Abstract

Background: Hormonal contraception is a contraceptive method that can prevent pregnancy because it contains estrogen and progesterone. The use of hormonal contraception for a long time can cause side effects including increased body weight, cholesterol deposits, and hypertension. The purpose of this study was to examine, analyze, and estimate the magnitude of the effect of the use of hormonal contraception on the incidence of weight gain and hypertension in women of childbearing age based on the results of previous similar studies. Subjects and Method: This study is a systematic review and meta-analysis using PRISMA flowchart diagrams. The process of searching for articles was carried out between 2004-2021 using 3 databases, including Google Scholar, PubMed, and Science Direct. The keywords used in the database search are ""Hormonal Contraception" AND “DPMA injection” AND "Weight Gain" AND "Hypertension". Inclusion criteria for full text articles used the observational Randomized Controlled Trial method, the articles used English, and reported the Mean and SD in multivariate analysis. Articles that met the requirements were analyzed using the RevMan 5.3 application. Results: A total of 14 articles with a Randomized Controlled Trial study design originating from Thailand, Indonesia, Pakistan, America, California, Africa, Ethiopia, and Kenya were meta-analyzed in this study. Meta-analysis of 7 articles showed that hormonal contraception had an effect on weight gain in women of childbearing age by 0.03 units but not statistically significant (SMD= 0.03; 95% CI= 0.07 to 0.14; p= 0.530). Meanwhile, 7 articles showed that hormonal contraception increased the incidence of hypertension in women of childbearing age by 0.10 units, but it was not statistically significant (SMD= 0.10; 95% CI= 0.17 to 1.37; p= 0.460). Conclusion: Hormonal contraception increases body weight and hypertension in women of childbearing age. Keywords: hormonal contraception, weight gain, hypertension, meta-analysis. Abstract

Kekurangan gizi yang terjadi selama kehamilan dapat berdampak  pada status gizi janin yang direpresentasikan oleh berat badan lahir yang tidak optimal. Angka kejadian BBLR di Kabupaten Magetan cukup tinggi. Tujuan penelitian ini untuk mengetahui hubungan status gizi pada ibu hamil trimester 1 dengan be berat badan lahir.  Jenis penalitian menggunakan survey analitik, rancangan peelitian cross sectional. Populasi adalah Semua BBL dan ibu dalam rentang waktu Januari-Desember 2019 di Puskesmas Panekan sejumlah 694 orang yang dii gunakan sampel Sebagian BBL dan ibu dalam rentang waktu Januari-Desember 2019 di Puskesmas Panekan sejumlah 254 bayi. Variabel bebas adalah status gizi ibu hamil. Variabel terikat berat badan lahir. Analisa data menggunakan Uji chi square dan Korelasi Cramer.Hasil penelitian status gizi ibu hamil yang mengalami KEK mayoritas melahirkan berat badan lahir kurang sebanyak 14 orang dibandingkan dengan ibu yang tidak KEK hanya sebanyak 2 orang. Ibu hamil yang tidak KEK mayoritas melahirkan berat badan lahir normal sebanyak 211 orang dibandingkan ibu yang mengalami KEK hanya sebanyak 25 orang. Di uji chi Square, dengan nilai p sebesar 0,000 (p< 0,05) , artinya secara statistik terdapat hubungan status gizi ibu hamil trimester 1 dengan berat badan lahir dan nilai Sig Cramer sebesar 0,503. Hasil penelitian ini menunjukkan bahwa terdapat hubungan cukup kuat status gizi pada ibu hamil trimester 1 dengan berat badan lahir di Puskesmas Panekan.Disimpulkan bahwa ada hubungan yang bermakna status gizi trimester 1 dan 2 dengan berat badan lahir . Di harapkan meningkatkan program perbaikan gizi salah satunya dengan memberi susu ibu hami ataupun makanan tambahan lainnya pada ibu hamil KEK dan lebih memperhatikan kesehatan ibu hamil dalam melakukan pemeriksaan untuk mencegah berbagai komplikasi pada kehamilan seperti KEK dan BBLR.Kata kunci : Status Gizi dan Berat Badan Lahir.

Abstract

Background: Intrauterine exposure to tobacco smoke is considered an important risk factor for low birth weight (LBW) infants, low birth weight (SGA) infants, and preterm delivery. Mothers who smoke during pregnancy are known to limit intrauterine fetal growth and result in low birth weight. This study aims to analyze the effect of exposure to cigarette smoke on pregnant women as active smokers on the incidence of low birth weight.Subjects and Method: This research is a meta-analysis study using PRISMA flowchart guidelines. The article search process was carried out between 2013-2021 using databases from PubMed, Google Scholar and Science Direct PICO, namely, population= pregnant women. Intervention= exposure to cigarette smoke. Comparison= not exposed to cigarette smoke. Outcome= low birth weight. The keywords used for the article search were “Pregnant or pregnancy” AND “smoking” AND “birth weight”. The analysis was carried out using RevMan 5.3 software.Results: There are 8 primary studies spread across 4 continents, namely the continents of Australia, South America, Asia, and Europe, which were selected for a systematic review and meta-analysis. The data found showed that exposure to cigarette smoke in pregnant women as passive smokers increased 2.01 times low birth weight compared to pregnant women who were not exposed to cigarettes (aOR= 2.01; 95% CI= 1.32 to 3.06; p<0.001).Conclusion: Exposure to cigarette smoke in pregnant women as passive smokers increases the risk of low birth weight.
