Abstract

Background: Mortality in Indonesia has significantly increased. Students’ comprehension about maternity care “woman centered care” philosophy is an important thing in high quality maternity care, it is one solution to decrease the number of mortality rate. Continuity of care maternal clinic model of learning (PKK-CoC) is giving for student to learn the philosophy.Objectives: reveals level and form of students’ comprehension about maternity care “woman centered care” philosophy.Method: Mixed method with sequential explanatory strategy. The quatitative are use analitic descriptive design, while the quantitative use case study design. Population and sample is 55 students of  Estu Utomo Boyolali institute health and science who have taken PKK-CoC. Sampling use criterion and intensity.Result: qualitatively, students overall comprehension is good. Quantitatively, students comprehension on personalized care, holistic care, collaborative care, and evidence based care is also good. Partnership care is the only aspect considered in effective.Conclusion: continuity of care shows that client and family are in good heatlth, zero mortality rate, client fells safe and comfortable, and client has correct and fast care. It is also found that students’ confidence, skills, and comprehension about maternity care “woman centered care” philosophy are increased. It is suggested including the PKK-CoC model into curriculum of midwifery education

Abstract

Perubahan pada masa postpartum memerlukan banyak penyesuaian sehingga dapat menimbulkan gangguan baik psikologis maupun fisik. Intervensi yang dapat dilakukan untuk mengurangi maternal depressive symptoms berupa non farmakologi yaitu senam nifas. Senam nifas merupakan suatu latihan yang sederhana di rumah sakit, dan dapat dilanjutkan di rumah yang bertujuan menolong dalam meningkatkan pemulihan fisik ibu postpartum dan juga dapat membantu mengurangi gejala depresi. Untuk mengetahui pengaruh senam nifas terhadap perubahan maternal depressive symptoms. Quasi experiment dengan pretest and posttest non equivalent control group design. Teknik sampel menggunakan consecutive sampling dengan jumlah 59 subyek penelitian terdiri dari kelompok intervensi dan kelompok kontrol. Analisis yang digunakan adalah uji paired sample t test dan independent t test dengan tingkat kemaknaan p value < 0,05 dan analisis multivariat dengan menggunakan regresi linear. Hasil uji t-test menunjukkkan bahwa ada perbedaan rata-rata penurunan skor maternal depressive symptoms pada ibu postpartum pada kelompok perlakuan dibandingkan kelompok kontrol sebesar 4,9 dengan nilai p value < 0,001, 95% CI=3,097-(6,884).Penurunan skor maternal depressive symptoms pada ibu postpartum yang melakukan senam nifas lebih besar dibandingkan ibu yang tidak melakukan senam nifas.

Abstract

One indirect causes of maternal mortality in Indonesia is "Three Delays", delays in: (1) deciding to seek appropriate medical help for an obstetric emergency; (2) reaching an appropriate obstetric facility; and (3) receiving adequate care when a facility is reached, which has relation with the lack of women's ability in empowering herself to actively participate and to make suitable decisions about midwifery care they need. The research aims to explore the partnership between woman and midwife in midwifery care. A descriptive qualitative study was conducted using a convenience sample of six low-risk women after they had given birth and six midwives from six geographically distinct midwife-led care clinic in Bantul Regency. Data were collected through semi‑structured in‑depth interviews. The interview was conducted two  to three times and recorded by audio record. Interviews were transcribed and analyzed by the thematic analysis according to Cresswell approach. Seven major themes were identified: (a) midwives ability to partner with women, (b) equality and reciprocity, (c) negotiation, (d) empowerment, (e) trust and time, (f) sharing power and responsibility, and (g) professional friendship. Most of the women are not capable to empower themselves such as less participating in decision making, less considerate in choosing a birthing position, and worsen by the limitations of midwives availability during the delivery process. Midwives need to reflect more the philosophy meaning inside the midwifery care and strengthen the midwifery professionals curriculum in order to persuade women as a partner who capable to empower themselves and to participate actively.

Abstract

Pregnancy is a time of increased vulnerability to the development of anxiety and depression. Mental health of pregnant women needs attention, because it will have an impact on their physical health. Anxiety and depression in pregnant women have a high risk of abortion, impaired fetal growth and even in serious conditions can be a cause of suicide. Pregnancy depression is associated with negative effects such as low birth weight, prematurity and cognitive or behavioral problems on the child. The role of health workers in maternal and child health services (MCH), so far has still encountered obstacles in knowing for sure the mental health status of pregnant women. So far, the focus of examination of pregnant women at health service centers is still limited to physical health checks, such as blood pressure monitoring, hemoglobin levels, physical complaints of pregnant women, weight gain and monitoring of nutritional intake, while mental health examinations have not become a special concern, especially in developing countries where there are still very few screenings related to mental health examinations during the perinatal period.To analyze the causes of health problems in pregnant women in developing countries. Scoping review With stages, among others: Identifying the research question, Identifying relevant studies,. Study selection, Charting the data, Collating, reporting the results, This review raises four themes that are the most common causes of mental health disorders during pregnancy in developing countries, namely economic factors, sexual partner violence, family support and education level.

Abstract

Ibu bekerja yang menyusui menunjukkan kecenderungan ibu berhenti menyusui lebih awal atau tidak menyusui bayinya. Hal ini salah satunya disebabkan tidak tersedianya fasilitas ruang laktasi di tempat ibu bekerja. Penelitian ini bertujuan untuk menggali bukti tentang ketersediaan ruang laktasi terhadap pemberian ASI eksklusif pada ibu bekerja. Metode yang digunakan adalah Scoping review menggunakan ceklist PRISMA-ScR. Penulis menggunakan kerangka Arksey dan O’Malley. Alat pencarian di database Proquest, Pubmed dan EBSCO dilakukan untuk mencari artikel mulai dari 2016-2021, original artikel, artikel dalam bahasa inggris (full text dan open access). Seleksi review dan karakterisasi dilakukan dengan penilaian critical appraisal menggunakan The Joanna Briggs Instititute (JBI). Dari 1.054 artikel yang berpotensi relevan, 9 artikel digunakan. Penelitian berasal dari 5 negara yang berbeda dengan metode kuantitaif dan kualitatif digunakan pada penelitian ini. Hasilnya disajikan dalam dua tema, yaitu ibu bekerja yang berhenti menyusui dan faktor resiko penyebab ibu bekerja yang berhenti menyusui. Kesimpulan studi ini mengungkapkan sebagian ibu berhenti menyusui dikarenakan tidak tersedianya fasilitas untuk mendukung ibu menyusui di tempat kerja. Oleh karena itu, ibu bekerja membutuhkan tempat yang mendukung dan nyaman untuk memfasilitasi ibu menyusui ibu bekerja.

Abstract

The safety aspect of health workers including midwives as the frontline in handling patients during the COVID-19 pandemic is at risk of being very vulnerable to being exposed to COVID-19 which threatens their lives and safety. Physical and mental fatigue in health workers during a pandemic increases the risk of burnout which can reduce the quality of service to patients. The purpose of this study was to review evidence related to the psychosocial experience of midwives in pregnancy and childbirth services during the COVID-19 pandemic. The method used was a scoping review beginning with the identification of scoping review questions using the PEO framework; identifying relevant articles to be determined based on inclusion and exclusion criteria; searching for articles using databases, namely Proquest, PubMed, Science Direct, and Wiley Online Library; selecting articles whose search flow is described through the PRISMA Flowchart; performing data charting and critical appraisal; compiling and reporting results. The results obtained based on 8 articles that had been critically appraised showed that 6 articles used qualitative studies, 1 article used mixed-method quantitative study, and 1 article used quantitative descriptive study. A total of 3 themes emerged, namely about the psychological impact, challenges and needs of midwives in pregnancy and childbirth services during the COVID-19 pandemic which affected the experience felt by midwives in pregnancy and childbirth services with psychological impacts such as fear and burnout. In addition, there were challenges faced by midwives and things that were needed by midwives in pregnancy and childbirth services during the COVID-19 pandemic. Abstrak: Aspek keselamatan tenaga kesehatan termasuk bidan sebagai garda terdepan dalam menangani pasien selama pandemic COVID-19 berisiko sangat rentan untuk terpapar COVID-19 yang mengancam jiwa dan keselamatannya. Kelelahan fisik dan mental pada tenaga kesehatan di masa pandemic meningkatkan resiko terjadinya burnout yang dapat mengurangi kualitas dalam pelayanan pada pasien. Tujuan penelitian ini untuk mereview evidence terkait dengan pengalaman psikososial bidan dalam pelayanan kehamilan dan persalinan di masa pandemic COVID-19. Metode yang digunakan ialah scoping review diawali dengan identifikasi pertanyaan scoping review menggunakan framework PEO; mengidentifikasi artikel yang relevan dengan ditentukan berdasarkan kriteria inklusi dan eksklusi; melakukan pencarian artikel menggunakan database yakni Proquest, PubMed, Science Direct, dan Wiley Online Library; melakukan seleksi artikel yang alur pencariannya digambarkan melalui PRISMA Flowchart; melakukan data charting dan critical appraisal; menyusun dan melaporkan hasil. Hasil yang diperoleh berdasarkan 8 artikel yang telah dilakukan critical appraisal, sebanyak 6 artikel menggunakan studi kualitatif, 1 artikel menggunakan kuantitatif mixed-method, dan 1 artikel menggunakan studi deskriptif kuantitatif. Sebanyak 3 tema yang muncul yakni tentang dampak psikologis, tantangan dan kebutuhan bidan dalam pelayanan kehamilan dan persalinan dimasa pandemic COVID-19 yang mempengaruhi pengalaman yang dirasakan oleh bidan dalam pelayanan kehamilan dan persalinan dengan adanya dampak psikologis seperti ketakutan dan burnout. Selain itu terdapat tantangan yang dihadapi oleh bidan dan  hal-hal yang menjadi kebutuhan bidan dalam pelayanan kehamilan dan persalinan di masa pandemic COVID-19.

Abstract

A midwife has the best role and opportunity in providing midwifery care services and promotion of Maternal Child Health (MCH). Midwifery care is conducted by placing women as partners with a holistic understanding, as a good experience for their physical, psychological, emotional, social, cultural, spiritual, and reproductive health. This review aims to observe scientific research related to characteristics as well as perspectives on personcentered maternal care implementation in midwifery care service. Method: This scoping review was conducted by identifying the review questions by using PEOS framework; identifying relevant article based on inclusion and exclusion criteria; searching article by database; selecting article by using PRISMA Flowchart plot. Result: Based on 10 selection article, 7 articles is obtained A score and the 3 other articles get B score. Furthermore, there are 2 obtained themes, namely the importance and the characteristic of PCMC. Conclusion: The importance thing in each midwifery service implements the care based on Person Centered Maternal Care. PCMC characteristic is providing responsive, supportive care of client, conducting good communication, and providing autonomy to client for determining the choice. Midwifery as care providers should make improvement in the services so that client obtains a good experience in every maternal care provided.

Abstract

Mothers unable to fight anxiety and fear, especially third-trimester pregnant women, will experience the release of catecholamine hormones in high concentrations and experience increased labour pain, obstructed labour, and discomfort during labour. The purpose is to analyze the effect of childbirth education in Reducing Anxiety in Facing Labor in Third Trimester Pregnant Women by including relevant sources of evidence from each article. The method in this study uses databases, namely Pubmed, ScienceDirect, and Wiley, published between 2012-2022. The results showed that of the nine articles reviewed, seven reports came from developing countries, namely Indonesia and Iran. Two words came from developed countries, namely Australia and that the existence of childbirth education would significantly reduce the level of maternal anxiety so that the delivery process could run smoothly and childbirth education will reduce the desire of pregnant women to give birth by SC. While previous research stated that the cause of anxiety in facing childbirth is, experience and stress can be influenced by family support, marital status, history of disease, mother's age and level of education.
