Abstract

BOR is an indicator of health services that can be used to determine the level of quality, level of facility utilization and efficiency of health services. The impact of decreasing BOR causes a decrease in economic income for hospitals. The aim is to analyze the factors causing the low percentage of BOR at the Mitra Medika Bondowoso Hospital, at the Makassa Community Lung Health Center, at the Sleman Regional Hospital. The results obtained from the factors causing the low BOR are the lack of facilities owned by hospitals, the shortage of staff which causes officers to have to work extra to carry out other tasks so that the officer's workload increases and services will take longer, Discipline of Health Officers, Analyzing priorities , lack of coordination between registration officers, patient care ward officers, errors when inputting patient information data, supervision activities carried out by each room head. The service rates at the hospital in question are the amount of costs that must be paid by the patient to obtain health services, the policies implemented by the hospital and the competition between each hospital.

Abstract

Diphtheria is a type of infectious disease that can be prevented by immunization. The cause of diphtheria is toxigenic Corynebacterium diphtheriae strains. Humans are the only reservoir for the spread of diphtheria which is transmitted either through droplets, through eating utensils, as well as through close direct contact with skin lesions. This disease is characterized by sore throat, fever, malaise, and on examination pseudomembranes are found in the tonsils, pharynx and/or nasal cavity. Objective: To find out the magnitude of the Diphtheria outbreak problem and the risk factors that influence it. Method: This research is a literature study. The search was carried out in the Google Scholar database published in the period 2013 to 2023. Results: Shows that in extraordinary cases of diphtheria there are some who have received the DPT vaccine and there are also those who have not received the DPT vaccine, as well as a lack of public knowledge regarding diphtheria which can lead to outbreaks diphtheria. Conclusion: Epidemiological investigations of diphtheria outbreaks at this stage are still influenced by several factors which often occur, namely incomplete coverage of DPT vaccine in the toddler group, diphtheria transmission is prone to occur in the age group ≤ 15 years and low level of knowledge of the population due to lack of providing education about diphtheria disease.

Abstract

Medical record documents become a source of information for health workers in carrying out data analysis, planning, making decisions to provide services and become accountability files for proof of services that need to be maintained until the time limit according to agreed and established rules. The stages of activities carried out consist of the preparation and implementation stages. , and evaluation. The preparation stage includes a pre-test questionnaire, preparation of material by the facilitator, preparation of educational media in the form of banners and leaflets as well as a post-test questionnaire which is carried out the day before the counseling is carried out. The implementation stages include the delivery of educational material regarding maintaining medical record documents in the archive room. From the results of the percentage analysis, there was an increase in the percentage of students' overall knowledge of all aspects of medical record maintenance education in the application room delivered by the facilitator. Educational activities regarding how to maintain medical record documents in the submission room for Health Information Management study program students increased knowledge. knowledge increased by 18.61 points after health education was implemented. This increase in the average knowledge value occurred because health education used methods and medians, namely lectures and discussions.
