The Use of PILA-Pack: Differences in Length of Stay of Hemorrhoidal Patients Fadli Robby Amsriza; Rizka Fakhriani

Abstract

Hemorrhoids are a common anorectal disease and can affect all ages. This research aims to specify the differences in Length of Stay (LOS) of hemorrhoidal patients who underwent a modified combined method of rubber ligation and sclerotic injection using Paran Injection Ligation for Ambeien pack (PILA pack) compared to a hemorrhoidectomy. This study consisted of 56 respondents who underwent hemorrhoidectomy, and the other 56 respondents experienced a modified combined method of rubber ligation and sclerotic injection using a PILA pack. Data showed the LOS's average of respondents who underwent hemorrhoidectomy was 47.33 hours or 1.97 days. Meanwhile, respondents' average length of stay who underwent a modified combined rubber ligation and sclerotic injection using a PILA pack was 20.44 hours or 0.85 days. Finally, the T-test results showed a p-value of 0.001 (p<0.05), indicating that there was a statistically significant difference in the average LOS between hemorrhoidal patients who underwent hemorrhoidectomy and hemorrhoidal patients who underwent a modified combined method of rubber ligation and sclerotic injection using PILA pack. Based on the result, it can be concluded that the mean LOS for hemorrhoidal patients receiving haemorrhoidectomy vs. hemorrhoidal patients undergoing a modified technique of rubber ligation and sclerotic injection utilizing the Paran Injection Ligation for Ambeien pack was significantly different.

Abstract

The three parts of the ear are the external ear, middle ear, and inner ear. The ear contains cerumen, a natural material that protects the external ear canal. Proper ear hygiene and health are essential to prevent ear infections, ear ringing, hearing loss, and sudden deafness. In addition to improper ear cleaning, other factors might contribute to ear disorders, including the misuse of personal listening devices. SD Muhammadiyah Sapen students would be educated on the significance of maintaining ear health through this service project. This community service activity is conducted online via mini-webinars or counseling using zoom meetings, presentations, educational films, and brochures on how to maintain ear health. This extension is quasi-experimental, with a pre-and post-test design involving a single group, utilizing the Paired Samples T-Test model for data analysis. The activity commences with a pre-test administered to students, continues with the presentation of information, discussion, and question-and-answer sessions, and concludes with a post-test designed to assess students' level of knowledge and the activity's success. Twenty students participated in this community service event. Conclusion: There is a significant difference between the average pre-test and post-test scores with a p-value of 0.01 (0.05).

Abstract

Gallstone migration into the cystic duct aperture can prevent bile from exiting the gallbladder during contraction. Gallbladder wall strain increases due to a distinct form of pain (biliary colic). A more prolonged cystic duct obstruction can cause acute gallbladder inflammation (acute cholecystitis). Cholecystectomy is a popular treatment for gallstone-related illnesses. We present a 49-year-old male with heartburn, nausea, and vomiting for one month, worsening in the last week. On general examination, vital signs and urine output are expected. On clinical examination, he had tenderness on the upper right quadrant abdomen, and Murphy's sign was positive. An abdominal ultrasound (US) showed multiple cholelithiasis. The patient was performed elective laparoscopic cholecystectomy with pneumoperitoneum by Muyo hook. The patient was discharged on the third day after surgery. Recent research has highlighted this unusual method. In this unique and challenging situation, Muyo Hook may be an option.
