Abstract

Sepsis neonatorum merupakan penyebab utama kematian pada neonatus di Indonesia. Kondisi ini dapat memicu terjadinya disseminated intravascular coagulation (DIC), lalu meningkatkan konsumsi platelet dan mengakibatkan trombositopenia. Penelitian ini bertujuan untuk mengetahui nilai diagnostik dan prognostik jumlah dan index trombosit, mean platelet volume (MPV) dan plateletcrit (PCT) pada sepsis neonatorum. Penelitan ini adalah penelitian retrospektif menggunakan data sekunder pada neonatus terduga  sepsis yang dirawat di RSU dr. Saiful Anwar Malang. Hasil penelitian menunjukkan nilai diagnostik jumlah trombosit adalah sensitivitas 89,3%, spesifisitas 75%, sedangkan MPV sensitivitas 25% dan spesifisitas 10,7%. Nilai prognostik trombosit untuk menentukan outcome sepsis neonatorum adalah sensitivitas 55,6% dan spesifisitas 58,3%, sedangkan MPV 33,3% dan 33,3%. Kadar PCT tidak bisa digunakan baik untuk  diagnosis maupun prognosis sepsis neonatorum. Kesimpulan penelitian ini adalah jumlah trombosit mempunyai nilai diagnosis dan prognosis yang lebih baik bila dibandingkan dengan MPV pada sepsis neonatorum. Namun penegakan diagnosis dan prognosis sepsis neonatorum harus tetap ditentukan berdasar pada anamnesa baik terkait faktor risiko maupun riwayat penyakit, gambaran klinis, dan pemeriksaan penunjang. Kata kunci: jumlah dan index trombosit, sepsis neonatorum.

Abstract

Tata laksana adekuat sepsis dapat memperbaiki luaran pasien. Surviving sepsis campaign (SSC) guidelines 2012 merupakan panduan internasional tata laksana sepsis berat dan syok septik, namun implementasinya dipengaruhi oleh sumberdaya dan fasilitas kesehatan. Penelitian ini mengevaluasi implementasi SSC di rumah sakit. Studi prospektif dilakukan antara Februari-Juni 2013 pada seluruh pasien usia 1 bulan–18 tahun yang memenuhi kriteria sepsis, dengan menilai penggunaan cairan resusitasi dan obat vasoaktif, waktu pemberian antibiotika, waktu pengambilan kultur, pemberian nutrisi, penggunaan ventilator, dan angka kematian. Di antara 40 pasien, 34 merupakan kasus syok septik. Seluruh pasien syok septik mendapatkan resusitasi cairan dan obat vasoaktif. Obat vasoaktif diberikan kurang dari 6 jam pada 21 pasien. Hanya 14 pasien mendapatkan antibiotika pada jam pertama, dan hanya 6 pasien dilakukan pemeriksaan kultur darah sebelum pemberian antibiotika awal. Sebanyak 28 pasien membutuhkan ventilator, namun 13 pasien tidak bisa mendapatkannya.Hal ini meningkatkan risiko mortalitas 2,1 kali (95% IK 1,2; 3,7). Saat perawatan di unit intensif, 27 pasien mendapatkan nutrisi pada 6-24 jam pertama, namun sebagian besar (32 pasien) mendapatkan kalori kurang dari 80% dalam 48 jam pertama yang meningkatkan risiko mortalitas 3 kali (95% IK 1,1; 8,2). Mortalitas terjadi pada 24 pasien. Hal ini menunjukkan adanya hambatan pelaksanaan SSC guidelines 2012 dalam hal pemberian antibiotika, pemeriksaan kultur darah, penggunaan ventilator, dan pemberian nutrisi adekuat. Ketidaktersediaan ventilator dan terapi nutrisi inadekuat meningkatkan risiko mortalitas.Kata Kunci: Antibiotika, kultur darah, mortalita, nutrisi, sepsis, ventilator

Abstract

Decrease of lymphocyte  apoptosis is  one factor  that  leads  to  chronic airway inflammation  in  allergic asthma. Previous  studies  revealed  the  role  of  interleukin  (IL)-4  in  preventing  lymphocyte  apoptosis;  however  there  are few  studies  about  the  role  of  lymphocyte-Bax  and  its  relationship  with  IL-4  in  asthma.  The  aim  of  the  study  is  to demonstrate IL-4 role in preventing lymphocyte apoptosis via lymphocyte-Bax inactivation in bronchioli and lungs  of  asthmatic  mice.  It  is  a  randomized  control  group  experimental  study .  The  subject  was  Balb/c  mice  that categorized  into  2  groups:  asthma  and  non-asthma.  Asthma  group  was  sensitized  by  ovalbumin intraperitoneally  in  day  0  and  14,  followed  by  inhalation  every  2-3  days  for  6  weeks.  In  week  8,  all  of  mice were terminated.  The  IL-4  and  lymphocyte-Bax  expression  was  examined  through  immunohistochemistry  method, whereas  lymphocyte  apoptosis  by  TUNEL  method.  Independent  sample  t-test,  Mann  Whitney  U  test, regression  analysis,  and  path  analysis  was  used  in  statistical  analysis  with  confident  interval  95%.  The bronchioli and lungs specimens were obtained from 18 mice (9 in each group). Lymphocyte apoptosis was similar between 2 groups (p=0.1 16), additionally lymphocyte-Bax  decreased in  asthmatic group (p=0.003). This  indicated  low  activity  of  lymphocyte  apoptosis  in  asthmatic  group.

Abstract

During the Covid-19 outbreak, the Ministry of Women's Empowerment and Child Protection recorded SYMPHONY PPA data of 3,087 cases of child abuse, namely 852 cases of physical violence, 768 psychological cases, and 1,848 sexual cases as of January 1 - June 19 2020. Emotional intelligence should be developed and strengthened since age is closely related to the moral, social, personal, and further development of the child for the better. The research objective was to determine the relationship between age and parental education as well as family social interaction on the emotional intelligence of pre-school children. The study used a cross-sectional analytic observational design and used the Chi-Square method. Total sample 96 parents of students in five Kindergartens (TK) in Tanjungpadan, Belitung in September 2021. The results showed no significant relationship parents' age and emotional intelligence with the acquisition of a p value of 0.204 > 0.05 and the number chi square of 2.673, there is a significant relationship parental education and emotional intelligence with a p value of 0.000 <0.05 with a chi square number of 22.034, and family social interaction on emotional intelligence has a significant relationship p value of 0.000 <0.05 and chi value squares 36,670. There is a relationship age and parental education as well as family social interaction on the emotional intelligence of pre-school children.

Abstract

Adequate pain management is an important and common requirement in health care in pediatric intensive care units. There are many pain assessment scales in children based on the literature, but the guidelines for using pain scales as a guide in pain management have not been widely discussed. One of the most common pain interventions is to provide opioid or non-opioid analgesics. Analgesic drugs given to critically ill children can provide different anti-pain outcomes when measured by a pain scale. Therefore, this literature review will compare the effectiveness of analgesics in pain management in critically ill children.

Abstract

Introduction: Children with Congenital Heart Disease are at high risk of feeding and growth problems. Nutrition practices for children with CHD still vary widely across institutions, including breastfeeding. This study aims to conduct a critical review to compare the effects of breastfeeding versus formula on the growth of infants with congenital heart disease. Methods: The article search was conducted online using the PubMed, EBSCO, and ProQuest databases with the keywords “Congenital Heart Disease,” “Human Milk,” “Formula,” and “Growth.” Result: Two articles were obtained in the form of systematic review studies. Results of the study stated that in infants with CHD with breastfeeding compared to formula milk Weight for age score is better with breastfeeding because breast milk is easier to digest Conclusion. Breast milk has been shown to have significant benefits on the growth of infants with CHD compared to formula, especially in terms of weight-for-age z-score.
