Abstract

Hospital-acquired infections (HAI) adalah infeksi yang terjadi pada pelayanan kesehatan selama menjalani  prosedur perawatan dan tindakan medis. HAI menyebabkan pemanjangan lama rawat inap, sehingga merugikan pasien dan meningkatkan biaya perawatan. Cara paling efektif untuk membatasi penyebaran dari HAI adalah melaksanakan hand hygiene (HH) sesuai aturan. Hasil studi pendahuluan di IGD RSUD Dr. Iskak Tulungagung ditemukan bahwakepatuhan hand hygiene perawat IGD hanya sebesar 30%. Tujuan dari penelitian ini adalah untuk mencari faktor-faktor yang mempengaruhi kepatuhan hand hygiene perawat IGD dan mencari solusi untuk meningkatkan tingkat kepatuhannya. Hasil penelitian menunjukkan rerata kepatuhan HH pada perawat sebesar 36% dengan kepatuhan tertinggi pada sebelum tindakan aseptis (50%) dan terendah pada setelah menyentuh sekitar pasien (20%) bahwa faktor-faktor yang paling mempengaruhi kepatuhan hand hygiene perawat IGD adalah pengetahuan, fasilitas dan pengingat/role model. Solusi yang disepakati untuk meningkatkan kepatuhan adalah meningkatkan pengetahuan dengan cara memberi pembuktian efektifitas hand hygiene dalam mengurangi jumlah bakteri di tangan melalui pemeriksaan agar gel. Diharapkan agar solusi yang dipilih ditunjang dengan peningkatan fasilitas dan pelaksanaan sistem pengingat melalui poster atau role model dapat meningkatkan kepatuhan hand hygiene dapat meningkat secara optimal.Kata Kunci: Hand hygiene, kepatuhan, role model

Abstract

Outpatient services often become the first gateway for patient access to hospital services. One of the minimum service standards for improving the quality of hospital services is collecting the medical record files in less than 10 minutes. This study aimed to prove whether the frontliners can help cut the process in the outpatient registration by revising the SOP and describing the outline of the outpatient service flow involving the frontliners at Ananda Srengat Hospital. The study applied quantitative analysis methods with a descriptive analysis technique to patients, medical records unit employees, outpatients, and related units. A total population of 10 people was taken within one week. The methods of collecting data were using a questionnaire instrument and a stopwatch. The results of this study are the changes in the SOP and the description of frontliner tasks in assisting the process of sorting files that can reduce the process of registration waiting time.

Abstract

Hospital-acquired infections (HAI) are infections that occur in healthcare while undergoing treatment procedures and medical treatment. HAI causes elongation of hospitalization, do harm to the patient and increases maintenance costs. The most effective way to limit the spread of HAI is to implement hand hygiene by the rules. The results of preliminary study in Ananda Hospital Srengat Blitar found that nosocomial infection rates is high (infusion phlebitis incident is 12.4%) and nurses hand hygiene compliance is low (hand hygiene compliance is 26%). The purpose of this study was to find the relationship between the completeness of handrub and posters on the nurses's intention to change to perform hand hygiene correctly. The research method is use the design explanatory research with cross sectional approach. The results showed that the completeness of handrub clinically significant influence intention to change to perform hand hygiene (P = 0.000), while between the poster and the intention to change had effect but not significant (P = 0.128) and completeness of handrub and posters simultaneously significant effect on the intention to change (P = 0.000). This study can be concluded that the completeness of handrub and posters are a combination that can help to increase the nurse's intention to change to perform hand hygiene than partially effect.Â
