Abstract

Covid-19 is an acute infection of the respiratory tract with a variety of clinical manifestations and severity. Lymphocyte count correlates to the severity of Covid-19. This study aims to find the relationship between lymphocyte count, mortality, and length of stay in COVID-19 patients. A retrospective cohort observational studies were conducted using medical records of confirmed COVID-19 patients in RSUP Dr. Sardjito Hospital between 1st January 2021 to 31st December 2021. The lymphocyte count cut-off point as a mortality factor was determined with the ROC curve and Youden’s index. Survival analysis was done using Kaplan Meier to investigate the relation of lymphocyte count to mortality and length of stay. The correlation between lymphocyte count and other factors affecting mortality and length of stay was analyzed with Cox regression. There were 217 subjects who fulfilled the inclusion and exclusion criteria. The cut-off point of lymphocyte count was set at 1.06,103 cells/μL. A total of 121 subjects have a lymphocyte count of <1.06,103 cells/μl. Higher lymphocyte counts (≥1.06,103 cells/μl) showed a reduced risk of mortality (HR 0.570; 95% CI 0.403 – 0.807, p=0.002). The duration of hospitalization was shorter in the group of patients with lymphocyte count <1.06,103 cells/μL OR 0.802, 95% CI 1.032-2.646; p=0.110) with a median of 128.77 hours (12.84 – 983.99 hours), which might be explained by higher mortality (p = 0.000) in the low lymphocyte patients’ group (64.8%). Low lymphocytes <1.06,103 cells/μL in COVID-19 patients is independently and significantly associated with increased risk of mortality and insignificantly associated with shorter length of stay.

Abstract

Hasil: Total subjek berjumlah 122 pasien, 12 drop out.

Abstract

In Indonesia, there are many traditional foods that have almost disappeared from society. Because many contemporary food trends are appearing in society one after another, so traditional food has the potential to disappear and be forgotten by local people. Therefore, we took the initiative to preserve the local wisdom of traditional food typical of Sundanese, West Java, namely roasted rice. We will market this traditional food throughout Indonesia, especially on the island of Java. This grilled rice will be different from grilled rice in general. By creating and innovating grilled rice with various menu variations that are liked by all groups from children, teenagers, to adults, such as varying it with good quality chicken, sausages, meatballs and fish. By providing menu variations and new flavors in processing this grilled rice, we aim that this grilled rice food will be different from grilled rice in general. However, it will be a new high quality grilled rice with various variations and unique flavors that will be able to maintain the existence of grilled rice as a hitz food for Indonesian millennials and can be enjoyed by various groups. Seeing the opportunities from this business, we are sure to develop the simple grilled rice business to become better known to the wider community. Keywords: Grilled rice, innovation, menu variation

Abstract

Background: Stroke is one of the main causes of death worldwide. Although neurological complications are the leading cause of death, half of acute stroke patients may die from other medical complications, including pulmonary infections. Case Presentation: It has been reported that a patient aged 65 years with ischemic stroke was treated in the ICU accompanied by diabetes mellitus, acute kidney injury, and hypertension. During treatment in the ICU, the patient experienced complications of pneumonia. The patient underwent endotracheal intubation and ventilator installation as well as a percutaneous dilatational tracheostomy (PDT). During treatment, the patient received antibiotic therapy, blood sugar regulation with insulin, and administration of antihypertensive drugs. The patient experienced clinical improvement and was discharged from the ICU after 18 days of treatment and was subsequently treated in the HCU. Conclusion: The length of stay of stroke patients in the ICU requires attention, especially the problem of nosocomial infections that may accompany the patient during the treatment. If there is an infection or other complications, an appropriate selection of antibiotics and supportive therapy is necessary during treatment.

Abstract

Fluid therapy is one of the most essential things in managing critical patients, such as ICU patients. Although it seems simple, this is difficult to do in this group of patients. The fluid needs of ICU patients vary according to the course of the disease. Therefore, fluids must be given according to individual needs, and each phase of the disease must be reassessed. To support this, there is a conceptual model that explains fluid administration based on the phases of the disease that the patient is going through. The ROSE concept (resuscitation, optimization, stabilization, and evacuation) describes the phases of a patient's illness and how fluids should be administered. In the resuscitation phase, the goal is lifesaving and is achieved by positive fluid balance. In the optimization phase, fluid balance is neutral and aims to save organs. In the stabilization phase, the fluid balance has started to move in a negative direction and aims to support the organs. Finally, in the evacuation phase, fluid balance is negative and organ repair has occurred. By implementing this model, it is hoped that ICU patients will have better outcomes
