<![CDATA[ABSTRAKBerdasarkan hasil identifikasi Tim pengabdi bahwasanya pengisiannya formulir rekam medis rawat jalan tidak sesuai dengan pedoman. ketidaksesuaian tersebut meliputi ketidak lengkapan pengisian data sosial pasien, penulisan nomor rekam medis diluar dari balok box yang telah disediakan, serta kurangnya elemen data yang diperlukan dalam pengisian formulir pemeriksaan umum rawat jalan. Adapun tujuan dilakukannya pengabdian masyarakat di Klinik Modern Poncokusumo Malang untuk memberikan edukasi terkait desain formulir rawat jalan. Kegiatan ini dilaksanakan selama 3 hari dengan peserta yang terlibat adalah petugas pendaftaran dan direktur Klinik Modern dengan jumlah 4 orang dengan metode sosialisasi dan diskusi interaktif. Pengabdian masyarakat diawali dengan memberikan edukasi terkait pentingnya formulir rawat jalan, selanjutnya memberikan sosialisasi terkait menganalisis formulir rawat jalan dan edukasi yang terakhir yaitu membuat desain formulir berdasarkan keilmuan yang sudah di berikan sebelumnya. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan peserta yang menunjukkan peningkatan sebesar 45% dari nilai pretest yaitu 50.00 dan nilai postest yaitu 95.00. Adanya peningkatan pengetahuan dan peserta dalam kegiatan ini dapat membantu dalam meningkatkan kemampuan petugas pendaftaran dalam membuat desain formulir yang sesuai dengan pedoman keilmuan rekam medis. ABSTRACTBased on the identification results of the service team, filling out the outpatient medical record form was not in accordance with the guidelines. These discrepancies include incomplete filling of the patient's social data, writing the medical record number outside of the box provided, and the lack of data elements needed in filling out the outpatient general examination form. The purpose of doing community service at the Modern Poncokusumo Clinic in Malang is to provide education regarding the design of outpatient forms. This activity was carried out for 3 days with participants involved being registration officers and the director of the Modern Clinic with a total of 4 people using interactive socialization and discussion methods. Community service begins with providing education regarding the importance of outpatient forms, then providing socialization related to analyzing outpatient forms and education, the last is making a form design based on the knowledge that has been given before. The results of the activity showed an increase in the participants' knowledge which showed an increase of 45% from the pretest score, which was 50.00 and the posttest score, which was 95.00.
