Abstract

Pendahuluan: Hipertensi merupakan salah satu penyakit kardiovaskular terjadi sebagai respon peningkatan peningkatan tekanan perifer. Peningkatan tekanan darah dapat menimbulkan pola dan gejala yang menjadi penentu dalam menegakan diagnosis diferensiasi sindrom pada penderita hipertensi. Tujuan penelitian ini untuk menganalisa tekanan darah dan diagnosis nadi berdasarkan diferensiasi sindrom pada penderita hipertensi esensial. Metodologi: Penelitian ini merupakan penelitian deskriptif dengan pendekatan one shot case study, sampel penelitian sejumlah 60 responden diambil pada populasi penderita hipertensi dengan teknik purposive sampling. Hasil: Mayoritas responden memiliki hipertensi derajat I (61,7%), diagnosa diferensiasi sindrom menunjukkan liver fire flaring upward (40,0%) dan diagnose nadi wiry pulse (50,0%). Ada korelasi yang bermakna dengan antara tekanan darah terhadap diagnosa diferensiasi sindrom (tekanan sistolik r=0,402, p=0,001

Abstract

Perubahan hemodinamik yang signifikan dapat terjadi pada awal pasca induksi hingga periode pemeliharaan anestesi sehingga adanya pemantauan hemodinamik perianestesi sangat diperlukan, termasuk pada pembedahan oftamology. Penelitian deskriptif longitudinal dilakukan dengan melibatkan 30 pasien yang menjalani tindakan pembedahan oftamology dengan anestesi umum. Pasien dengan penyakit jantung dikeluarkan dari penelitian ini. Penelitian ini menggunakan lembar monitoring perianestesi sebagai instrument penelitian. Hasil penelitian ini menemukan bahwa ketidakstabilan hemodinamik paling banyak terjadi di intra anestesi akibat adanya agen anestesi. Adanya monitoring secara detil di intra anestesi khususnya 5 menit pasca induksi agen anestesi sangat diperlukan.

Abstract

Effective and safe anesthesia in eye surgery is essential to ensure optimal perioperative care for both children and adults, so hemodynamic monitoring is essential. The use of local anesthesia can increase systolic blood pressure and diastolic blood pressure without changing heart rate. A quantitative descriptive study with a cross-sectional approach involved 138 patients with purposive sampling techniques. Univariate analysis with frequency distribution was used in this study. The results showed that pre-anesthetic systolic TD ≥ 147 mmHg as much as 56%, pre-anesthesia diastolic TD < 78 mmHg as much as 51%, MAP ≥ 101 mmHg as much as 54%, and heart rate < 79 x / minute. While systolic TD after anesthesia ≥ 147 mmHg as much as 59%, diastolic TD ≥ 78 mmHg as much as 62%, MAP ≥ 101 mmHg as much as 64%, and heart rate < 79 x / minute as much as 58%. Changes in systolic and diastolic blood pressure were found in pre and post-anesthesia, so more detailed monitoring is needed.

Abstract

Pain is the most common complaint found in perioperative patients, so optimal pain management is needed. Proper pain management can be done if the officer is able to know the intensity of pain felt by the patient and what factors are related. The aim of this study was to determine the pain intensity of post-CS patients with spinal anesthesia.This quantitative correlation study with a cross-sectional approach will be conducted involving 87 respondents based on the Slovin Formula. Sampling was carried out using purposive sampling to determine the selected sample. This study will use univariate data analysis techniques (frequency distribution and percentage), and bivariate data analysis (chi-square test). The results of the data analysis showed that the majority of patients had postoperative pain intensity on a scale of <2 as many as 52 people (60%). Age factors, gravida status, BMI, preoperative analgesics, and preoperative pain intensity have p-values> 0.05, namely 0.623, 0.137, 0.341, 0.570, 0.620. It can be concluded that there is no relationship between age, BMI, pre-operative pain intensity, and pre-operative analgesic administration with post-operative pain intensity in CS patients with spinal anesthesia.

Abstract

Patient safety principles have not been optimally implemented in anesthesiology services. Nurse anesthetists, as providers of anesthetic nursing services, must have a good patient safety culture so that they can ensure patients remain safe during the anesthesia process. This study aims to determine the description of patient safety culture among nurse anesthetists in Indonesia. This quantitative study with a cross-sectional approach involved 369 nurse anesthetists as samples. The sampling technique used was purposive sampling. The research instrument used was the Hospital Survey on Patient Safety Culture (HSOPSC) questionnaire developed by the Agency for Healthcare Research and Quality (AHRQ) in Indonesian, with a Cronbach's Alpha value of ≥ 0.70 and a range of 0.809-0.918 for all items consisting of 12 components. Data analysis in this study used descriptive statistics, namely frequency distribution with percentage or proportion measures. This study shows that the majority of nurse anesthetists have a patient safety culture in the poor category (57%), with the lowest scores in the dimensions of non-punitive response to errors and staffing, while the highest scores were in the dimension of teamwork within the unit. Service providers need to take proactive measures to identify and change system weaknesses at all levels of service.

Abstract

The implementation of the case manager (CM) has not been optimal according to what was expected. Even though there is a positive perception about the role of CM held by nurses and NA as part of the Professional Care Providers (PPA) is very necessary. The knowledge factor is the factor that has the most influence on positive perceptions about the role of CM. There are many things that can influence nurses' and NAs' knowledge about CM. This study aims to determine the factors related to nurses' and NAs' knowledge about CM. This quantitative correlational research used a cross-sectional approach involving 384 nurses and 129 NAs. The sampling technique used is multistage random sampling and simple random sampling. Each respondent has filled out a structured questionnaire via Google Form and has been analyzed using descriptive statistics, chi-square test, and binary logistic regression. This study used a questionnaire on knowledge about CM with a Cronbach Alpha value of 0.634. This research found that the majority of nurses and NAs had insufficient knowledge about CM, 248 people (65%) and 80 people (62%). The factors age, education and length of service were associated with nurses' knowledge about CM (p=0.001; p<0.001; and p<0.001). The factors age and years of service were associated with NA's knowledge about CM (p=0.017; and p=0.024). This study also found that knowledge was a determining factor in nurses' knowledge about CM (OR=2.53; 95%CI 1.60-4.01; p<0.001). These findings support the theory that age, education and years of service are related to knowledge about CM. Socialization about case managers and increasing the role of case managers in services needs to be carried out by hospital management.

Abstract

Pre-anesthesia assessment is very important in ensuring the success of surgical procedures, especially in cases involving comorbidities, such as cardiovascular disorders. Pre-anesthesia assessment is often only performed by 19.5% of anesthesiologists, and the results are not recorded immediately after the procedure in the patient's medical records due to time constraints. This study aims to determine the factors that influence anesthesiologists in conducting pre-anesthesia assessments on surgical patients with cardiovascular disorders. The research design used was a correlational analytical study with a cross-sectional approach. The population was anesthetists in Indonesia, and the sample was selected using purposive sampling. Univariate analysis was performed by determining the frequency distribution and percentage of general characteristics of respondents, while bivariate analysis with chi-square was performed to determine the relationship between factors affecting anesthetists in conducting pre-anesthesia assessment on surgical patients with cardiovascular disorders. The sample size in this study was 245 respondents. The results of the study showed that factors associated with the implementation of pre-anesthesia assessment in surgical patients with cardiovascular disorders were length of service (p 0.020), workload (p 0.018), and number of anesthesiologists (p 0.009).

Abstract

Geriatric patients undergoing surgical procedures are required to strictly comply with regulations related to pre-anesthetic preparation. Intraoperative and postoperative complications occur relatively frequently, particularly among elderly patients. To date, there has been limited research examining compliance with pre-anesthetic preparation among geriatric patients with a history of cardiovascular disorders. This study aimed to determine the level of compliance with pre-anesthetic preparation among geriatric patients with a history of cardiovascular disorders at Sanjiwani Regional General Hospital, Gianyar. This study employed a descriptive design with a cross-sectional approach. The sample was obtained using a consecutive sampling technique, with a sample size of 102 geriatric patients. The questionnaire used was a standard questionnaire with a Cronbach's Alpha value of 0.80. Data were collected using a questionnaire and analyzed using descriptive statistics. Result: The results showed that the majority of respondents had a moderate level of compliance with pre-anesthetic preparation, accounting for 64 respondents (62.7%), while 10 respondents (9.8%) demonstrated a low level of compliance. Collaboration among various parties is expected, not only involving healthcare professionals but also emphasizing the importance of family involvement, to improve patient compliance and ensure that surgical and anesthetic procedures during the intraoperative period can be carried out smoothly.
