Abstract

Latar belakang: Karsinoma nasofaring (KNF) adalah suatu karsinoma sel skuamosa yang berasal dari sel epitel nasofaring. Cyclooxygenase (COX) merupakan enzim pada jalur biosintetik dari prostaglandin, tromboksan dan prostasiklin dari asam arakhidonat. Ekspresi seluler COX-2 meningkat di atas normal pada stadium awal karsinogenesis dan selama perkembangan serta pertumbuhan invasif tumor. TNF-α diperlukan untuk proliferasi dan fungsi yang normal dari NK cells, sel T, sel B, makrofag dan sel dendrit. Akan tetapi bukti yang terbarumenunjukkan bahwa TNF-α merupakan suatu mediator utama pada cancer-related inflammation dan juga berperan sebagai tumour-promoting factor.Tujuan: Untuk mengetahui ekspresi COX-2 dan TNF-α sebagai mediator cancer-related inflammation pada KNF. Metode: Penelitian ini bersifat deskriptif secara cross sectional design dengan 30 sampel dilakukan di RSUP. H. Adam Malik Medan. Ekspresi COX-2 dan TNF-α pada KNF diperiksa dengan immunohistokimia. Hasil: Dengan menggunakan Fisher’s exact test tidak ditemukan hubungan yang signifikan antara ukuran tumor primer, ukuran kelenjar getah bening serta stadium klinis dengan ekspresi COX-2 (p > 0,05). Ditemukan hubungan yang signifikan antara ukuran tumor primer dan ekspresi TNF-α (p < 0,05). Tidak ditemukan adanya hubungan antara ukuran kelenjar getah bening dan stadium klinis dengan ekspresi TNF-α (p >0,05). Dengan uji Spearman’s rho didapatkan bahwa COX-2 berkorelasi signifikan dengan TNF-α pada KNF (p <0,05). Kesimpulan: Terdapat korelasi signifikan ekspresi COX-2 dan TNF-α pada KNF.Kata kunci: Karsinoma nasofaring, cyclooxygenase-2, TNF-α.

Abstract

BACKGROUND: Saliva has been suggested as a substitute of serum for the detection of 25 Dihydroxyvitamin D (25(OH)D) in healthy people. However, investigation of salivary 1,25(OH)D has not been clearly reported. Vitamin plays important roles in inhibiting cancer progression. Current study was conducted to investigate serumal and salivary 25(OH)D) and 1,25(OH)D levels of healthy and head and neck cancer (HNC) subjects.METHODS: Research were conducted at Haji Adam Malik Hospital, Medan, Indonesia. Forty HNC and 40 healthy subjects were recruited and selected based on inclusion and exclusion criteria. Medical records were documented, followed by anthropometric evaluation and serum and saliva collection. Laboratory investigation for 25(OH)D and 1,25(OH) was performed using Enzyme-linked immunosorbent assay (ELISA) methods.RESULTS: Significant serumal (p=0.002) and salivary (p=0.016) 25(OH)D mean level differences of HNC and normal groups were obtained. More serumal or salivary 25(OH)D deficient subjects were found in control group than those in HNC group. Meanwhile, serumal and salivary 1,25(OH)D mean levels of HNC group were not significantly different with the ones of control group. There were significant correlations of serumal-salivary 25(OH)D as well as serumal-salivary 1,25(OH)D levels in normal group.CONCLUSION: Serumal and salivary 25(OH)D and 1,25(OH)D levels of HNC group were relatively normal. Salivary 25(OH)D and 1,25(OH)D could be suggested as substitutes for serumal ones.KEYWORDS: vitamin D, 25(OH)D, 1,25(OH)D, head and neck cancer

Abstract

Introduction: Polymorphism of the enzyme in metabolyzing carcinogen such as CYP2E1 and GSTs are the genetic factor contributing for NPC. NPC also is related with several proteins that induce the growth and progression of tumor. Identification of the genetic variation and protein are beneficial for clinical use. Review: CYP2E1 polymorphisms are associated with increased activity and enzyme transcription, thereby increasing the activation of nitrosamines as carcinogens in NPC. In addition, polymorphism of GSTs as the phase II enzyme decrease the act to detoxify carcinogen and inhibit oxidative stress. Several studies showed the relation of those polymorphism with the risk factor of NPC. The protein expression in NPC was also studied by several researchers. Several proteins in NPC such as p38 MPAK, TNF-α, NF- κB, PPAR-gamma, LMP-1, VEGF, COX-2, and MMP-9 are related with tumor growth, prognosis and also help in treatment of NPC. Conclusion: Identifying GSTs and CYP2E1 polymorphism may be help in determining risk factor for NPC due to the association of those with increased susceptibility for NPC. Analyzing the protein expression by immunohistochemistry also help the clinician to identify the prognosis and considering therapy for NPC.

Abstract

Introduction: Nasopharyngeal carcinoma (NPC) is a type of squamous cell carcinoma that develops from the nasopharyngeal epithelial cells and has become one among the most popular frequent malignancy in the head and neck. When patients with cancer are diagnosed with COVID-19, more intense surveillance or treatment should be considered. Discussion: In the COVID 19 pandemic, Personal protection provisions should be strengthened. created for cancer patients and cancer survivors. To avoid the spread of COVID-19, many improvements have been made in the treatment of NPC patients at the Haji Adam Malik General Hospital in Medan. Conclusion: The management carried out on NPC patients, there are several examinations that the patient needs to do first in the COVID 19 pandemic at the General Hospital Haji Adam Malik Medan

Abstract

Introduction: Nasopharyngeal carcinoma (NPC) is one kind of cancer that is the most common type of head and neck malignancy, arising from the posterior and lateral parts of the nasopharynx especially from torus tubarius or pharyngeal recess or the other name is Rosenmüller fossa. This shows significant differences from other head and neck cancers, clinical behavior, cause, epidemiology, treatment, as well as prognosis. Clinical trials of COVID-19 vaccines with better follow-up will provide useful data on vaccine advantages in patients receiving cancer treatment, due to the absence of complete data to determine the relationship between active oncology treatment and the power to induce immunity to COVID-19 using the vaccine. Discussion: The primary aim of vaccines is to produce stimulation by the antigen we desire in the presence of sufficient infection to elicit memory immunity with tolerable protection.[15] Due to the increasing frailty of cancer patients with infection as well as deaths from COVID-19, there is an urgent need to vaccinate patients with COVID-19 cancer. Various considerations for the protection and benefits that are expected to have an effect with treatment due to their standart management and changes in the immune system of patients with malignancies. Conclusion The COVID-19 vaccine, granted approved or permitted, will be considered the basic of prevention of this COVID-19 pandemic for patients with nasopharyngeal carcinoma. There are advantages and disadvantages about Covid-19 vaccine. Then, a quality direction on clinical trials of patients with cancer and COVID-19 vaccines becomes very attractive. Finally, comprehensive oncological care requires cancer patients to be prioritized for the COVID-19 vaccine

Abstract

Introduction: Nasopharyngeal carcinoma (NPC) is in the fourth place according to the most most common malignancies in Indonesians. Three types of nasopharyngeal carcinoma are squamous cell, non-keratinizing, and undifferentiated carcinoma. Case report: A 51-year-old man came to the ENT-Head and Neck Surgery Outpatient clinic of General Hospital Haji Adam Malik Medan on November 2, 2020 with main complaint of lump on the neck and nasal congestion. The patient underwent a contrast-enhanced nasopharyngeal MSCT scan with the result: T2N0M0. The histopathology results: Non-Keratinizing Squamous Cell Carcinoma. The patient was given 70 gy of Chemoradiation (CRT) followed by adjuvant chemotherapy with Cisplatin + 5 Fu.  Conclusion: There was no visible mass on the nasopharynx after using this combination therapy

Abstract

Karsinoma nasofaring (KNF) merupakan keganasan yang terjadi pada epitel nasofaring. Sebagian besar lokasi penderita KNF terpusat pada benua Asia dan memiliki garis keturunan yang berasal dari Cina bagian Selatan, terutama yang merupakan bagian dari suku Fujian dan Kantonesia. Penggunaan anti-VCA mulai dikenalsebagai salah satu penanda uji tapis. Penelitian ini bertujuan untuk mengetahui hubungan nilai serologi IgA anti-VCA dengan gambaran histopatologi KNF tanpa menilai faktor risiko lainnya. Jenis penelitian yang digunakan berupa analitik observasional dengan desain potong lintang. Sampel diambil secara consecutive sampling  dari penderita KNFyang memenuhi kriteria inklusi dan eksklusi sejumlah 33 orang. Hasil penelitian menunjukkan dari 33 sampel, didapati 22 orang sampel berjenis kelamin laki-laki (66,7%), dengan kelompok usia 50-59 tahun sejumlah 12 orang (36,4%) dan 14 orang sampel berprofesi sebagai wiraswasta (42,4%). Sebagian besar sampel memiliki gambaran histopatologi tipe II sebanyak 21 orang (63,6%), tipe III sebanyak 12 orang (36,4%) dan tidak dijumpainya gambaran histopatologi tipe I. Pada pemeriksaan ELISA IgA anti-VCA EBV ditemukan immunoglobulin tersebut pada 22 orang sampel (66,7%). Analisis chi-square menunjukkan hubungan antara nilai serologi IgA anti-VCA EBV dengan gambaran histopatologi KNF dengan nilai p0,002
