Abstract

Good medical record number numbering is one of the keys to the success or goodness of medical record management of a health service, of course, if supported by a good system. Quality human resources and good procedures or work procedures as well as adequate facilities or facilities. The special purpose is to identify the policy of the medical record numbering system, identify the factors causing the duplication of medical record numbers. One patient file is accompanied by one medical record number. The purpose of medical record numbering is to distinguish one patient's medical record from another, but in Madani Hospital there are still duplications of patient medical record numbers that can cause health services to be disrupted and the patient's disease history is not well documented, this study aims to determine the number of duplicated medical record numbers at Madani Hospital. This research uses a type of descriptive research, namely research.

Abstract

To manage a medical records unit and produce quality health information, adequate resource support is needed, including human resources, facilities and infrastructure. The aim of this research is to determine the management of human resources in the medical records unit in improving the efficiency and effectiveness of services at the UPT Puskesmas Kedai Durian. This research uses a descriptive qualitative research type with a cross sectional approach where data collection was carried out by interviews in September 2022 with employees of the medical records unit at the Toko Durian Health Center, Medan City with the number of officers in the medical records unit being 4 people, including 1 D3 medical records person, 1 person D3 nursing, 1 person D4 midwifery, and 1 person S1 nursing. Based on research results, in managing human resources in the medical records unit to improve the efficiency and effectiveness of services at the UPT Puskesmas Kedai Durian, the majority of employees in the medical records unit do not have a medical records background and have never attended previous training. In conclusion, the UPT of the Kedai Durian Health Center in Medan City is still not efficient and effective in providing services in the Medical Records Unit.

Abstract

Privacy, confidentiality and release of information are communications containing certain confidential information provided by patients to their doctors to be kept confidential, unless the patient has given written consent to release the information from their medical records. The aim of this research is to determine the procedures for the medical record information release system in ensuring the legal aspects of confidentiality at RSU. Imelda Indonesian Worker. This type of research is descriptive, namely a research method carried out with the aim of creating an objective picture of a situation. Research time is from January to August 2023 at RSU. Imelda Indonesian Worker. The subjects in this research were medical records officers, the research informant was the head of medical records. From the results of the research, it is known that the procedures for the release of medical record information system run in accordance with SPO number 002/SPO/MRMIK/2022, there is no specific SPO regarding the confidentiality of medical records but confidentiality has been carried out well in accordance with the SPO for the release of medical record information and in line with the 24 year Health Ministerial Regulation. 2022 regarding medical records, at RSU. Imelda, an Indonesian worker, releases a lot of information for insurance. The obstacle in releasing medical information is patients/families who do not bring a power of attorney and identification, where the patient's family does not understand the importance of a power of attorney. It is hoped that hospitals will create special SOPs regarding the confidentiality of medical records and provide information to patients/families about the requirements and procedures for releasing medical information by placing banners or posters in the information section or other strategic places.

Abstract

Legally protecting the rights of patients participating in the Health Social Security Administering Agency (BPJS) in obtaining health services, the government issued Law Number 24 of 2011 which determined that two State-Owned Enterprises, namely PT Askes (Persero) and PT Jamsostek (Persero) were changed to Public Service Materials to implement the five programs mandated by Law Number 40 of 2004, namely the Health Insurance program for BPJS Health and other programs are submitted to BPJS Employment. Hospitals in providing health services to BPJS participants are hampered by the lack of facilities and infrastructure in accordance with applicable standards. This research uses a descriptive research method with a qualitative approach, namely research conducted to systematically, factually and accurately describe or explain the facts and characteristics of a particular population. Based on the research, it was concluded that the legal protection of patient rights at RSU Imelda Pekerja Indonesia Medan was categorized as good. This is proven by the fulfillment of hospital rules and regulations, information on patient rights and obligations, information on diagnoses and procedures for medical procedures from health workers, patient complaints/complaints provided by the hospital.

Abstract

Pelayanan tentang kebidanan sudah ada sejak dahulu. Banyaknya perkembangan kesehatan sehingga banyak juga tempat praktik kesehatan seperti bidan. Dalam hal ini sering didapati atau terjadinya malpraktik kebidanan yang merugikan masyarakat. Menurut berita yang ada praktik bidan tertentu banyak yang tidak sesuai dengan prosedur penerbitan ijin praktek sehingga timbul malpraktik yang merugikan konsumen/masyarakat. Tujuan dari pengabdian ini adalah memberikan Sosialisasi kepada bidan tentang mal praktik kebidanan ditinjau dari aspek hukum. Hasil dari pengabdian ini adalah bidan menyadari akan bahaya dari asuhan yang diberikan kepada pasien tidak sesuai dengan standar dan etika profesi, dan terjadi perubahan perilaku bidan dalam memberikan asuhan kebidanan setelah dilakukan pengabdian

Abstract

Kesehatan masyarakat merupakan hak yang fundamental, dan kepercayaan terhadap penyediaan layanan kesehatan sangat krusial. Malpraktik medis menjadi ancaman serius terhadap kepercayaan ini, menciptakan kerentanan bagi pasien dan merusak integritas sistem kesehatan. Penelitian ini menyajikan sebuah proyek penyuluhan hukum kesehatan yang bertujuan untuk memberikan pemahaman lebih dalam kepada masyarakat tentang hak-hak mereka dan perlindungan hukum terkait malpraktik medis.Melalui serangkaian kegiatan penyuluhan yang melibatkan seminar, lokakarya, dan distribusi materi informatif, masyarakat diarahkan untuk mengenali dan memahami hak-hak pasien serta proses hukum yang dapat diambil dalam kasus malpraktik medis. Hasil penelitian menunjukkan peningkatan yang signifikan dalam pemahaman masyarakat terhadap hak-hak pasien dan tata cara hukum yang terlibat.Proyek ini juga mencatat peningkatan jumlah pelaporan kasus malpraktik medis, mencerminkan keberhasilan dalam mendorong masyarakat untuk berpartisipasi aktif dalam melindungi diri mereka sendiri. Kolaborasi yang baik antara ahli hukum kesehatan, praktisi medis, dan organisasi masyarakat sipil memberikan dasar yang kuat untuk pendekatan holistik dalam meningkatkan kesadaran masyarakat.Penelitian ini memberikan implikasi yang signifikan terhadap transparansi dan akuntabilitas dalam pelayanan kesehatan, dengan harapan bahwa peningkatan pemahaman masyarakat akan memberikan kontribusi pada perubahan positif dalam kualitas pelayanan kesehatan dan kepercayaan masyarakat terhadap sistem kesehatan. Dengan demikian, proyek ini tidak hanya memberikan pemahaman hukum kesehatan yang lebih baik, tetapi juga membuka pintu bagi partisipasi aktif masyarakat dalam melindungi hak-hak mereka dalam konteks pelayanan kesehatan

Abstract

Banyak tenaga kesehatan dan pengelola fasilitas kesehatan yang belum sepenuhnya memahami aspek hukum terkait praktik mereka. Ketidaktahuan ini dapat mengakibatkan pelanggaran hukum, ketidakpatuhan terhadap standar etika, dan penurunan kualitas pelayanan. Oleh karena itu, edukasi tentang hukum kesehatan sangat penting untuk memastikan bahwa layanan kesehatan diberikan dengan cara yang aman, efektif, dan sesuai dengan hukum. Pelatihan ini bertujuan untuk meningkatkan Pemahaman Hukum Kesehatan, Membantu RSU IPI dalam memahami dan menerapkan hukum kesehatan yang relevan untuk meningkatkan kualitas pelayanan. Mengurangi risiko pelanggaran hukum yang dapat mengakibatkan sanksi atau dampak negatif bagi pasien dan penyedia layanan kesehatan. Metode: pelatihan ini meliputi seminar, pelatihan, dan lokakarya yang diadakan di RSU IPI Medan. Metode pengajaran mencakup presentasi oleh ahli hukum kesehatan, studi kasus, dan sesi tanya jawab untuk mengatasi masalah konkret yang dihadapi oleh peserta. Pelatihan ini diharapkan Tenaga medis memiliki pemahaman yang lebih baik tentang hukum kesehatan dan bagaimana menerapkannya dalam praktik sehari-hari. Penerapan hukum kesehatan yang tepat akan berkontribusi pada peningkatan kualitas pelayanan, kepuasan pasien, dan kepatuhan terhadap standar etika, Fasilitas kesehatan dapat mengurangi risiko hukum dan meningkatkan akuntabilitas dalam praktik mereka.

Abstract

RL-4b adalah laporan standar untuk data keadaan morbiditas pasien rawat jalan yang merupakan laporan rekapitulasi dari jumlah kasus baru dan jumlah kunjungan yang terdapat pada unit rawat jalan rumah sakit untuk tahunan. Data RL-4b dikumpulkan dari tanggal 1 Januari sampai dengan 31 Desember setiap tahunnya. Tujuan dari penelitian ini adalah untuk mengetahui faktor - faktor penyebab keterlambatan pengiriman laporan RL-4b pada Aplikasi SIRS Online di RSIA Artha Mahinrus. Jenis penelitian yang digunakan adalah penelitian deskriptif dengam pendekatan kualitatif yaitu penelitian yang pengumpulan datanya dilakukan dengan maksud untuk menggambarkan atau mendeskripsikan tentang suatu keadaan secara objektif.Cara pengumpulan data yang dilakukan oleh peneliti yaitu dengan cara observasi langsung ke lokasi penelitian dan wawancara kepada subjek penelititan, wawancara yang dilakukan berupa daftar pertanyaan yang diajukan oleh peneliti kepada subjek penelitian. Subyek dari penelitian ini adalah 4 orang, yang terdiri dari 1 orang kepala rekam medis dan 3 orang petugas rekam medis.RSIA Artha Mahinrus perlu menyediakan dana supaya bisa mengirimkan petugas pelaporan untuk mengikuti sosialisasi atau pelatihan ke Dinas Kesehatan mengenai pengiriman laporan RL-4b Pada Aplikasi SIRS Online. RSIA Artha Mahinrus juga perlu membuat lebih detail lagi SOP pelaporan RL-4b terkait langkah- langkah untuk pengisian formulir RL-4b. RSIA Artha Mahinrus juga perlu untuk memperbaiki jaringan yang ada di RSIA Artha Mahinrus supaya disaat ingin mengirimkan laporan RL-4b ke Dinas Kesehatan tidak terjadi kendala.

Abstract

Stroke is a neurological disease that can cause physical, emotional, and cognitive impairment in sufferers. Recovery of cognitive function in post-stroke patients requires an active role for the family as the primary caregiver in the rehabilitation process at home. This study aims to explore family readiness in caring for post-stroke patients. The research method used a qualitative approach with a phenomenological design. Data collection was conducted through in-depth interviews with five participants selected using a purposive sampling technique at the Imelda Buruh Indonesia General Hospital in Medan. Data analysis used thematic analysis through a process of coding, categorization, and theme determination. The results of the study identified five main themes, namely: (1) family knowledge about post-stroke patient care, (2) family skills in meeting the patient's basic needs, (3) family readiness in supporting cognitive function recovery, (4) utilization of health services, and (5) barriers to care, such as limited time and emotional burden. The findings indicate that families play an important role in the patient's recovery process, but still face limited knowledge and challenges in implementing optimal care. Therefore, ongoing education and support from health workers are needed to improve family readiness in caring for post-stroke patients at home.

Abstract

Medical discharge summaries are essential documents in healthcare services that provide a comprehensive overview of a patient's clinical condition throughout hospitalization. The consistency of completing medical discharge summaries within an Electronic Medical Record (EMR) system plays a crucial role in ensuring documentation quality, patient safety, and the efficiency of administrative processes and health insurance claims. This study aimed to explore healthcare workers' experiences in maintaining consistent medical discharge summaries within the Electronic Medical Record system at Imelda Pekerja Indonesia General Hospital. A qualitative research design with a phenomenological approach was employed. The participants consisted of the Attending Physician (DPJP), the Head Nurse of the Emergency Department, medical record officers, Case Mix officers, and Hospital Information System (HIS) officers. Data were collected through in-depth interviews and analyzed using data reduction, data display, and conclusion drawing techniques. The findings revealed that the consistency of medical discharge summary documentation was influenced by three major factors: organizational support through training and socialization programs, healthcare workers' understanding of clinical documentation standards and Standard Operating Procedures (SOPs), and hospital policies supporting the implementation of Electronic Medical Records. In addition, workload, differences in perceptions regarding documentation completeness, and technical issues within the EMR system remained significant challenges affecting documentation consistency. The study concludes that strengthening healthcare workers' competencies, optimizing validation features within the Electronic Medical Record system, and implementing continuous monitoring and evaluation are necessary to improve the quality of medical documentation and hospital healthcare services.
