Abstract

World Health Organization (WHO) menjelaskan angka sectio caesarea meningkat secara global dan akan terus meningkat selama dekade mendatang. Tren persalinan melalui sectio caesarea juga mengalami peningkatan di beberapa provinsi di Indonesia. Di Provinsi Kepulauan Riau angka persalinan sectio caesarea mencapai 23,07% dari seluruh persalinan yang ada. Penelitian ini dilakukan untuk menganalisis faktor – faktor yang berhubungan dengan kejadian sectio caesarea di Instalasi Bedah Sentral RSUD Kota Tanjungpinang. Metode penelitian ini adalah penelitian kuantitatif dengan desain cross sectional. Sampel penelitian berjumlah 150 ibu bersalin dengan sectio caesarea yang diambil menggunakan teknik purposive sampling. Data diolah menggunakan uji chi square. Hasil analisa bivariat menunjukkan terdapat hubungan antara kejadian SC dengan Fetal Distress (p value = 0,000), kejadian SC dengan Riwayat SC (p value = 0,000), dan kejadian SC dengan Preeklampsia Berat (PEB) (p value = 0,019), sedangkan kejadian SC dengan Cephalopelvic Disproportion (CPD) tidak terdapat hubungan dengan (p value = 0,058). Diharapkan pelayanan antenatal care pada ibu hamil semakin ditingkatkan untuk mengatasi faktor resiko peningkatan kejadian sectio caesarea. Serta diperlukan penanganan yang cepat dan tepat sesuai standar prosedur operasional (SPO) dalam pelaksanaan bedah sesar cito dan elektif untuk mengurangi angka morbiditas dan mortalitas pada ibu dan bayi.

Abstract

In the field of palliative care nursing, Breaking Bad News (BBN) involves providing accurate information regarding the patient's status, which is often distressing for both the patient and their family. BBN is a collaborative endeavour including doctors and nurses. Patients frequently anticipate nurses to provide a reiteration of information previously conveyed by doctors. The phenomena are the lack of specialized training for nurses in Breaking Bad News (BBN), resulting in families frequently requesting nurses to provide further explanations on their health and treatment plan. The objective of this study was to investigate the firsthand encounter of nurses in BBN (Breaking Bad News). The approach employed qualitative phenomenology. 10 individuals were interviewed using semi-structured interviews to collect data. The Colaizzi Technique was employed for the analysis. Outcome: The study yielded four distinct themes: the collaborative efforts of nurses and doctors in BBN through therapeutic communication, the challenges faced by nurses when families lack understanding, the provision of BBN by nurses based on their professional experience, and the positive aspects observed by nurses in families. The study's findings revealed that nurses often face situations when they were required to Break Bad News (BBN), while lacking formal training in this domain. Nurses and doctors should undergo training in BBN (Breaking Bad News) to ensure they are sufficiently prepared to deliver such news to patients.BAHASA INDONESIA Dalam keperawatan paliatif, Breaking Bad News (BBN) memberikan informasi tentang kondisi pasien yang sebenarnya, yang sering kali merupakan berita buruk bagi pasien dan keluarganya. BBN merupakan peran kolaboratif antara dokter dan perawat. Fenomena yang terjadi adalah perawat tidak mendapatkan pelatihan khusus dalam menyampaikan BBN, dan perawat sering diminta oleh keluarga pasien untuk menjelaskan kembali informasi yang telah diberikan oleh dokter. Tujuan dari penelitian ini adalah untuk mengeksplorasi pengalaman perawat dalam memberikan BBN. Metode yang digunakan adalah fenomenologi kualitatif. Data dikumpulkan dengan wawancara semi-terstruktur dengan 10 partisipan. Analisis menggunakan Teknik Colaizzi. Hasil penelitian didapatkan empat tema, yaitu kolaborasi antara perawat dan dokter memberikan BBN dengan komunikasi terapeutik, perawat mengalami kesulitan saat keluarga tidak mengerti, perawat memberikan BBN berdasarkan pengalaman kerja, dan perawat melihat hal positif pada keluarga. Kesimpulan dari penelitian ini adalah perawat sering diminta untuk memberikan BBN, namun perawat belum pernah mendapatkan pelatihan khusus dalam memberikan BBN. Rekomendasi untuk perawat dan dokter agar mendapatkan pelatihan tentang BBN, sehingga perawat lebih siap dalam memberikan BBN kepada pasien.

Abstract

Cardiac arrest is one of the leading life-threatening emergencies, requiring life-saving procedures commonly known as Cardiopulmonary Resuscitation (CPR). In hospitals, nurses are the first responders to cardiac arrest cases. The success of CPR is greatly influenced by several factors that affect nurses' ability to act in emergency situations. This study aims to explore the in-depth experiences of nurses while performing CPR at X Hospital Batam. The research involved 10 critical care nurses, recruited through snowball sampling, with data saturation being achieved. In-depth semi-structured interviews were conducted, audio-recorded, and transcribed verbatim to collect the data. The data were analyzed using Colaizzi’s method of analysis. The findings of this study identified five key themes: The Important Role of Nurses during CPR, Accuracy in Therapy Administration, Nurses' Experiences of Pleasant and Unpleasant Emotions, Expectations of CPR Success, and Focusing on the Patient during Family Presence in CPR, among others. In conclusion, the study highlights the diversity of feelings, experiences, abilities, and knowledge that critical care nurses encounter while performing CPR at X Hospital Batam.

Abstract

Background: Healthcare practitioners faced psychological challenges, including stigmatization, during the COVID-19 pandemic. Both healthcare practitioners and the general population experienced stigma due to uncertainty. Aim: To understand factors associated with social stigma towards healthcare practitioners caring for COVID-19 patients. Method: A sequential mixed methods study was conducted. Quantitative data were collected first through an online survey distributed via WhatsApp to nursing directors at private hospitals, using the Indonesian version of the stigma scale. A total of 195 healthcare professionals (184 medical, 9 non-medical) participated. High-stigma participants were interviewed. Data were analyzed using descriptive statistics, ANOVA, and hierarchical regression. Thematic analysis was used for qualitative data. Results: Participants included nurses (84.5%), midwives (5.2%), doctors (5.7%), and non-medical professionals (4.6%). Probable COVID-19 status positively correlated with profession type (r=0.144, p<0.047). Stigma scores negatively correlated with COVID-19 testing type (r=-0.147, p<0.042) and working tenure (r=-0.147, p<0.041). Hierarchical regression showed that working tenure, probable status, and testing type predicted stigma. Profession type and confirmed cases did not predict stigma. Qualitative findings indicated junior nurses (1-2 years tenure) experienced stigma, especially when visiting testing facilities and having probable status. The negative correlation between tenure and stigma was consistent in qualitative data. Conclusions: Working tenure, probable COVID-19 status, and testing type are associated with stigma among healthcare practitioners. Recommendation: Future studies should use more rigorous designs and validated COVID-19-specific tools.

Abstract

Asma adalah suatu kondisi paru-paru yang kronis yang ditandai dengan sulit bernafas. Saluran pernapasan penderita asma sangat sensitif dan memberi respon yang sangat berlebihan jika mengalami rangsangan atau gangguan, angka prevalensi diseluruh dunia mengenai asma terus mengalami peningkatan. Fenomena yang ditemukan diwilayah Rumah Sakit Bakti Timah Karimun pada tahun 2023 bulan Januari – Juli di instalasi rawat jalan terus mengalami peningkatan kasus. Penelitian ini dilakukan untuk mengetahui faktor-faktor yang berhubungan dengan serangan asma pada pasien asma di instalasi rawat jalan Rumah Sakit Bakti Timah Karimun. Metode penelitian ini adalah kuantitatif dengan pendekatan cross sectional, sampel berjumlah 40 pasien rawat jalan yang berobat atau kontrol dengan diagnosa asma di Rumah Sakit Bakti Timah Karimun dipilih mengunakan teknik accidental sampling untuk menjadi responden. Data diolah dengan mengunakan uji statistik chi square. Analisa univariat hasil penelitian ini sebagian besar adalah memiliki alergi (70%), dan mendapatkan serangan asma saat kondisi cuaca dingin (80%), serta usia rata-rata responden asma adalah dewasa tengah 41-60 tahun (60%). Hasil analisa bivariat menunjukkan adanya hubungan antara faktor alergi dengan terjadinya serangan asma dengan nilai p-value (0,016), dan tidak adanya hubungan antara faktor perubahan cuaca dengan serangan asma dengan nilai p-value (0,582), serta adanya hubungan faktor usia yang mempengaruhi terjadinya serangan asma dengan nilai p-value (0,009). Dengan mengetahui berbagai macam faktor-faktor yang berhubungan dengan serangan asma, diharapakan agar penderita dapat menghindari faktor pemicu tersebut yang dapat mengakibatkan terjadinya serangan asma.

Abstract

Abstract Drowning is one the most common accident in Riau Island. Drowning can cause hypoxia, acidosis  lactate and hypothermia, this can result in cardiovascular dysfunction, ie heart rhythm disorders to cardiac arrest.  Basic life  support is needed to restore breathing and circulation fungction of a person. Marine and Air Police Directorate Riau Islands is the front line of providing assistance and rescue at sea and waters. Previously there was no proper BLS training, therefore the training is needed to prepare first responders. This study aimed to determine of Basic Life Support education to increase on knowledge, attitudes and skills member of Marine and Air Police Directorate Riau Islands. This research used a quantitative method with a research design with pre experimental design research design used with one group pre-post design without control. The total number of samples taken with purposive randomsampling of Subditpatroliairud Members and Police Boat members with 50 subjects and data analysis using Paired T-Test. Theaverage before and  after education BLS were 58,13-83,33 for knowledge, 78,77-83,33 on attitude, 29,84-79,23 on skill. Paired T-Test on  knowledge, attitudes and skills with a p-value of 0,000 (p-value<0,05) therefore Ha is accepted. Therefore, there is an effecton BLS training on improving knowledge, attitudes and skills member of Marine and Air Police Directorate.
