EFFECT OF COENZYME Q10 APPLICATION ON POST GINGIVAL CURETTAGE: TRANSFORMING GROWTH FACTOR-β1 LEVEL (TGF-β1) IN GINGIVAL CREVICULAR FLUID Dahlia Herawati

Abstract

Background: Curettage is one of the common periodontitis treatments. Mechanical cleaning of periodontal pocket requires additional materials so the wound healing in the operating area is more optimal. Coenzyme Q10 is one of the ingredients can be used to control levels of Reactive Oxygen Species (ROS). Coenzyme Q10 concentrations of 1:9 and 2:8 in previous studies have been shown to stimulate fibroblast proliferation but its impact on TGF-β1 levels is still unknown. Purpose: to compare the effect of Coenzyme Q10 concentrations 1:9 and 2:8 with PerioQTM on TGF-β1 expression post-curettage. Method: In vivo, study was conducted on 12 periodontitis induced - Oryctolagus cuniculus. Coenzyme Q10 concentrations 1:9 (group I) and 2:8 (group II) were applied post-curettage and the results were compared with PerioQTM (group III). The TGF-β1 was collected from gingival sulcus fluid. It was taken with paper point no. 15 on days 3, -7, and -14 followed by Enzyme-Linked Immunosorbent Assay (ELISA) examination. Data were analyzed with Two-way ANOVA and post hoc test with a significance level of p<0.05. Result: TGF-β1 levels after application of Coenzyme Q10 concentration 1:9 and PerioQTM reached the peak on day 7 and decreased on day 14 while Coenzyme Q10 concentration of 2:8 reached the peak on day 14. Conclusion: The application of these three ingredients affects TGF-β1 levels of gingival sulcus fluid. The expression pattern of TGF-β1 concentration 1:9 is similar to PerioQTM, which reaches a peak at 7 days post-curettage and decreases the day after.  Keywords: Coenzyme Q10, Gingival Crevicular Fluid, Periodontitis-Induced, Transforming Growth Factor-β1, Ubiquinone

Abstract

Abstract Introduction: High labial frenulum attachment affects periodontal tissue to gingivitis and central diastema that interfere aesthetic function, so frenectomy treatment is needed. Frenectomy with convensional technique using blade had a wide wound because attraction of lip muscles and cause a lot of bleeding. Minimal bleeding can be achieved by modifying frenectomy with Incision Below the Clamp (IBC). The objective of this paper was to report frenectomy treatment with IBC technique can minimize bleeding. Case History: A 21-year-old woman came to the Periodontics Clinic of Prof. Soedomo Dental and Oral Hospital UGM with chief complaint of gapped upper front teeth that interfered appearance. After examination, there was aberration labial frenulum superior cause central diastema. Treatment planned before diastema closure was frenectomy with IBC technique using clamp with parallel positition and attached to labial mucosa, then incision under the clamp to base of vestibule and suturing in mucolabial fold. Discussion: Frenectomy with IBC technique made small wounds on the labial mucosa because the attraction of the orbicularis oris muscle laterally was hold back by clamp and suturing immediately after incision will hold muscle attraction after clamp was release and reduced bleeding. Surgery with few bloods will provide a sense of calm and comfort for patient and dentist. Conclusion: Frenectomy for aberrantia frenulum labialis superior using IBC technique completely remove the frenulum and orthodontic treatment can be continued. Abstract

Background: Aberrant frenum can cause many problems such as central diastema, food impaction, dental caries, and aesthetic problems. This case can be handled with frenectomy, which is Z-Plasty technique can be used to treat thick and wide frenulum associated with papillary penetrate and central diastema. Case Report: A 17 years old girl was referred from orthodontist to Department of Periodontics, RSGM UGM Prof. Soedomo Yogyakarta for treatment of maxillary labial frenulum. Intra-oral examination revealed a diastema on the maxillary central incisor of 2.5 mm and frenulum attachment extended to the palatine papilla. In this case, collaborative perio-ortho treatment was carried out, frenectomy with Z-Plasty technique done to correct thick frenulum and fixed orthodontic treatment to correct central diastema after completion of periodontal treatment. Evaluations are carried out every 7 days for the first 2 weeks then at 3 and 6 months after periodontal procedure. Conclusion: Frenectomy using Z-Plasty technique was able to correct wide and thick frenulum that extends to the palatine papilla. Good wound healing, minimal scarring and color matching the surrounding tissue, and perio-ortho collaboration treatment was successfully carried out with satisfactory results according to the patient's wishes.
