Abstract

Salah satu cara untuk menurunkan angka kesakitan, angka kematian, membatasi penularan serta penyebaran penyakit, dan mengurangi dampak negatif yang ditimbulkan HIV/AIDS adalah dengan terapi antiretroviral, yang salah satu keberhasilannya dinilai dari perbaikan imunologis dan diketahui bahwa cluster of differentiation 4 (CD4) sebagai penanda status kesehatan sistem imun, maka pemeriksaan CD4 dapat digunakan untuk menentukan pasien yang memerlukan pengobatan profilaksis infeksi oportunistik (IO) dan terapi antiretroviral. Untuk mengetahui faktor-faktor yang mempengaruhi jumlah CD4 pada pasien HIV/AIDS yang diterapi antiretroviral. Penelitian ini menggunakan desain penelitian literature review atau kajian literatur. Sumber penelitian diambil dari beberapa database dengan kata kunci “("increase CD4" AND "decrease CD4" AND "hiv" OR "human immunodeficiency virus" AND "acquired immunodeficiency syndrome" AND "antiretroviral")”. Dari 612 artikel, terdapat 8 artikel yang sesuai dengan kriteria retriksi. Dari beberapa artikel tersebut didapatkan faktor-faktor yang mempengaruhi angka CD4 adalah jenis kelamin, usia, stadium klinis, jumlah CD4 awal, kepatuhan dalam terapi antiretroviral (yang dipengaruhi oleh beberapa faktor, seperti dukungan sosial, tingkat pendidikan, penggunaan ponsel, jenis kelamin, waktu kunjungan ke tempat pelayanan kesehatan, faktor ekonomi), jumlah viral load, ada tidaknya koinfeksi tuberkulosis (TB), dan infeksi hepatitis C (HCV). Faktor yang mempengaruhi jumlah CD4 pada pasien HIV/AIDS yang diterapi antiretroviral adalah jenis kelamin,, usia, stadium klinis, kepatuhan pasien dalam terapi antiretroviral, jumlah CD4 awal, jumlah viral load, dan ada tidaknya infeksi oportunistik (TB, HCV).

Abstract

Introduction: Diabetes mellitus (DM) is a non-communicable disease that has increased from year to year. Type 2 diabetes mellitus is not caused by lack of insulin secretion, but is caused by the failure of the body's cells to respond to the hormone insulin (insulin resistance). Insulin resistance was found to be a major contributor to atherogenic dyslipidemia. Dyslipidemia in DM risks 2 to 4 times higher than non-DM. Although dyslipidemia has a great risk for people with type 2 diabetes mellitus, this conventional risk factor only explains a portion (25%) of excess cardiovascular risk in type 2 DM. Discussion: In uncontrolled type 2 DM patients, LDL oxidation occurs faster which results from an increase in chronic blood glucose levels. Glycemic control as a determinant of DM progressivity is determined through HbA1c examination. HbA1c levels are associated with blood triglyceride levels. Meanwhile, triglyceride levels are associated with total cholesterol and HDL cholesterol levels. HbA1c levels are also associated with LDL cholesterol levels. Conclusion: There is a relationship between lipid profile and the progression of type 2 diabetes mellitus. Keywords: type 2 diabetes mellitus, dyslipidemia, HbA1c, glycemic control, lipid profile

Abstract

ABSTRACT Background: Tuberculosis (TB) is an opportunistic infection the most often found in Human Immunodeficiency Virus (HIV) infection and is the leading cause of death among Orang Dengan HIV / AIDS (ODHA). The risk of TB transmission is 26 to 31 times greater in people with HIV than people without HIV. TB-HIV patients tend to have negative acid-resistant bacteria (AFB) laboratory tests and atypical radiological images. Both of these become obstacles in determining the diagnosis and management. The heavier level of immunosuppression in TB-HIV patients as indicated by a decrease in the level of cluster of differentiation 4 (CD4), the less characteristic the radiological picture was. CD4 count is associated with clinical manifestations of HIV patients that can be seen from radiological images, so that it can be used in accelerating diagnosis and management of TB-HIV. This study aims to determine the relationship between CD4 and the radiological picture of TB-HIV patients at PKU Muhammadiyah Hospital of Surakarta. Methods: This study used a cross sectional research design and was conducted from November to December 2020 at the PKU Muhammadiyah Hospital of Surakarta. The number of research subjects as many as 30 patients who were taken with a non-probability purposive sampling technique. Retrieval of data using patient medical record data. Data were analyzed using Fisher's exact test. Results: Fisher's exact test showed no association between CD4 cell count and radiological features of TB-HIV patients (p = 1,000). Conclusion: There is no significant association between CD4 cell count and radiological features of TB-HIV patients.
