Abstract

Cavernous sinus thrombosis (CST) is a rare diagnosis although must be included in the differential of headache. CST is either infectious or aseptic. Aseptic thrombosis usually occurs secondary to trauma or is post surgical. Sinusitis appears to be the most common cause for septic CST. Otitis media, facial cellulitis (located medial third paranasal) and odontogenic infections are also commonly implicated. Cavernous sinus thrombosis (CST) is a rare phenomenon that requires clinical suspicion and emergent imaging for correct diagnosis. Mycotic aneurysm is a rare complication of cavernous sinus thrombosis and may be treated with antibiotics, carotid ligation, internal carotid artery balloon occlusion, or Guglielmi detachable coil (GDC) embolization.

Abstract

Acute heart failure (AHF) is a common and growing medical problem associated with major morbidity and mortality. Despite the high prevalence of this condition and its associated major morbidity and mortality, diagnosis can be difficult, and optimal treatment remains poorly defined. Identification of the acute triggers for the decompensation as well as characterization of cardiac filling pressures and output is central to management. Evaluation of patients with Heart Failure (HF) is critical for the appropriate selection and monitoring of therapy as well as for the prevention of recurrent hospitalizations.Diagnostic and management AHF is a challenge because of the heterogeneity of the patient population, the existence of a universal definition of understanding, not understanding the pathophysiology, and the lack of bases guideline Evidence-based medicine

Abstract

Abstrack. Hypertension is one of the degenerative diseases, blood pressure generally increases slowly with age, elderly disease prevention process is inseparable from the role of the family, family support is needed to help to treat hypertension. Research Purpose To know the relationship of informative and emotional support families with medication adherence in elderly hypertensive. Methods The study design using analytical observational with cross sectional approach. The population is total visits of elderly with hypertension in last month at Ranomuut Public Health Center, Manado City. The sampling technique used purposive sampling, with the number of samples is 40. To collecting the data using questionnaires. Then the collected data is processed using computer assistance with chi-square test with significance level (α) 0.05. Result The informative support compliance where p value = 0.011 and emotional support to compliance with p value = 0.034. Conclusions thus there is a relationship with the family support medication adherence in elderly hypertensive. Hipertensi merupakan salah satu penyakit degeneratif, umumnya tekanan darah bertambah secara perlahan dengan seiring bertambahnya umur, Proses pencegahan penyakit lansia tidak terlepas dari peran keluarga, Dukungan keluarga sangat dibutuhkan untuk membantu mengatasi hipertensi. Tujuan Penelitian diketahuinya hubungan dukungan Informatif dan Emosional keluarga dengan kepatuhan minum obat pada lansia hipertensi. Metode penelitian menggunakan Desain penelitian Analitik Observasional dengan pendekatan cross sectional. Populasi adalah Banyaknya Kunjungan lansia Hipertensi 1 Bulan Terakhir dipuskesmas Ranomuut Kota Manado. teknik Pengambilan sampel menggunakan Purposive Sampling, dengan jumlah 40 sampel. Pengumpulan data menggunakan kuesioner selanjutnya data yang telah terkumpul diolah menggunakan bantuan komputer dengan menggunakan uji chi sqaure dengan tingkat kemaknaan (α) 0,05. Hasil penelitian dimana dukungan informatif dengan kepatuhan didapatkan nilai p=0,011 dan dukungan emosional dengan kepatuhan didapatkan nilai p=0.034. Kesimpulan Terdapat hubungan dukungan keluarga dengan kepatuhan minum obat pada lansia hipertensi. Kata kunci: Dukungan Informatif dan Emosional Keluarga, Kepatuhan Minum Obat, Lansia Hipertensi

Abstract

Objective:  To assess the cardioprotective effects of Carvedilol in preventing subclinical left ventricular dysfunction (SLVD) in breast cancer patients after completing FAC chemotherapy.Methods: This prospective study employed a quasi-experimental clinical trial conducted from September 2018 to May 2019. Breast cancer patients receiving FAC chemotherapy were divided into two groups: intervention (IG) and control (CG). The IG received Carvedilol 6.25 mg b.i.d., which was increased every three weeks until reaching a tolerated dose. The study evaluated changes in left ventricular global longitudinal strain (GLS) and the incidence of SLVD (GLS reduction ≥15% and GLS >-18%) 24 weeks after initiating the FAC regimen.Result: Of the 81 women enrolled in the study, 31 were in the IG. No significant changes in GLS were observed during or after completing FAC chemotherapy in the IG, whereas the CG showed contradictory results. At the end of the follow-up period, the delta GLS reduction was lower in the IG (0.7; 95% CI -0.60, 3.60) compared to the CG (3.00; 95% CI -2.16, 4.19), with a p-value of 0.035. Similarly, the percentage reduction in GLS was 3.6% in the IG and 14.29% in the CG, resulting in a p-value of 0.05. The incidence rate of SLVD (GLS reduction ≥15% and GLS > -18%) was lower in the IG (41.9% and 25.8%) than in the CG (58% and 48%).Conclusion: Carvedilol may have a cardioprotective effect in preventing the incidence of SLVD, as evaluated by GLS reduction and changes, in women with breast cancer after completing a full cycle of the FAC regimen.
