Immunization is a measure to prevent infectious diseases by administering vaccines to induce immunity against those diseases. Satisfaction is a feeling arising from comparing the actual outcome which leaves a lasting impression with prior expectations. This study aimed to determine the relationship between the Posyandu (Integrated Health Post) service satisfaction index and immunization coverage among children under five in Lam Neuheun Village, Aceh Regency. An observational study design with a cross-sectional approach and quantitative survey methods was employed. The study population consisted of mothers with children aged 0–59 months, with a total of 38 respondents. Data were collected using a structured questionnaire and analyzed using the Chi-Square test. The results showed that all indicators of Posyandu service satisfaction such as service requirements, procedures, timing, costs/fees, service specifications/types, provider competence, provider behavior, handling of complaints and suggestions, and facilities and infrastructure had a significant relationship with immunization coverage status (p<0,05). Generally, the community held a positive perception of, or was satisfied with, Posyandu services. However, satisfaction with services was not fully matched by high rates of complete immunization coverage. Low immunization coverage was also influenced by family factors, knowledge, perceptions regarding immunization safety, and socio-cultural factors. Thus, this study demonstrates that community satisfaction with Posyandu services is a crucial factor in health service delivery. Posyandu is expected to maintain and further improve the quality of services that the community has already rated positively. Beyond providing health services, Posyandu is also expected to play a more active role in educating the public about the benefits of immunization, vaccine safety, and the management of mild post-immunization side effects, ensuring that the community receives accurate information and is not easily swayed by misinformation.
