Abstract

Objective: The aim of this case report is to describe treatment management restoration after endodontic reintervention treatment with indirect bonded restoration using morphology- driven preparation technique.Methods: The preparation technique that can be used is morphology driven, preparation design, while consider bio emulation criteria such as the height of contour, sigmoid nature of enamel dentin interface, and the direction of enamel rods with respect to axial and proximal surfaces. This preparation technique improves resistance by preserving dentin and enamel, also increases bond strength by improving the quality of bonding substrate, marginal integrity and aesthetics.Results: The tooth was inadequately treated about two months ago which made the clinician decided to do an endodontic reintervention. After that, an indirect restoration with lithium disilicate was planned for the patient. The preparation design is a long-wrap overlay, butt-join preparation in the interproximal box (1 mm – 1.2 mm) and rounded inside angles.Conclusion: Restoration after reintervention endodontic treatment with indirect bonded restoration using morphology-driven preparation techniques shows successful treatment with periapical tissue healing and good tissue response.

Abstract

Objective: Immature permanent teeth with open apex can be challenging in endodontic treatment along with its restoration. Several treatment options were available such as apexification, apical barrier technique and regenerative endodontics. This case report aims to describe regenerative endodontics approach in the treatment of immature permanent teeth with wide open apex (blunderbuss) in the maxillary central incisors.Methods: A 9-year-old female referred to Dental Hospital Department of Conservative with chief complaint of fractured upper central incisors due to trauma. Regenerative endodontics procedure was performed on both teeth with calcium hydroxide as intra-canal medicament.Results: The treatment was considered successful with no clinical signs or symptoms, complete apex closure and increased root wall thickness and root length as seen on follow-up radiograph.Conclusion: Regenerative endodontics is the first-choice treatment for immature teeth with pulp necrosis and produce a satisfying clinical outcomes.

Abstract

Introduction: Endo-periodontal lesions are characterized as bacterial infection conditions that cause widespread periodontal tissue destruction and inflammation or pulp necrosis conditions. These lesions occur simultaneously in the periodontal and periapical tissues of a single tooth. Treatment of these lesions requires a multidisciplinary approach that integrates endodontic and periodontal therapy. Knowledge of the pulp and periodontal relationship, the complexity of the root canal system is essential in the management of these cases. The use of periapical radiographs and CBCT are crucial for the success of this treatment.Case:  A second mandibular right molar was diagnosed as a primary endodontic with secondary periodontal lesion, and treated with a combination of endodontic and periodontal therapy. Tooth 47 with Vertucci type II root canal configuration was subjected to multi-visit endodontic treatment, beginning with placement of root canal dressings between visits and final composite resin restoration. Subsequent procedure with scaling, root planning and curettage from Periodontics department. A post-treatment control showed good results.Discussion: Endo-perio lesions are clinical conditions that involve inflammatory conditions in the pulp and periodontal tissues simultaneously. This relationship can occur through anatomical pathways such as the apical foramen and lateral canals, as well as non-physiological pathways due to trauma or improper procedures. Treatment of these lesions requires a multidisciplinary approach to achieve optimal results.Conclusion: The treatment of endo-perio lesions accompanied by Vertucci type II in molar teeth can be completed well, because the dentist is able to perform the examination appropriately and involve multidisciplinary treatment.

Abstract

Introduction: The mandibular first premolars usually have a single root canal, with the presentation of root canal variations being quite rare. Mandibular first premolars with three root canals are very rare. The anomaly occurs during the tooth development phase, causing anatomical variations. Complex root morphology poses a challenge to the dentist in performing treatment. A good and thorough knowledge of the morphology and possible root canal variations can help to obtain adequate treatment.Case Report: A 21-year-old male with a complained of pain in the area of the lower right posterior tooth, accompanied by discomfort when biting about 3 months ago. Objective examination revealed caries on the disto-oclusal of the lower right first premolar. Vitality, percussion, and palpation tests were negative. Periapical radiographs showed a radiolucent area in the disto-oclusal region that had reached the pulp, the number of root canals was 3 and radiolucent in the apical lingual root. The diagnosis for tooth 44 was asymptomatic irreversible pulpitis with asymptomatic apical periodontitis. The root canal configuration was determined to be Vertucci class VIII. Root canal treatment using rotary crown down preparation technique and obturation with warm vertical compaction, class II composite resin restoration.Discussion: Vertucci class VIII is a rare root canal configuration. Successful treatment of complex root canals can be supported by proper radiographic interpretation, understanding of root canal anatomy, adequate preparation, medicament and obturation techniques.Conclusion: Root canal treatment of mandibular first premolars with Vertucci class VIII configuration using warm vertical compaction obturation technique showed good results.
