Abstract

Preeklamsi merupakan kumpulan gejala atau  sindrom meliputi  hipertensi  disertai dengan gangguan  organ multisistem, yang terjadi hanya pada kehamilan dengan etiologi pasti sampai saat ini masih belum diketahui. Keadaan patologi  tersebut sampai saat ini masih merupakan salah satu penyebab utama morbiditas dan mortalitas ibu dalam obstetri. Pada pergantian abad ke-19 ke abad ke-20, teori preeklamsi yang populer merupakan kontaminasi racun atau toksin selama kehamilan dan dianut oleh para ilmuwan secara luas sehingga muncul istilah toxemia gravidarum. Pada saat ini teori yang diterima secara luas adalah preeklamsi berhubungan dengan implantasi abnormal trofoblas, yang diyakini menyebabkan iskemik pada plasenta dan berdampak sistemik. Meskipun tatalaksana telah dikembangkan sesuai dengan kedokteran berbasis bukti, tetapi masih terdapat keterbatasan dalam terapi, yaitu menangani gejala dan persalinan merupakan obat terbaik untuk preeklamsi. Hipotesis dan kontribusi ilmiah sebelumnya telah memengaruhi pemahaman patofisiologi preeklamsi saat ini dan telah menyimpulkan suatu strategi untuk tatalaksana dan kriteria klasifikasinya. Memahami teori masa lalu dan mengenal etiologi serta patofisiologi preeklamsi akan berkontribusi pada tatalaksana yang lebih baik di masa depan yang lebih mengedepankan promotif dan preventif.

Abstract

Preeclampsia is a relatively common complication in obstetrics. The real problem currently faced is the adverse effects of preeclampsia on the mother and fetus, while the pathophysiology useful for prevention remains unclear. The clinical symptoms and complications that arise are highly variable and ultimately lead to multi-organ failure. This condition results in preeclampsia being one of the three causes of maternal death worldwide, including in Indonesia. The incidence of preeclampsia/eclampsia in various literature and studies ranges from 4to10% of pregnancies.

Abstract

In the past three months, our institution have cared for 17 cases of pregnancies involving mothers under 20 years old. The youngest case involved a 14-year-old adolescent. These adolescent pregnancies are accompanied by various pregnancy pathologies, such as preterm birth, cephalopelvic disproportion, preeclampsia and eclampsia, mental disorders, and the potential for death for both the mother and the baby. In past decades, the majority of adolescent pregnancies were due to early marriage, while currently, unmarried pregnancies are on the rise. This is noteworthy, as it may be not only in Bandung and its surrounding areas but also throughout Indonesia and even globally. The increase in adolescent pregnancies results in the potential for maternal deaths, which are expected to increase in the future if this is not prevented and controlled properly.
