Abstract

Rhesus hemolytic disease of the newborn (RHDN) occurs due to alloimmunization of maternal red blood cells.  Complications of RHDN in infants can lead to hyperbilirubinemia, kernicterus, and anemia. In Indonesia, RHDN prevention has not been a common activity so it is very important for pediatricians to recognize early signs of an infant with severe anemia and severe hyperbilirubinemia, to prevent long-term mortality and morbidity. A case with longitudinal observation of long-term growth and development of a child with a history of RHDN has been reported. Throughout the monitoring carried out, the child did not experience serious problems in the medical field but showed disorders or developmental delays that did not improve over time with various therapeutic efforts. Based on a serial monitoring and treatment data that has been carried out on the child, we assumed there has been permanent brain damage caused by hyperbilirubinemia conditions accompanied by co-morbid sepsis. Brain damage is characterized by low DQ values from the beginning, progressive head circumference leads to microcephaly conditions and the presence of brain defects in the form of arachnoid cysts. The management of child development disorders with RHDN to date has not shown encouraging results. Therefore, experts have shifted their focus towards preventive measures, such as intra-uterine fetal transfusion, which has been shown to significantly decrease the occurrence of RHDN-related child development disorders over the past decade.

Abstract

Background: An umbilical granuloma is a common benign condition in neonates characterized by the overgrowth of granulation tissue at the base of the umbilicus following umbilical cord detachment. While the standard treatment involves the use of silver nitrate, with its associated risks and need for clinical visits, alternative treatments such as the application of sodium chloride (NaCl) capsules have gained attention for their efficacy, safety, and cost-effectiveness in home settings. Case Presentation: Herein, we present the case of a 3-week-old female infant diagnosed with an umbilical granuloma post umbilical cord detachment. The management involved the application of a 0.5 gram NaCl capsule directly to the granuloma, covered with sterile gauze, and repeated over several days. This approach resulted in a significant reduction in granuloma size and total resolution within two days, without the complications or irritation sometimes associated with silver nitrate. Parents reported ease of administration at home and high satisfaction levels due to the treatment's effectiveness and minimal need for healthcare visits. Conclusion: The use of a 0.5-gram NaCl capsule offers a valid and effective home-based treatment alternative for umbilical granuloma. It provides a practical option, especially where access to silver nitrate is limited, with the added benefits of ease of use and reduced medical costs. With proper parental education and adherence to the treatment protocol, NaCl can serve as a first-line or alternative therapy. Further large-scale studies are recommended to verify long-term effects and potential complications.

Abstract

Indonesia achieved polio eradication along with several other members of the Southeast Asia Regional Organization (SEARO) in 2014 because it succeeded in overcoming the disease caused by the wild polio virus. Unfortunately since then Indonesia has experienced several reported outbreaks. This literature review aims to analyze the factors causing the re-emergence of poliomyelitis in Indonesia after being declared polio-free in 2014 and effective steps to stop outbreaks and prevent the re-emergence of poliomyelitis. Awareness of health workers and the public regarding polio must be increased so that Indonesia and the world can soon be polio-free. The last two outbreaks occurred in Indonesia in 2022 and 2023. The first outbreak in 2022 was in Pidie Regency, Aceh, Indonesia. In response to the poliomyelitis outbreak, the government immediately launched a simultaneous vaccination program. Since then no cases of poliomyelitis have been found until another case was found in December 2023 in Solo, Central Java, Indonesia. Once again, to respond to the poliomyelitis outbreak, the government held a simultaneous vaccination program in three districts, namely Pamekasan and Sampang, East Java and Klaten, Central Java. Followed after that by all regions in Central Java and East Java. It is expected that this quick response from the government can stop the outbreak again. The target of SubPIN (National Immunization Subweek) is to break the chain of transmission, so it must ensure that Outbreaks Response Immunization (ORI) coverage is high at least 95% and evenly distributed throughout the region. Apart from that, information to the public regarding the dangers of disease and the importance of complete vaccination to prevent disease must continue to be provided.

Abstract

Based on WHO (World Health Organization) data in November 2023, Indonesia was still ranked in the top two cases of Tuberculosis (TB) in the world. The increase in patients with resistance to Anti Tuberculosis Drugs (ATD) available in Indonesia was increasing. TB infections not only happened in adults but children were also a group that was vulnerable to TB infection. The central and regional government had prepared various strategies to archive the elimination of TB in Indonesia by 2030.The strategies that had been prepared by the Indonesian government require support from the entire community, especially health workers. Exposed to TB elimination program had been carried out by both central and regional governments. Various trainings regarding the recognition of TB disease, therapy and programs to increase TB detection had been held by the government. How ever not all levels of society, even health worker, were exposed to this TB elimination program. Until 2024 the TB elimination program is still focused on increasing the number of TB cases detection. This shows that strong efforts from the government are still needed to achieve TB elimination. The government need to be more aggressive in disseminating information about TB elimination programs through mass media, conducting seminars or training for health workers and the community. It is hoped that this action can increase case findings and cure rate of TB by as much as 90 percent so the incidence of infection can decrease significantly in the next couple years.
