Abstract

A toxic workplace environment has become a critical issue in healthcare organizations because negative workplace behaviors can reduce employees’ psychological well-being and work engagement, ultimately affecting service quality. This study aimed to analyze the level of toxic workplace environment, identify the dominant dimensions contributing to workplace toxicity, measure employee work engagement, and examine the effect of a toxic workplace environment on work engagement among employees at ENT X Special Hospital, Jakarta. This research employed a quantitative approach with a descriptive-verification design. Data were collected from 50 employees using questionnaires based on toxic workplace environment and work engagement indicators, supported by interviews and organizational document analysis. Data were analyzed using descriptive analysis, validity and reliability tests, classical assumption tests, Pearson correlation analysis, and simple linear regression using SPSS. The findings revealed that the toxic workplace environment was categorized as high, with bullying identified as the most dominant dimension, followed by harassment and abusive supervision. Employee work engagement was categorized as moderate, with absorption identified as the strongest dimension. Statistical analysis showed that a toxic workplace environment had a significant negative effect on work engagement, indicated by a very strong negative relationship between the variables. Despite experiencing a highly toxic work environment, employees maintained moderate levels of engagement due to professional commitment and ethical values in providing healthcare services. This study concludes that organizational efforts to reduce workplace toxicity through anti-bullying policies, effective leadership practices, improved communication, and employee protection mechanisms are essential for establishing a healthier and more sustainable healthcare work environment.
