Abstract

Herniated nucleus pulposus (HNP) is the leading cause of back pain in old age, occurring as a result of tearing of the fibrous annulus and the nucleus pulposus exiting through the tear. To optimize pathology, fat suppression techniques are needed to suppress fat and fluid. Fat Suppression techniques include Dixon and SPAIR. The use of Dixon is relatively longer but can produce four images at once, while SPIR has the advantage of being more resistant to inhomogeneous magnetic fields. The purpose of this study was to determine the difference between T2WI SPIR and T2WI diagnostic image information Dixon Sagital Cut Lumbar MRI with Hernia Nucleus Pulposus (HNP) cases as well as find out which one is better between the two techniques. The type of research used is quantitative research with an experimental approach that uses prospective data aimed at determining the difference in diagnostic image information T2WI SPIR and T2WI Dixon MRI examination of Sagital Cuts. The study population is patients who perform Lumbar MRI examinations with HNP cases at the Radiology Installation of NTB Provincial Hospital for the period May-July 2023. This study applied total sampling, using a research sample of 17 patients. The results showed a difference in image information between the T2WI SPIR and T2WI Dixon sequences on Lumbar MRI examination of Sagital Cut with a p-velue value smaller than α (<0.05), namely in the anatomy of the Spinal Cord with a p-value (0.021), and the anatomy of the Corpus Vertebraewith value p-value (0.001). The T2 Spir sequence is more optimal in showing anatomical information on the Sagittal Cut lumbar MRI examination compared to the T2 Dixon sequence. due to the more homogeneous emphasis of fat on the anatomy of the Spinal Cord and Corpus Vertebrae

Abstract

MRI memiliki peranan yang penting pada aplikasi sains biologis medis, karena MRI mampu menunjukkan gambaran shoft tissue tubuh dan proses metabolisme didalamnya. Tujuan dari penelitian ini yaitu untuk mengkaji optimalisasi pemeriksaan MRI lumbal pada kasus low back pain dengan protocol wholespine sequen T2 TSE sagital di Rumah Sakit Kasih Ibu Denpasar. Jenis penelitian pada karya tulis ilmiah ini merupakan penelitian kualitatif dengan pendekatan studi kasus. Sampel dalam penelitian ini adalah tiga orang pasien dengan Klinis Low Back Pain. Data yang digunakan merupakan data sekunder yang diperoleh melalui observasi, wawancara, dan dokumentasi. Hasil dan kesimpulan penelitian ini menunjukkan bahwa prosedur pemeriksaan MRI Lumbal pada kasus Low Back Pain dengan protocol Wholespine T2 sagital di Rumah Sakit Kasih Ibu Denpasar tidak ada persiapan khusus. Saran dari penulis dalam pemeriksaan MRI Lumbal dengan Kasus Low Back Pain di Rumah Sakit Kasih Ibu Denpasar yaitu pada saat anamnesa harus ditanyakan dengan jelas riwayat pasien.

Abstract

Background: Double Inversion Recovery (DIR) is a newer sequence compared to the T2 FLAIR sequence in MRI Brain examinations used for epilepsy cases. Further research is needed to determine the optimal sequence for MRI Brain in epilepsy cases.Methods: This research is a quantitative study with an experimental approach. This study uses DIR and T2 SPACE sekuen to assess image quality in terms of Contrast to Noise Ratio (CNR). The data will be analyzed using SPSS.Results: The normality test shows a p-value 0.05, indicating that the data is not normally distributed. In the Wilcoxon test, the negative rank indicates a decrease in image quality (CNR) from the DIR sequence to T2 FLAIR SPACE. The test statistics show a p-value 0.05, indicating a significant difference in image quality (CNR) between the DIR and T2 FLAIR SPACE sekuen.Conclusions: The Contrast to Noise Ratio (CNR) image quality in Double Inversion Recovery (DIR) is higher compared to T2 FLAIR SPACE, thus making the DIR sequence more optimal in assessing image quality (CNR) compared to the T2 FLAIR SPACE sequence.
