Abstract

Saat infeksi COVID-19 menjadi pandemi ketiga dan terluas di dunia, studi mengenai tatalaksana  preventif dan kuratif COVID-19 sangat dibutuhkan. Studi sebelumnya menunjukan kemungkinan vitamin D sebagai profilaksis dan terapi untuk COVID-19, terutama saat hipovitaminosis D yang sering terjadi. Studi ini memuat dua kasus terkonfirmasi COVID-19 di Rumah Sakit Bethesda Yogyakarta Indonesia dan data tentang tanda, gejala klinis serta pemeriksaan laboratoriumnya termasuk status vitamin D yang diukur menggunakan metode laboratorium standar. Keduanya menunjukan hasil defisiensi vitamin D. Laporan kasus berbasis bukti ini bertujuan untuk mengetahui hubungan antara status vitamin D dan tingkat keparahan COVID-19. Pencarian literatur dilakukan di database PubMed, CENTRAL, EbscoHost dan ProQuest dengan kata kunci  ‘vitamin D’, ‘deficiency’, ‘status’, ‘COVID-19’, ‘severity’, ‘risk factor’. Didapatkan satu artikel terkini yang mempresentasikan kasus studi ini, yaitu studi kohort yang menunjukan bahwa terdapat hubungan signifikan antara defisiensi vitamin D dengan tingginya risiko infeksi dan derajat keparahan COVID-19.

Abstract

Despite the typical primary symptoms reported, the coronavirus disease 2019 (COVID-19) outbreak could be manifested as many symptoms. Neurological manifestations of COVID-19 are hypothesized to be caused by multiple pathophysiologies. As one of the possible prodromal symptoms in the absence of fever and respiratory symptoms, many individuals might be unaware of contracting COVID-19 infection and could possibly infect other people. This case series involved a hundred and twenty COVID-19 patients in which nine of them reported headaches as the chief complaint to provide the prevalence rate. Medical histories were assessed to provide the prevalence rate and laboratory findings of involved subjects. Narrative review of possible mechanisms of the manifestation were also denoted. To date, the proportion of non-specific manifestations as the first symptom needs to be further explored as it could be one of the initial symptoms and early manifestations of COVID-19.

Abstract

Introduction: Multiple well-circumscribed round nodules, also known as “Cannonball” appearance in the lungs, are typically associated with specific pulmonary metastases. However, limited reports denote the finding, especially if correlated with primary squamous cell lung carcinoma in the younger population. This is the first case report in Indonesia of a 19-year-old male with primary squamous cell carcinoma in the lung and chest radiograph image mimicking a “Cannonball” appearance Case Presentation: A 19-year-old male was presented with a history of progressing hemoptysis for two weeks with no other abnormal respiratory symptoms but a reduction of vocal fremitus. The subject is an occasional smoker and had a history of lung tuberculosis in 2009 and a suprarenal or adrenal mass. X-ray imaging revealed multiple bilateral pulmonary nodules, mimicking the “Cannonball” appearance. A fine needle aspiration (FNA) biopsy of a nodule was done and interpreted to be primary squamous cell carcinoma. Symptomatic treatment was given and the subject was discharged on the fifth day. Conclusions:  A “Cannonball” appearance or its mimic is not always typical to certain carcinoma based on what this case report finds. In the case of a tumor, radiograph findings should also be correlated with other supporting tests such as biopsy and clinical presentation.
