Abstract

Retention of medical records is carried out because medical records will continue to grow until the storage rack is full and there is no longer enough so that medical records will not be stored forever in the storage room. The results of observations in the preliminary study at RSUD Dr. H. Moch. Ansari Saleh, inactive medical records are still not stored in the inactive storage room as they should be. This study aims to review the retention of medical records in RSUD Dr. H. Moch. Ansari Saleh Banjarmasin from preparation, implementation to evaluation. This research method uses qualitative research with a case study design. The subjects of this study were taken using a purposive sampling technique with the subject of 1 Head of Medical Record and 3 Medical Record Officers at Dr. Hospital. H. Moch. Ansari Saleh Banjarmasin. The results of the study found that the preparation of medical record retention in RSUD Dr. H. Moch. Ansari Saleh Banjarmasin already has a policy, for the schedule there are still SOPs for retention, special human resources to carry out retention already exist, while the retention implementation facilities are still not sufficient to store inactive medical records. no treatment for 3 years and after being assessed for use, it will be destroyed using an incinerator machine, there are still obstacles, namely the limited space for storing inactive medical records. The conclusion of this study, the implementation of retention that is not in accordance with the existing SPO, an assessment is taken of a form that has value for storage, the obstacles found are the inactive medical record storage room in the active storage room.

Abstract

Inappropriate medical record storage systems cause misfiles. Human resources and facilities factors have a role in misfile incidents. Missfile is a medical record file that is lost and misplaced on the medical record file storage rack in the filling room. This research is a quantitative research with a descriptive approach. The number of medical records in 2021 that is accurate is 6176 medical records with a sample of all medical records. This study obtained data on medical record missfile incidents as much as 1.81% of files, while the factors that play a role in misfile incidents are human resource factors, policies and medical record storage systems. In making efforts to prevent the occurrence of medical record misfiles, it is necessary to strengthen the implementation of policies, increase the competence of officers and add storage shelves so that medical records do not accumulate

Abstract

Use of information systems is a strategic means for providing registration services that are oriented towards customer satisfaction. Several research results show that there are manual and electronic patient registration systems. The development of Android-based information systems is increasing. Medical records at the Almeta Clinic currently still use a manual system from registration to storage. This research is development research (RnD) with 6 research stages. Carrying out research by creating an outpatient medical resume information system design, researchers created a DFD (Data Flow Diagram), ERD (Entity Relationship Diagram) as well as the appearance and design of a patient registration information system design created with Microsoft Access and Microsoft Visio in accordance with the identification of user needs and requirements. data. The media expert's results showed 80% with the qualification indicators "Very Good" and the results "Decent". validation results from material experts show 82.5% of the qualification indicators are "Very Fulfilling" and the results are "Decent".

Abstract

The Implementation of Electronic Medical Record (EMR) Increases Work Efficiency in Medical Record Management. EMR enables faster and easier access to information, improving data integration between hospital management systems and other systems to reduce human error. This study aims to determine the implementation of inpatient registration in EMR at Bhayangkara TK III Hospital Banjarmasin. This research is qualitative with a descriptive approach, data collection through observation and interviews. The research subjects consisted of 3 people, namely the Head of the Medical Record Unit, the Head of the IT Unit, and the Inpatient Registration Officer. The results of this study indicate that the implementation of inpatient registration in terms of human resources has followed EMR training, there is a Standard Operating Procedure (SOP), and officers apply the SOP. The inpatient registration facilities meet the needs of officers, but there are still obstacles, namely errors in the SIMRS Khanza network during service hours.

Abstract

The implementation of electronic medical records (EMR) aims to accelerate services and improve the quality of services at the Health Center. Conceptually, EMR makes it easier for officers to work, shortens data distribution time and minimizes file storage space. This study aims to evaluate the implementation and storage of EMR at the SP Health Center in SMD City. The study used a qualitative approach with a phenomenological design, data collection techniques with observation and interviews with 4 informants selected purposively. The main research instrument was the researcher himself supported by interview and observation guidelines. The data analysis used the Miles and Hubberman approach. The results of the study revealed that there were still incomplete clinical information filling and electronic medical records did not support electronic signatures and the security of medical records was not guaranteed so that the implementation of filling in clinical information and storing electronic medical records had not gone well. Examining this matter requires emphasis and monitoring of health workers who fill in and store them.

Abstract

Improving maternal health literacy is a critical priority in efforts to reduce pregnancy-related complications. The DARLING (Data and Information Services for Pregnancy) web-based application was developed as an interactive educational tool for pregnant women. However, the success of such an application is not solely determined by its content, but also by its functionality, system reliability, and user experience. This study employed the User Acceptance Test (UAT) method using the Guttman scale to assess the feasibility of the DARLING application. The study population consisted of 30 pregnant women selected through accidental sampling at Basirih Baru Health Center. The three main variables evaluated were functional aspects, non-functional aspects, and user experience. The data were analyzed using quantitative evaluation methods. The findings showed that all functional indicators received a 100% approval rate, non-functional aspects scored 99.17%, and user experience reached 96.67%. The total score from 12 indicators resulted in an overall feasibility percentage of 98.61%, categorized as “feasible.” The DARLING web-based application was very well received by pregnant women, with a feasibility rate of 98.61%. These findings indicate the application's strong potential as a digital pregnancy education medium. Further research is recommended to evaluate the educational content more thoroughly and to develop additional features that support the specific needs of pregnant women.
