Abstract

Background: Oral rehabilitation procedures such as crown lengthening, implants for anterior teeth, and endodontic procedures with limited tooth structure require provisional restoration for a long time. The material used in fabricating provisional fixed partial dentures is polymethyl-methacrylate (PMMA). PMMA is divided into heat cured and self-cured based on the polymerization. Heat-cured PMMA has better mechanical properties than self-cured PMMA. Despite its advantages, however, there are also some disadvantages, such as high water sorption, low color stability, and wear of use. Research on the use of eggshells as reinforcement in denture bases has been conducted. However, the use of eggshells in provisional fixed partial dentures has not been done yet. Eggshell, as an environment by-product waste material, is composed of 95% calcium carbonate (CaCO3), magnesium, potassium, and other inorganic components, can be used as alternative natural reinforced materials for PMMA provisional restoration.Objectives: To describe use of provisional restoration for a long time, PMMA as provisional restoration material, as well as the merit of eggshell as natural reinforcement material in water sorption and color stability of heat-cured PMMA provisional fixed partial dentures.Conclusion: Eggshell, which is composed of calcium carbonate, can be converted into hydroxyapatite as a reinforcement in long term provisional restoration, could improve the physical properties of heat-cured PMMA, such as water sorption and color stability.

Abstract

Cleft lip and palate (CLP) are one of the birth defects commonly found in Medan and surrounding areas with approximately 150 cases handled annually at Mitra Sejati and Grand Med Hospitals. Generally, labioplasty surgery is the only given treatment, but fabrication of the feeding plate before or after surgery is important to cover the palatal defect. From the prosthodontics aspect, fabrication after surgery cannot be performed because most parents do not know the importance of the feeding plate. To overcome this problem, community service was conducted by fabricating feeding plates that will be given 1 week after surgery. This was achieved by getting information about patients with CLP, general examination by paediatricians, the impression of the oral cavity in the operating room before surgery, outline design, and feeding plate fabrication at the dental laboratory. Subsequently, the insertion was carried out to evaluate whether infants can drink normally using a bottle. A total of 18 infants with CLP recognized the benefits of the feeding plate in Mitra Sejati and Grand Med Hospitals. The education given had a positive effect on knowledge about the importance of feeding plate and motivation for its use to make infants drink normally and gain weight according to age.

Abstract

The most common temporomandibular disorder (TMD) is disc displacement with reduction (DDWR). Clicking as a characteristic symptom of DDWR requires conservative treatment such as occlusal splint. However, other accompanying symptoms such as pain in the masticatory muscles require additional treatment like physiotherapy. This paper aims to describe DDWR treatment using a stabilization splint and physiotherapy using infrared lamp therapy. A 33-year-old male came to the Prosthodontics Clinic with chief complaints that there is soreness on right jaw when he wakes up in the morning and was aware of it since a year ago. The patient has a habit of teeth grinding during sleep. Clinical examination, a sound on the right and left jaw joint when opening and closing the mouth, mouth opening without pain of 50.5 mm with deviation to the right, pain in the muscles around the face, especially the masseter, temporalis and trapezius. Radiograph shows the position of the right and left mandibular condyles an-terior to the articular tubercle. A stabilizing splint is used for symptomatic treatment and physiotherapy is carried out in the form of infrared lamp therapy. It is concluded that the combination of stabilizing splint and infrared lamp therapy can reduce pain and clicking in TMD’s patients.

Abstract

A maxillofacial prosthesis (MFP) was inserted to replace the hard and soft tissues and teeth lost after maxillectomy surgery in a case of ameloblastoma. Facial deformity, soft tissue invasion and limited mouth opening caused difficulties in the moulding pro-cess. This article presents anatomical moulding modifications to improve treatment success in MFP placement. A 19-year-old female came to RSGM USU referred from RSU Murni Teguh for immediate post-surgical placement of a maxillary closure device. The patient was diagnosed as ameloblastoma with facial deformity, and tissue invasion on the left side. The patient's facial con-dition was asymmetrical and difficult to open the mouth, making impression difficult. Anatomical impression was performed with modification and selection of appropriate stock tray, followed by fabrication of surgical obturator MFP and Class IV Aramany in-terim as prosthetic rehabilitation device. It was concluded that MFP insertion in post-maxillectomy patients with impression dif-ficulties, modification of the stock tray moulding spoon can determine the success of treatment.

Abstract

Introduction: Osteoarthritis (OA) is a disorder of the temporomandibular joint which results in permanent changes to TMJ. Scissors bite and bad habits are one of the etiological factors of OA. OA causes deviation when opening the mouth, limitations of mouth opening, and joint crepitus sounds. Use of stabilization splints (SS), physical therapy, and consumption of chondroitin sulfate-glucosamine supplements aimed at reducing joint pain, relaxing TMJ muscle, and preventing joint damage. Case report: A 22-year-old female patient came to North Sumatera of Dental Hospital with complaints stiff jaw when opening her mouth, especially in the morning. Complaints of pain occurred since skelling treatment 3 years ago. Clinical examination showed scissors bite and a habit of chewing on the right side. Palpation of the right anterior temporalis muscle revealed familiar pain and joint crepitus sound. Based on DC/TMD assessment, the patient had OA where the mouth opening is 35 mm without pain and 38 mm with pain accompanied by a left deviation of 1.5 mm. On CBCT examination there was flattening on the TMJ surface. Treatment is carried out by using SS every day and consuming chondroitin sulfate-glucosamine supplements once a day accompanied by self-therapy. Self-therapy is carried out alone by practicing opening and closing the jaw for 15 minutes every day. After 6 months is obtained without assisted mouth opening of 36 mm to 41 mm. There is no pain on palpating muscles but there is still a crepitus sound when opening and closing the mouth. Conclusion: Treatment of OA with SS, self-therapy, and consumption of Chondroitin Sulfate-Glucosamine supplements is effective in reducing TMJ joint pain and muscle stiffness through chondroitin stimulation mechanism so that prevents further degenerative processes. Abstract

Backgrounds: Provisional fixed partial denture (FPD) is an important procedure in prosthetic treatment such as crown or bridge. In particular cases, like crown lengthening as preliminary treatment, implant procedure until osseointegration process, and temporomandibular disorder as an occlusal therapy, the use of provisional FPD will take 3-6 months until the insertion of definitive restoration. Therefore, materials used in making a provisional FPD should be able to preserve the prepared tooth, maintain the periodontal conditions, and have superior esthetic for a long time. Self-cure polymethyl methacrylate (PMMA) is a most commonly used material due to its biocompatibility, high wear resistance, ease of application, and superior esthetic. Nonetheless, unmodified self-curing PMMA has weaknesses due to its degradation process in the oral cavity, which affects its mechanical and physical properties. To overcome this, addition of reinforcement material, namely chitosan, in provisional FPD is necessary. Chitosan is polymer compound obtain through partial deacetylation of acetyl glucosamine through deacetylation of chitin base and modified into magnetic nanoparticles with size 100-400 nm to increase absorption power. The addition of nanotechnology to polymeric materials has shown significant appeal and improved mechanical and physical properties. Objectives: This review aims to explain the use of chitosan material in addition to self-cure PMMA provisional FPD to improve its mechanical and physical properties. Conclusion: Chitosan material can be an alternative option to improve the mechanical and physical properties of self-cure PMMA. Keywords: provisional fixed partial denture, self-cure PMMA, chitosan

Abstract

In recent years, zirconia all-ceramic restorations are widely used in crown and bridge treatment due to their superior mechanical properties and aesthetics. Zirconia is the strongest ceramic material, thus it is the treatment of choice for posterior dental restorations. One factor that affects the fracture resistance of zirconia restoration is the margin design. The shoulder and chamfer are the recommended margin design to obtain maximum fracture resistance from zirconia restoration. Shoulder is recommended because it has greater fracture resistance while chamfer is more conservative and able to withstand maximum masticatory load. However, there is still a difference of opinion regarding the fracture resistance of zirconia crown with shoulder and chamfer margin designs. This literature review aims to discuss the role of chamfer and shoulder margins design on the fracture resistance of zirconia crown. The shoulder margin design results in a wide ledge, space for adequate restoration contours and maximum aesthetics that provide resistance to occlusal forces and minimizes stress that can cause fracture. The chamfer margin design on zirconia crown has a difference in the rounded internal angle of the preparation resulting in better force distribution, better marginal fit and more resistance to fracture compared to the shoulder margin design. Chamfer margin design is more conservative and resistant to fracture due to better marginal fit which distribute stress more evenly. Abstract

Lithium disilicate is a glassy ceramic material that mimics the aesthetics and strength of natural tooth structure, making it very popular in recent years. Lithium disilicate has good flexural strength, translucency, and high mechanical strength of up to 360±60 MPa which is superior to feldspathic porcelain or leucite-reinforced glass ceramic. These excellent properties make it suitable for both anterior and posterior use. Naturally, ceramic materials are inherently brittle and tend to fracture easily in repetitive function. There are several methods of fracture treatment, one of which is repair. Composite resin is frequently used for ceramic repair as a simple and fast solution. The bond strength of ceramic repairs on lithium disilicate using hydrofluoric (HF) acid is higher compared to other methods. Hydrofluoric acid that has been used as a surface treatment for lithium disilicate is 4% and 5% HF acid. This article reviews the effect of HF as a surface treatment material on bonding strength between lithium disilicate and composite resin. The application of hydrofluoric acid in ceramic surface promotes the reaction with the glass matrix that contains silica and form hexafluorosilicates. This glass matrix is selectively removed and the crystalline structure is exposed. As a result, the surface of the ceramic becomes rough and this roughly etched surface helps to provide more surface energy prior to combining with the silane solution. Silane coupling agent forms a chemical covalent bond between silica on the lithium disilicate surface and composite resin. This bond will increase the micromechanical interlock. Abstract

Background: Enucleation and eviseration are surgical procedures often used for the removal of the eye. Some post-eviserated patients have complaints of discomfort when using conformer or ocular prosthesis. Ocular prosthesis modified with permanent soft liner (PSL) on the intaglio surface is an alternative to reduce the discomfort. Objective: This case report aims to explain the use of PSL in the manufacture of custom ocular prostheses in post-eviscerated patient. Case report: A 72-year-old male patient came to RSGM USU complaining of difficulty to socialize because he lost his eye and wanted to make a prosthetic. The patient had evisceration 4 months ago and was given a silicone surgical conformer. Tearing was the common complaint, followed by itchiness, soreness and uncomfortable when the conformer is fitted. Custom ocular prosthesis is modified using PSL on the intaglio surface. The use of PSL provides comfort to patients because it acts as a shock absorber and shock distributor that can distribute pressure more evenly so that the tissue in the eye socket receives less impact force. Conclusion: Fabrication of new ocular prosthesis with PSL is a novel way to minimize trauma to underlying supporting tissues so it can reduce the discomfort of the patient.Keywords: Evisceration, Custom ocular prosthesis, Permanent soft liner

Abstract

Background: All ceramic materials are often used in crown restorations for anterior and/or posterior teeth due to their aesthetic value and high strength. However, some all-ceramic materials such as monolithic zirconia have the disadvantage that they can cause wear problems on antagonistic teeth due to their high hardness. As an alternative to overcome this problem, hybrid ceramic materials are being developed. Objectives: This literature review aims to look at using hybrid ceramic materials as an alternative material for making crowns. A hybrid ceramic material blends the characteristics of ceramic and composite materials, resulting in a material with excellent mechanical strength and aesthetic qualities. It has a similar elastic modulus value to dentin thus reducing the occurrence of wear on antagonistic teeth. Hybrid ceramic materials is fabricated by CAD/CAM system with subtractive (milling) and additive (3D printing) processes. The subtractive fabrication method (milling) produces restorations with high strength properties but has a longer and more complicated manufacturing process. The additive (3D printing) method, on the other hand, is faster and requires less material than subtractive (milling). Summary: Therefore, hybrid ceramic materials should be considered as an alternative material for crown restorations. Keywords: Hybrid ceramic; CAD/CAM; additive (3D printing); subtractive (milling)
