<![CDATA[ABSTRAKPatient safety adalah suatu sistem pelayanan kesehatan yang menjamin pasien agarterhindar dari cedera yang tidak diinginkan dan kejadian nyaris terjadi yang dapat merugikanpasien. Salah satu sasaran keselamatan pasien yaitu ketepatan identifikasi pasien. Ketepatanidentifikasi pasien dapat diukur menggunakan rekam medis dengan dua identitas. Identitas pasienminimal harus tercantum dalam rekam medis adalah nama dan nomor rekam medis pasien.Berdasarkan studi pendahuluan di RSU Surya Husadha Ubung, 34 (68%) dari 50 rekam medistidak lengkap dibagian review identitas pasien. Penelitian ini bertujuan untuk mengetahuihubungan ketidaklengkapan rekam medis pasien rawat inap dengan insiden keselamatan pasien diRSU Surya Husadha Ubung tahun 2018. Rancangan penelitian menggunakan deskriptif analitikkuantitatif dengan jenis penelitian model kohort. Metode pengumpulan data menggunakanobservasi yaitu checklist. Analisis data yang digunakan adalah uji univariat dan uji bivariate denganalat ukur statistic chi-square. Hasil penelitian menunjukkan tidak terdapat hubunganketidaklengkapan nama pasien pada rekam medis rawat inap dengan insiden berkaitan denganketepatan identifikasi pasien di RSU Surya Husadha Ubung tahun 2018 dengan p-value 0,361 >0,05 dan tidak terdapat hubungan ketidaklengkapan nomor rekam medis pasien pada rekam medisrawat inap dengan insiden berkaitan dengan ketepatan identifikasi pasien di RSU Surya HusadhaUbung tahun 2018 dengan p-value 0,361 > 0,05.Kata kunci : Ketidaklengkapan Rekam Medis Pasien, Insiden Keselamatan Pasien.ABSTRACTPatient safety is a health care system that guarantees patients to avoid unwanted injuries and nearmisses that can harm patients.
