Abstract

As life expectancy increases, there is more elderly population and incontinence cases that require long-term catheter. Long-term use of catheters can cause morbidity for patients, because they have to come to the hospital every 2 weeks on weekdays to change the catheter, thus compromising their work or if they are elderly who need to be accompanied by their families, it can burden their families. Improper catheter care can also cause urinary tract infections (UTIs). Therefore, a solution is needed. Clean intermittent catheterization (CIC) or also called self-catheterization can be a solution for this problem. The use of CIC can make patients do not have to carry tubes and catheter bags everywhere. Patients also do not need to change the catheter every 2 weeks, therefore are more efficient in terms of cost and time. We gave CIC education to long-term catheter users at the Urology Polyclinic of Tabanan Hospital using a urinary tract mannequin. The outcome revealed that all patient's post-test scores increased compared to the pre-test. The patients can apply CIC independently so that they no longer change catheters at the hospital, and no urinary tract infections occur based on urinalysis.

Abstract

Benign Prostatic Hyperplasia (BPH) or benign prostate is a disease caused by swelling or an increase in the prostate gland. BPH occurs due to an increase in the volume of the prostate, the prostate is a solid organ located under the bladder, in front of the rectum and encloses the posterior urethra. One of the clinical manifestations of BPH is Lower Urinary Tract Symptoms (LUTS). To determine the degree of severity of these symptoms can be measured using the International Prostate Symptom Score (IPSS) questionnaire. This study aims to determine the relationship between prostate volume and the degree of IPSS in BPH patients with LUTS at the Urology Polyclinic of Tabanan Hospital. Sampling using consecutive sampling technique with the type of observational analytic research with cross-sectional techniques obtained as many as 40 respondents. The data obtained were analyzed univariately and bivariately using the gamma corelations test method with a 95% confidence level (p <0.05) using SPSS 25.0. The results obtained are as follows: there is a significant relationship between prostate volume and the degree of LUTS (p = 0.006) with the strength of the relationship in the strong category (r = 0.618). The conclusion is that prostate volume and IPSS degree have a significant relationship.

Abstract

Benign Prostatic Hyperplasia (BPH) merupakan kondisi yang sering terjadi pada pria lanjut usia dan berhubungan dengan proses inflamasi kronis di prostat. Inflamasi dapat mempercepat pertumbuhan sel prostat dan memperburuk gejala BPH. Pola makan tinggi lemak jenuh, gula, dan rendah serat dapat meningkatkan inflamasi, sedangkan pola makan kaya antioksidan dan senyawa anti-inflamasi dapat membantu menekan inflamasi. Oleh karena itu, penerapan pola makan anti-inflamasi menjadi pendekatan non-farmakologis yang potensial dalam pengelolaan BPH. Kegiatan pengabdian masyarakat ini dilakukan dengan metode penyuluhan dan pelatihan kepada pasien BPH di Poli Urologi RSUD Tabanan. Materi yang diberikan meliputi edukasi mengenai BPH, peran inflamasi, serta prinsip-prinsip pola makan anti-inflamasi. Selain itu, peserta mengikuti sesi pelatihan memasak untuk meningkatkan keterampilan dalam mengolah makanan sehat. Pendampingan berkala dilakukan untuk memantau kepatuhan pasien dalam menerapkan pola makan yang disarankan. Kegiatan ini diikuti oleh 30 pasien BPH dan keluarganya. Hasil evaluasi menunjukkan adanya peningkatan nilai post-test dibanding pre-test, yang mengindikasikan peningkatan pemahaman peserta terhadap pola makan anti-inflamasi. Selain itu, pasien melaporkan perubahan positif dalam pola makan mereka, termasuk peningkatan konsumsi sayuran, buah-buahan, dan sumber protein nabati. Edukasi dan pendampingan pola makan anti-inflamasi terbukti meningkatkan pemahaman pasien tentang pengaruh nutrisi terhadap inflamasi dan gejala BPH. Program ini diharapkan dapat diterapkan secara berkelanjutan untuk membantu pasien mengelola kondisi mereka secara lebih optimal melalui pendekatan nutrisi yang berbasis ilmiah.
