Abstract

Pulmonary atresia with an intact ventricular septum is a condition that is characterized by a complete obstruction to right ventricular outflow. This condition is fatal if untreated. Ductus arteriosus communicate between pulmonary artery and proximal descending aorta. It is essential in fetal life as a communicating shunt to send blood from right ventricle bypassing the underdeveloped lungs. Ductal patency may be lifesaving in few cyanotic congenital heart diseases to maintain pulmonary blood flow.Case Illustration : We present a case of a 1‑day‑old, 3405 g child who presented to casualty with bluish discoloration. The child had central cyanosis, tachypnea with saturation 80% on 1 L/minute nasal cannula and desaturate to 40%. Transthoracic echocardiography (TTE) revealed intact intraventricular septum with pulmonary atresia and patent ductus arteriosus (PDA). The patient scheduled to an emergency PDA stenting. A heating blanket is installed on the operation table, oxygen saturation with Pulse‑oxymeter, noninvasive blood pressure, end-tidal CO2 monitor, and temperature monitoring were done. Anesthesia induction was performed with 50% oxygen in air, fentanyl 3 µg/kg, ketamine 2 mg/kg, atracurium 0.5 mg/kg and maintained with sevoflurane. The procedure was performed about 20 to 30 minutes. After procedure, oxygen saturation was increased up to 60%. The child was successfully extubated after 24 hours in the Neonatal Intensive Care Unit (NICU) with saturation 92%.Conclusion: PDA stenting is a less invasive palliative procedure for ductal dependent complex congenital heart diseases. Anesthesiologist's role were maintaining arterial saturation and hemodynamics, securing vascular assess, maintaining temperature. It needs thorough understanding of underlying physiology.

Abstract

Obstetric emergencies are a challenge both for the obstetrician and the anesthesiologist. The incidence of caesarean sections as per the National Family Health Survey published in 2015–16 was 17.2%. In 7.6% of cases, the decision to conduct a caesarean section was taken after the onset of labour pain. Caesarean sections are classified depending on the urgency into four categories. The target decision to delivery interval for category 1 caesarean section is less than 30 min. The management of these emergencies involves a rapid assessment with minimal investigations. Although general anaesthesia is considered to have higher morbidity and mortality, category 1 caesarean sections may still warrant this technique. Rapid sequence spinal anaesthesia is replacing general anaesthesia for category 1 SC emergency. In category 2 and 3 caesarean sections, spinal anaesthesia still remains the technique of choice

Abstract

Hapkido merupakan salah satu cabang olahraga beladiri yang berasal dari Korea dan belum lama populer di Indonesia. Saat ini Hapkido masih merupakan cabang ekshibisi, namun akan dipertandingkan secara resmi pada PON XXI Aceh-Sumatera Utara. Cedera olahraga merupakan risiko yang tidak dapat dihindari dalam olahraga Hapkido, terbukti dengan adanya 2 atlet yang mengalami patah tulang dan lebih dari 20 atlet yang mengalami sprain dan strain pada ajang ekshibisi di Papua tahun lalu. Masalah utama yang dihadapi adalah rendahnya pengetahuan dan kemampuan pelatih dalam penanganan cedera olahraga, padahal intervensi awal yang tepat dapat mengurangi risiko komplikasi dan mempercepat proses penyembuhan. Tujuan kegiatan ini adalah untuk meningkatkan pengetahuan dan keterampilan pelatih Hapkido DIY dalam mencegah dan melakukan pertolongan pertama pada cedera olahraga. Metode pelaksanaan dilakukan melalui pelatihan dengan pendekatan ceramah dan diskusi case study yang diikuti oleh seluruh pelatih Hapkido DIY. Evaluasi keberhasilan dilakukan dengan pengukuran tingkat kehadiran dan pengetahuan peserta melalui pre-test dan post-test. Kegiatan dilaksanakan di pusat latihan Hapkido DIY pada tanggal 25 Januari 2024. Hasil kegiatan menunjukkan peningkatan skor pengetahuan pada seluruh peserta sebesar 50,67% dari rerata pre-test 39,13 menjadi post-test 58,96. Selain itu, diperoleh rerata skor persepsi peserta sebesar 84,82 yang menunjukkan respon positif terhadap program pelatihan. Program ini telah berhasil meningkatkan pengetahuan pelatih Hapkido DIY dan diharapkan dapat dilaksanakan secara berkesinambungan untuk menjamin keselamatan atlet di masa mendatang.
