Abstract

 Background: Breast cancer is influenced by various risk factors, including age and obesity. Older women who are overweighted and obese have a higher risk of developing breast cancer. This study aims to find the correlation between age and body mass index (BMI) with histopathological features of breast cancer patients in RSUP Haji Adam Malik Medan.Methods: This research is an analytical study using a cross-sectional design with 103 samples obtained from medical record data by random sampling. The data obtained were then adjusted to the research criteria and grouped based on predetermined variables.Results: The majority of the age group was 41-50 years (36.9%), obesity BMI (40.8%), histopathological subtype of invasive carcinoma of no special type (NST) (85.4%), and grade II (46.6%). Kruskal-Wallis test result between histopathological subtypes and breast cancer grading based on age, respectively, obtained p=0.503, r=.325; (α>0.05), and p=0.207, r=0.393; (α>0.05), while based on BMI obtained p=0.017, r=0.021; (α<0.05), and p=0.018, r=0.018; (α<0.05). The OR value (95% CI) obtained on overweight-obese BMI with invasive carcinoma NST subtype was 7.63 (7.27–14.90) and other subtypes were 2.40 (1.14–13.75), and for grades II and III, they were respectively 3.57 (1.32–8.09) and 3.27 (1.17–9.91).Conclusions:  There was a correlation between BMI with histopathological subtypes and breast cancer grading, but the correlation tended to be weak. Whereas, with age, there was no correlation. BMI overweight-obese were more likely to have invasive carcinoma NST subtype and higher-grade of breast cancer.

Abstract

Abstract: Major adverse cardiovascular events (MACE) are the most relevant outcome in cardiovascular secondary prevention because they are the main cause of mortality and morbidity in patients with cardiovascular disease. Due to the contrasting results between studies, omega-3 supplementation is believed to have cardioprotective effects, but its significance in reducing MACE is still in question. This study aims to determine the effect of omega-3 supplementation on the prevention of MACE. A literature search was carried out on several databases from September 820, 2021. Selection of literature is done through the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) flowchart. Literature that is eligible was then assessed qualitatively. We compared the hazard ratio of each key outcome of interest, which was included in the primary and secondary end-point of each study. Among 2049 publications found from the initial literature search, 6 publications were included in this study. The analysis showed a significant reduction in cardiovascular risk in participants with a high dose of pure EPA. Studies with a lower dose of omega-3 that contains a combination of EPA and DHA did not provide similar significant results. Abstract

Background: The decidual cast represents a condition which the entire endometrial lining is shed in intact condition through the vagina. This phenomenon is infrequent and has been linked to various factors, including ectopic pregnancy and the use of exogenous progesterone. In the current report, we present a case finding of ectopic pregnancy upon discovery of decidual cast.Case: Female, 36 years old, came with complaints of pain in the pelvic area accompanied by patches of bleeding from the genitals since 1 week ago. On the 8th day since the complaint was felt, the patient complained of severe pain accompanied by discharge of tissue from the genitals. On histopathological examination of the tissue, a decidual cast was found in the absence of chorionic villi. The pregnancy test was positive and on ultrasound examination we found a mass with a cavity measuring 1.55 x 1.53 cm in the left fallopian tube so that the diagnosis of tubal ectopic pregnancy was established and a laparoscopic procedure was carried out. Discussion: The decidual cast is a proliferation of the endometrial lining that is released through the vagina in an intact form. This finding of decidual cast is often found in ectopic pregnancies and is associated with exposure to exogenous progesterone. The release of the decidual cast through the undilated cervix will cause severe pain referred to as membranous dysmenorrhea. The differential diagnosis of this decidual cast finding is the occurrence of abortion. However, the results of histopathological examination of the tissue obtained a decidual cast without chorionic villi so that the possibility of abortion can be removed.Conclusion: The identification of a decidual cast is frequently linked to the occurrence of an ectopic pregnancy. Therefore, its presence should alert clinicians to consider the possibility of an ectopic pregnancy and conduct further investigations.
