Abstract

Latar belakang: Karsinoma nasofaring (KNF) adalah suatu karsinoma sel skuamosa yang berasal dari sel epitel nasofaring. Cyclooxygenase (COX) merupakan enzim pada jalur biosintetik dari prostaglandin, tromboksan dan prostasiklin dari asam arakhidonat. Ekspresi seluler COX-2 meningkat di atas normal pada stadium awal karsinogenesis dan selama perkembangan serta pertumbuhan invasif tumor. TNF-α diperlukan untuk proliferasi dan fungsi yang normal dari NK cells, sel T, sel B, makrofag dan sel dendrit. Akan tetapi bukti yang terbarumenunjukkan bahwa TNF-α merupakan suatu mediator utama pada cancer-related inflammation dan juga berperan sebagai tumour-promoting factor.Tujuan: Untuk mengetahui ekspresi COX-2 dan TNF-α sebagai mediator cancer-related inflammation pada KNF. Metode: Penelitian ini bersifat deskriptif secara cross sectional design dengan 30 sampel dilakukan di RSUP. H. Adam Malik Medan. Ekspresi COX-2 dan TNF-α pada KNF diperiksa dengan immunohistokimia. Hasil: Dengan menggunakan Fisher’s exact test tidak ditemukan hubungan yang signifikan antara ukuran tumor primer, ukuran kelenjar getah bening serta stadium klinis dengan ekspresi COX-2 (p > 0,05). Ditemukan hubungan yang signifikan antara ukuran tumor primer dan ekspresi TNF-α (p < 0,05). Tidak ditemukan adanya hubungan antara ukuran kelenjar getah bening dan stadium klinis dengan ekspresi TNF-α (p >0,05). Dengan uji Spearman’s rho didapatkan bahwa COX-2 berkorelasi signifikan dengan TNF-α pada KNF (p <0,05). Kesimpulan: Terdapat korelasi signifikan ekspresi COX-2 dan TNF-α pada KNF.Kata kunci: Karsinoma nasofaring, cyclooxygenase-2, TNF-α.

Abstract

Latar belakang: Rinitis alergi merupakan masalah kesehatan global dengan prevalensi yang terusmeningkat serta dapat berdampak pada penurunan kualitas hidup penderitanya. Ko-morbiditas rinitis alergi salahsatunya adalah otitis media yang sangat erat hubungannya dengan gangguan fungsi tuba Eustachius yangberkaitan dengan tekanan telinga tengah. Tujuan: Mengetahui hubungan rinitis alergi dengan disfungsi tubaEustachius dengan menggunakan timpanometri. Metode: Satu studi kasus-kontrol. Disfungsi tuba ditentukandengan pemeriksaan timpanometri (MEP negatif/<-25 daPa) dan hasil tes fungsi tuba (ETF) yang tidak baik.Analisis hasil dengan uji Chi-square dan regresi logistik. Hasil: Penelitian ini melibatkan 60 sampel. Tipetimpanogram terbanyak pada kedua kelompok yaitu tipe A, dimana kelompok RA tipe A sebesar 73,3% (rerataMEP -40,8 daPa), tipe As 10%, tipe Ad dan tipe B masing-masing 3,3%, tipe C 10%. Kelompok kontrol tipe Asejumlah 93,3% dan tipe As 6,7%. Pada kelompok kasus 83,3% hasil tes fungsi tuba abnormal dan 16,7%normal, sedangkan kelompok kontrol yaitu 93,3% normal dan 6,7% abnormal. Uji chi-square menunjukkanperbedaan yang signifikan antara klasifikasi RA dan disfungsi tuba Eustachius (p=0,006) dan perbedaan yangsignifikan antara jenis alergen dan disfungsi tuba Eustachius (p=0,041). Hasil uji multivariat terdapat bahwa tiperinitis alergi berpengaruh tiga kali lebih besar terhadap disfungsi tuba Eustachius (OR 2,9; 95% IK: 1,52-241,90dan nilai p=0,022) sedangkan jenis alergen dua kali lebih berpengaruh terhadap disfungsi tuba dengan OR 2,2;95% IK: 0,01-7,31 dan nilai p=0,295. Kesimpulan: Rinitis alergi memiliki kemungkinan 3 kali lebih seringmengalami disfungsi tuba Eustachius dari pada kelompok kontrol OR:2,5 (95% CI 2,36-2,99) P=0,000.Kata kunci: rinitis alergi, disfungsi tuba Eustachius, rerata tekanan telinga.ABSTRACTBackground: Allergic rhinitis is a global health problem with increasing prevalence and can have impacton the quality of life of sufferers. One of co-morbidity of allergic rhinitis is otitis media which is closely relatedto Eustachian tube dysfunction, associated with middle ear pressure. Purpose: To determine the relationshipbetween allergic rhinitis and Eustachian tube dysfunction, using tympanometry. Methods: A case-control study.Tubal dysfunction was determined with tympanometry (MEP negative / <-25 Dapa) and tubal function test resultwas not good (≤15 daPa). Chi-square test and logistic regression analysis used for data analysis. Results: Therewere 60 samples. The most common tympanogram type in both groups were type A, which AR group was 73.3%(mean -40.8 daPa MEP), type As 10%, type ad and type B respectively 3.3%, type C 10.0% and control groupstype A 93.3%, type As 6.7%. In AR group, 83.3% showed abnormal tubal function and 16.7% was normal. As incontrol group, 93.3% was normal and 6.7% was abnormal. Chi-square test showed a significant differencebetween AR classification and Eustachian tube dysfunction (p = 0.006), and also between type of allergens andEustachian tube dysfunction (p= 0.0041). Multivariate test revealed AR classification contributed to Eustachiantube dysfunction OR:2.9 (95% CI 1.52-241.90) p=0.022, not between type of allergens and Eustachian tubedysfunction OR:2.2 (95% CI 0.01-7.31) p=0.295. Conclusion: Allergic rhinitis contributes three times higher toEustachian tube dysfunction than the control group. Abstract

Indonesia is one of the few countries in the world with very high burden of tuberculosis (TB). Tubercular pleural effusion (Pleural TB) is the most common form of extra-pulmonary TB, however the remains a common clinical challenge. This research from May 2017 through September 2017 a total of 42 patients with exudative lymphocytic pleural effusions suspected to pleural tuberculosis were enrolled in this study. Adenosine deaminase (ADA) and Gene X-pert were examined from pleural fluid. Diagnosis was made clinically or based from sputum/pleural fluid culture. The result of this research showed out of all participants, 64,3% (27/42) had ADA in positive level (>40 IU/L) and 31% (13/24) had Gene X-pert positive Mycobacterium Tb. The Gene X-pert sensitivity was 40.7% with specificity 86.6%, PPV and NPV were 44.8% and 57.1% respectively. Kappa Compatibility study show that Gene X-pert point was 0.227 (p 0.066). The usefulness of Gene X-pert to diagnose pleural TB is limited by its poor sensitivity. Gene X-pert cannot be used as single diagnostic tool for tuberculous pleural effusion

Abstract

Background: Cases of multidrug resistant tuberculosis (MDR TB) is increasing in number in the world and requires early detection to prevent further transmission. GeneXpert MTB/RIF is a tool that can be used for detection of rifampicin resistance, as a surrogate marker for MDR TB. This study aims to assess the sensitivity and specificity of the GeneXpert MTB/RIF in diagnosis of MDR TB. Methods: diagnostic test study was conducted at a poly MDR TB General Hospital Haji Adam Malik Medan. The subjects were all suspected MDR TB who had results positive GeneXpert MTB/ RIF with sensitive rifampin or resistant  rifampin and had a drug sensitivity test results with the proportion method Lowenstein Jensen medium. Data retrieved from the medical records, between January until December 2013. Results: founded 64 samples that had results of GeneXpert MTB/RIF test positive and had the results of drug sensitivity, 87.5% of rifampin-resistant samples were also resistant to isoniazid. The GeneXpert MTB/RIF examination showed the sensitivity of 92.86% and the specificitu of 59.09%. Conclusion: GeneXpert MTB/ RIF has a high sensitivity for diagnosing MDR TB compared the gold standard drug sensitivity testing proportion method on Lowenstein Jensen medium. Abstract

Background: The relationship between diabetes mellitus (DM) and tuberculosis (TB) is a significant health problem and more prominent in developing countries where TB is endemic and DM prevalence increases. The prevalence of pulmonary TB increases with increasing prevalence of DM. Uncontrolled diabetic patients with high hemoglobin (HbA1c) levels cause TB to become more severe and are associated with higher mortality and also have a significant effect on radiological manifestations of pulmonary TB. Methods: The was a case-control study conducted between January to December 2016 at RSUP H Adam Malik, Medan to 43 with TB-DM patients and 41 TB-without DM patients was studied from. Radiological examination with chest radiograph was done in both groups of samples. The HbA1c levels was examined in the TB with DM group. Abstract

Background: Workers in the ceramics industry are often exposed to silica content which is unwittingly inhaled and deposited in the lungs. Macrophages will release Interleukin 8 (IL-8), a chemoattractant that causes neutrophil recruitment to the alveoli and releases proteolytic enzymes that damage the lung parenchyma and cause a decrease in lung function. This study aimed to determine whether dust level correlate with IL-8 serum in ceramic industry workers. Method: This research is an analytic study with a cross-sectional design conducted in March–June 2019 in the X Ceramic Industry in Mabar, Medan. Personal Dust Sampler was used to measure dust level of the study subjects at work sites. Lung function was measured by spirometry. Statistical analysis was performed using Statistical Package for the Social Sciences (SPSS) software. Results: A total of 35 male workers were divided into 3 working sections, 11 (31.4%) in the pre-compression section, 13 (37.1%) in the compression section and 11 (31.4%) in the sintering section. Dust levels at each working section were 24.8, 29.2, and 6.11, respectively. The lung function examination showed restrictive impairment in 21 subjects (60%). Statistical analysis showed that the higher the level of dust in the workplace, the lower the value of Forced Expiratory Volume 1 (FEV1) and Forced Vital Capacity (FVC) values, although this correlation was not statistically significant (r = -0.03 and -0.22 respectively; p> 0.05). Conclusion: There was no significant relationship between workplace dust levels and lung function in ceramic workers.

Abstract

Background: Delayed in diagnosis and treatment of tuberculosis will increase the risk of MDR TB. WHO recommends Xpert MTB/RIF as diagnostic tools to identify MDR TB. Availability of Xpert MTB/RIF is still limited, another diagnostic tool is needed. Neutrophyl lymphocyte ratio (NLR) was presumed to be able to identify the probability of MDR TB. The aim of this study is to evaluate the comparison of NLR in tuberculosis and MDR TB patients. Methods: This is an analytic descriptive study with case series approach in Adam Malik General Hospital Medan. This study held since January – December 2015 with number of sample as much as 100 bacteriological confirmed TB patients and 100 MDR TB patients. We performed leukocyte differential count from peripheral blood examination to obtain NLR Result: Mean NLR of TB patient 4.62±2.37 and MDR TB 3.28±1.44. There was significant difference of NLR between both groups using Mann-Whitney test (P=0.001). The cut off value by ROC analysis was 2.91 with sensitivity, specificity, positive predictive value, negative predictive value and accuracy was 77%, 50%, 60.6%, 68.4% and 63.5% respectively. Conclusion: There was significant difference of NLR between bacteriological confirmed TB patients and MDR TB patients. Value of NLR

Abstract

Background: Tuberculous pleural effusion is an accumulation of fluid in the pleural cavity produced by Mycobacterium tuberculosis (MTB). The gold standard of TB pleural effusion diagnosis is to obtain TB bacilli in pleural fluid or pleural tissue. However, this is often constrained due to the low identification level of these bacilli and the slow growth of MTB cultures. This study aimed to assess the pleural fluid leukocyte level in establishing a diagnosis of pleural effusion caused by TB. Methods: This was a diagnostic study conducted on 111 patients with pleural effusion, caused by TB, malignancy or non-TB infections that were assigned by supporting examinations obtained from medical records, which then assessed for pleural fluid leukocytes. Statistical analysis was performed using Kruskal Wallis Test and Receiver Operating Characteristic (ROC) curve to attain the cut-off point of pleural fluid leukocyte level. Results: Pleural fluid leukocyte levels in TB cases were significantly different when compared to pleural effusion caused by malignancy and non-TB infections (P<0.001). Pleural fluid leukocyte level ≥1100 cell/mm3 was a cut-off diagnostic test for tuberculous pleural effusion with a sensitivity of 77% and specificity of 60.3%. Conclusion: Pleural fluid leukocyte level ≥1100 cell/mm3 could assist in diagnosing tuberculous pleural effusion.

Abstract

Backgrounds: Tuberculosis (TB) is still a public health problem in the world.Vitamin C is a water-soluble micronutrient, protects the host from reactive oxygen and reactive nitrogen intermediates generated during mycobacterial infection, involved in the synthesis of collagen, iron transport, and acts as physiological antioxidant. Vitamin C plays role in killing Mycobacterium tuberculosis by fenton reaction. This study was to determine the effect of vitamin C supplementation on chest X-ray (CXR) improvement in pulmonary TB patients during intensive phase. Methods: This was a quasy experimental study with single blind-randomized controlled trial design. Subjects were divided into 2 groups, first group was pulmonary TB patients treated with anti tuberculosis drugs (ATD) plus vitamin C and the second group was treated with ATD plus placebo. This study was conducted in several public health center in Medan City. The subjects were pulmonary TB patients with positive AFB sputum in intensive phase from December 2017 to May 2018. Data were analized using t-test dependent. Results: From 80 total subjects, 53 males (66%) and 27 females (34%). After 2 months of treatment, improvements in CXR images were found in the ATD plus vitamin C group, about 21 subjects (52.5%) had no more lesions on CXR, while the same condition was found on 15 subjects (37.5%) in the placebo group. Conclusion: Vitamin C supplementation in pulmonary TB provided better improvements of CXR images. (J Respir Indo. 2020; 40(2): 82-7)
