Abstract

Latar Belakang: Sindrom Koroner Akut (SKA) merupakan masalah jantung penyebab kematian tertinggi di dunia pada tahun 2019 sebesar 17,5 juta kematian atau sekitar 31% dari keseluruhan secara global. sebesar 7,4 juta orang dan diperkirakan akan mencapai 23,3 juta kematian pada tahun 2030 diakibatkan oleh sindrom koroner akut. Tujuan: Mendiskripsikan karakteristik pasien sindrom koroner akut di UGD RS JIH Yogyakarta. Metode: Penelitian ini menggunakan rancangan deskriptif retrospektif. Teknik sampling yaitu total sampling. Jumlah sampel 75 pasien sindrom koroner akut pada bulan Juli – Desember 2020 di UGD RS JIH Yogyakarta. Pengumpulan data menggunakan kuesioner sederhana yang tercatat di rekam medik. Hasil: Usia pasien sindrom koroner akut di UGD Sakit JIH Yogyakarta terbanyak berada pada rentang >60 tahun dengan jumlah 39 pasien (52%). Jenis kelamin terbanyak laki-laki 52 pasien (68%). Tingkat pendidikan terbanyak memiliki tingkat pendidikan perguruan tinggi dengan jumlah 42 pasien (56%). Jenis pekerjaan pasien terbanyak memiliki pekerjaan wiraswasta dengan jumlah 30 pasien (40%). Riwayat penyakit mayoritas hipertensi 46 pasien (61,3%), riwayat pasien merokok dengan jumlah 46 pasien (61,3%). Kesimpulan: Karakteristik pasien sindrom koroner akut di UGD RS JIH Yogyakarta berdasarkan usia >60 tahun, dengan jenis kelamin sebagian besar adalah laki-laki, tingkat pendidikan sebagian besar perguruan tinggi, jenis pekerjaan sebagian besar adalah wiraswasta, dan sebagian besar pasien merokok.

Abstract

Penyakit jantung koroner (PJK) terjadi akibat adanya sumbatan pada arteri koroner sehingga mengakibatkan terganggunya suplai oksigen ke otot jantung. Tanda yang sring muncul adalah nyeri dada atau rasa tidaknyaman yang menjalar ke leher, rahang, bahu sebelah kiri hingga punggung. Durasi nyeri yang timbul pada setiap orang dirasakan berbeda-beda. Pasien dengan PJK memiliki peluang menjalani perawatan ulang dalam waktu kurang lebih 30 hari. Tujuan penelitian ini adalah untuk mengetahui hubungan durasi nyeri pre-hospital dengan lama rawat ini di ruang intensif. Ini merupakan penelitian deskritif korelasi dengan pendekatan cross-sectional. Jumlah sampel yakni 66 responden dengan menggunakan total sampling. Data diperoleh dari rekam medis rumah sakit pada bulan Juli sampai Desember 2019. Selanjutnya, data diolah dengan menggunakan uji Chi square. Hasil penelitian ini menunjukkan bahwa durasi nyeri pre-hospital lebih dari 20 menit pada seluruh responden. Durasi rawat inap pada 49 responden yekni lebih dari 3 hari dan sebanyak 17 responden dirawat kurang dari 3 hari. Hubungan antar variabel menunjukkan terdapat nilai yang signifikan (p < 0.05) dan memiliki keeratan yang kuat. Kesimpulan penelitian durasi nyeri pre-hospital dapat meningkatkan lama rawat inap pada pasien PJK.    Kata Kunci: Penyakit jantung coroner, lama rawat, nyeri, ruang rawat intensif

Abstract

Background: Intravenous access placement is one of the most frequently performed nursing procedures in the Emergency Department. In older adults, age-related physiological changes such as fragile veins, reduced vascular elasticity, and vein collapse increase the risk of failed peripheral intravenous cannulation on the first attempt. Multiple insertion attempts may increase patient discomfort, prolong treatment time, and elevate the risk of complications including phlebitis. The Adult Difficult Intravenous Access (A-DIVA) Scale is a predictive tool developed to identify patients with difficult intravenous access and assist healthcare providers in selecting appropriate insertion strategies. Objective: To describe the implementation of the A-DIVA Scale in improving the success of intravenous access placement among older adults in the Emergency Department of RSUD Kota Yogyakarta. Methods: This descriptive observational study was conducted in the Emergency Department of RSUD Kota Yogyakarta between September and November 2023. Thirty nurses and midwives were recruited using total sampling. Data were collected through direct observation using the A-DIVA Scale and an observation sheet assessing intravenous insertion success. Data were analyzed using descriptive statistics and presented as frequencies and percentages. Results: The first-attempt success rate of intravenous access placement using the A-DIVA Scale reached 96.7% (29 successful procedures), while only one procedure (3.3%) failed. Most patients were classified as having a moderate risk of difficult intravenous access (A-DIVA score 2–3), accounting for 50.0% of the sample. Conclusion: The implementation of the A-DIVA Scale demonstrated a very high success rate in peripheral intravenous access placement among older adults. The instrument may serve as an effective screening tool to identify patients with difficult intravenous access, thereby improving procedural success and supporting patient safety in emergency care. Abstract

Background: Interdialytic weight gain (IDWG) is common in patients with end-stage renal disease (ESRD) undergoing hemodialysis. IDWG should not exceed 4.8% of the patient's dry body weight, or approximately 1.5–2.0 kg. Excessive IDWG can increase morbidity and mortality. Website-based complementary therapies can help patients manage their interdialysis symptoms at any time and on their own.Objective: To design and determine the validity of the website-based complementary therapy education media “Interdialysis Symptom Management” for ESRD patients.Methods: This research used the ADDIE (Analysis, Design, Development, Implementation, Evaluation) approach. This website-based educational media contains five menus: Home, PGTA, IDWG, Complementary Therapy, and Patient Journal. Data were collected through interviews, questionnaires, and expert validation (material, language, and media experts). The study was also piloted on 15 hemodialysis patients at PKU Muhammadiyah Yogyakarta General Hospital. The data were analysed quantitatively.Results: Six experts' content validity results indicated that the media was highly feasible, with CVI scores of 0.92-1.00 (≥0.80). Field tests also revealed that the media was feasible for use, with an average score of 77.75%. The developed media was well received by the patients.Conclusion: Website-based complementary therapy proved to be very feasible and received positive feedback from ESRD patients undergoing hemodialysis.
