSIs was giving an injection and most of the situations causing NSIs involved accidental self-action, and recapping needles. Factors associated with the incidence of NSIs were work career or experience of <5 years (p < 0.01; adjusted odds ratio [AOR]=5.04; 95% confidence interval [CI] = 2.04–12.42), non-compliance with working procedures (p = 0.01; AOR = 2.47; 95%CI= 1.26–4.82), female HCWs (p = 0.03; AOR = 2.21; 95% CI = 1.01–4.55), and unsafe workplace conditions (p = 0.04; AOR = 2.23; 95% CI = 1.01–4.92). Conclusion: Personal and environmental factors such as gender, work experience, compliance with working procedures, and workplace conditions are among the risk factors for NSIs. Occupational health and safety policies and programs focusing on the relevant factors, accompanied by adequate supervision, are necessary to reduce the risk of NSIs occurring.

Abstract

The Workload Indicators of Staffing Need (WISN) software application training aims toimprove the knowledge and skill of Karangasem Regional Public Hospital (RSUD Karangasem) staff in undertaking workload-based human resources planning analysis using WISN software. The training was conducted in 24 September 2016 at RSUD Karangasem. Fifty seven managerial staff of RSUD Karangasem participated in the training. The training consist of WISN software installation, presentation, discussion, and practices on how to use WISN software. The trainees were asked to fill in pretest and posttest questionaires. The test scores were analyzed statistically using paired t-test. There was knowledge improvement of 6,21 after the training with p value < 0.001. All trainees were also able to conduct analysis using WISN software under the guidance of the trainers. The training has been well performed and successfully improved the knowledge and skill of RSUD Karangasem staff in applying WISN method. They stated their willingness to apply WISN method to their institution. However, further technical guidance is required to set up the system for the implementation of WISN method on human resource planning in RSUD Karangasem.

Abstract

Background: As aging, there is also a degenerative process in the function of the body, especially teeth and mouth. One of this problem is tooth loss that can affects the function of the oral cavity. Aim: The purpose of this study is to know the factors related to the use of denture, among elderly people in Selemadeg. Method: Descriptive study used with cross sectional study, that be done in Selemadeg with 70 samples, obtained by simple random sampling. Data of this research obtained from interviews with the questionnaire. Data were tested using Chi-square test (p<0,05). Results: The result of this study showed the use of denture on elderly people in Selemadeg village are 12 people (17,1%). The use of denture based on economic status is above UMK (75%), with p value <0,001. Based on knowledge is the majority of respondents have a high level of knowledge (91,7%), with p value <0,001. Based on the experience is most of respondents had a good experience (58,3%), with p value <0,001. Conclusion: It was concluded that the use of denture among elderly people in Selemadeg village tends to be low, and based on economic status, knowledge, experience, and availability of time had statistically significant trend.
