Abstract

The prevalence of breast cancer in developing countries as well as developed countries still occupies in high level. The awareness of breast cancer risk factors can enhance the early detection program. The objective of this research is to achieve a complete description concerning risk factors: social, economy and demography statuses that will be advantageous for integrated prevention program of  breast cancer. This descriptive research used 485 datas out of 560 medical records that belonged to Rumah Sakit Onkologi Surabaya’s new breast cancer patients in 2014. The results of this research showed that 31.34% of breast cancer patients attained in the age of 41-50 years old and 51-60 years old; 41.65% acquired university education, 53.61% lived in Java; 69.69% did not have any history of cancer in their family; 85.36% did not have any history of breast cancer in their family; 57.31% were housewives; 42.27% obesity; 46.19% had been pregnant more than twice; 38.56% gave birth more than twice; 82.06% never used any contraception; 8.66% usedcontraception for 1-5 years; 63.71% breast-feeding; 27.84% breast-feeding to 3 or more children; and 21.03% breastfeeding for 0 to 6 months. There were some risk factors that are detectable and preventable. Prevalensi kanker payudara di negara berkembang dan negara maju masih sangat tinggi. Kesadaran akan faktor risiko kanker payudara dapat meningkatkan peluang untuk melakukan program deteksi dini. Penelitian ini bertujuan untuk mendapatkan gambaran yang lengkap perihal faktor risiko: status sosial, ekonomi, dan demografi yang akan bermanfaat untuk perencanaan program penanggulangan kanker terpadu. Penelitian deskriptif ini menggunakan 485 data dari 560 data rekam medis pasien kanker payudara baru 2014 di Rumah Sakit Onkologi, Surabaya. Hasil penelitian menunjukkan 31,34% pasien berusia 41−50 tahun dan 51−60 tahun; 41,65% berpendidikan universitas; 53,61% bertempat tinggal di Pulau Jawa; 69,69% tidak memiliki riwayat keluarga dengan kanker; 85,36% tidak memiliki riwayat keluarga dengankanker payudara; 57,31% ibu rumah tangga; 42,27% obesitas; 46,19% hamil lebih dari 2 kali; 38,56% melahirkan lebih dari 2 kali; 82,06% tidak pernah menggunakan kontrasepsi; 8,66% menggunakan kontrasepsi selama 1-5 tahun; 63,71% memiliki riwayat menyusui; 27,84% menyusui 3 anak atau lebih; dan 21,03% menyusui selama 0−6 bulan. Didapatkan sejumlah faktor risiko yang dapat diwaspadai serta dicegah.

Abstract

Background : Flexor tendon injury of the hand is common and it used to end up with poor outcomes. It gives negative effects to patient’s daily life. Successful treatment for these cases is a challenge for surgeons. Before 1967, Injury in Zone II was called as “No Man’s Land”. Flexor tendon repair and rehabilitation have been substantially improved through advances in repair and rehabilitation. Method : We reviewed literatures from PubMed, MEDLINE, Cochrane and Google Scholar. It was researched using the terms flexor tendon zone II injury, flexor tendon zone II repair and flexor tendon zone II rehabilitation. Topics covered included anatomy, suture repair and material, and rehabilitation. Result : There is no significant difference of rupture rates and functional outcomes in the number of core suture. Braided polyester suture is the choice for core suture and monofilament for peripheral suture. There is no significant difference in rehabilitation using early passive motion or early active motion. The preference for post operative treatment is by using short splint without immobilization of the wrist. Discussion : Repair sutures techniques and suture materials have been improved as well as active mobilization rehabilitation protocols including a change of wrist position by modification of splints. Improvement in putting splint from a traditional dorsal blocking splint into splint that not immobilize the wrist. Despite all of these modifications, tendon ruptures have not been eliminated. The definitive answer remains elusive.

Abstract

Backgrounds: The main objective of Frontoethmoidal encephalomeningocele (FE) treatment are neural morbidities defect correction and aesthetically pleasing looks. Staged procedures are used to be performed in Indonesia. This article aimed to reveal the result of FE correction through the single-stage modified Chula technique (ST-MCT) procedure in collaboration with the neurosurgery team. Case Reports: A rare case of 5 years old girl diagnosed with FE was reported in this study. The FE was slowly increased in size, causing apparent facial deformity and the appearance of telecanthus. An ST-MCT procedure in collaboration with the neurosurgery team was conducted to correct the defects. The IOD values, IPD values, postoperative complications, and anesthetic improvements were evaluated in this study. Result: The patient was well after the surgery, with no complications and short length of stay. There were noted improvement of ICD, IOD and IPD postoperative follow up. At three years after the surgery ICD= 29mm; IOD= 26mm; IPD= 52 mm (normal range). Summary: ST-MCT procedure conducted in collaboration with neurosurgery team had shown excellent correction of ICD, IOD, and IPD values, no complication, shorter length of stay, and minimal scars. It considers as the most proper technique to reach a good result of correction and aesthetically pleasing looks in FE cases.

Abstract

Highlights: This case shows autoaugmented mastopexy as a natural alternative to silicone implants for patients seeking breast enhancement. The combination of breast lift and autoaugmentation using the patient’s own tissue is presented as an effective method for achieving a firmer breast appearance while maintaining a natural look. The use of tailored preoperative design, including breast ultrasound, enhances surgical safety and outcomes. Abstract: Introduction: Breast ptosis is a common concern among women due to factors such as aging, pregnancy, breastfeeding, and weight loss. To achieve a beautiful breast shape, mastopexy (breast lift) with augmentation is often required. This case report highlights the relevance of the chosen augmentation method in addressing ptosis. Case Illustration: A 30-year-old woman had breast ptosis after her 2nd child and wanted a breast lift to have lifted and firmer breasts. Due to limited fat tissue available for transfer and her refusal to use silicone implants, the patient chose autoaugmented mastopexy. Before surgery in August 2022, breast ultrasound showed abnormalities. Preoperative design was made to determine skin and pedicle positions, using a wise pattern with a superior pedicle to lift the nipple-areola complex (NAC) and an inferior pedicle for autoaugmentation. Surgery was done carefully to achieve symmetry and desired result. Discussion: Breast tissue changes a lot during and after pregnancy and hormonal factors increases the risk of ptosis. According to Regnault classification, the patient was classified as 3rd degree ptosis. While prosthetic implants are common in breast surgery, the patient declined that option. Autoaugmented mastopexy offers a more natural result without the risks of implants. Conclusion: Breast lift with autoaugmentation is suitable for patients with breast ptosis seeking a firmer appearance without added volume. This technique utilizes the patient's own tissue to achieve satisfactory results, although outcomes may vary depending on individual factors.
