Abstract

Maintenance hemodialysis (HD) patients experience an increased risk of exposure to COVID-19 and complications of COVID-19 due to comorbidity conditions and the obligation to routine hemodialysis. Patients undergoing HD with end-stage kidney disease (ESRD) are particularly susceptible to SARS-CoV-2 infection and have a high mortality rate. This study aims to assess the correlation of ffactor actors that affect theurvival of HD patients with COVID-19. The design of this study is analytical research with a cross-sectional study retrospective approach. The study was conducted from August to December 2021 at the Medical Record Installation of USU Hospital Medan. The subjects of this study were all regular HD patients at USU Hospital Medan. To assess the relationship of the correlation f factor actors that influence the urvival of HD patients with COVID-19, the Pearson correlation test is used and if the data is not normally distributed, then the test used is the Spearman correlation test. The results of the analysis are said to be significant when p<0.05, with a degree of confidence of 95%. Pearson's correlation values for Hb and platelets with mortality were -0.282 and -0.285 but with p-values above 0.05 so the results were insignificant. The pearson correlation value for albumin with mortality is -0.462 with a p value of 0.013 which means that the test results are significant. Spearman's correlation values for leukocytes and D-Dimer with mortality were 0.296 and 0.307 but with a p-value above 0.05 so the results were insignificant. Spearman's correlation value for CRP with mortality is 0.383 with a p value of 0.044 which means the test results are significant. Albumin levels showed a significant correlation with mortality of HD patients with COVID-19. Meanwhile, CRP levels showed a significant positive correlation with mortality of HD patients with COVID-19.

Abstract

ABSTRACT Patients on hemodialysis have several mental and physical difficulties. Improved self-acceptance is necessary since psychological problems can have a substantial impact on patient’s quality of life, ranging from stress to depression. The purpose of this study is to assess how well mindfulness-based interventions (MBIs) can help hemodialysis patients feel more accepting of themselves. Using a pretest–posttest control-group design and a quasi-experimental methodology, the study included 68 patients receiving regular hemodialysis at RSP Prof. Dr. Chairuddin P. Lubis USU. The subjects were split into two groups, each with 34 patients: the intervention group and the control group. The MBI intervention chosen by the author is Mindfulness-Based Cognitive Therapy (MBCT). With an average score of 102 ± 14.98, the results demonstrated a substantial rise in self-acceptance scores in the intervention group. On the other moment, the control group's average score of 109.56 ± 6.25 did not indicate any improvement. Following the intervention, there was a significant difference in the intervention and control groups' self-acceptance scores, as indicated by a Mann-Whitney test (p=0.040). According to the research, MBIs help hemodialysis patients feel more confident in themselves. Future studies can build on this work to provide more thorough and successful therapies aimed at enhancing hemodialysis patients' sense of self-acceptance. Keywords: Hemodialysis, Mindfulness, Mbis, MBCT, Self-Acceptance  ABSTRAK Pasien hemodialisis memiliki banyak masalah fisik dan psikis, masalah psikis yang muncul bisa berupa stress hingga depresi yang dapat memengaruhi kualitas hidup pasien, sehingga dibutuhkan peningkatan penerimaan diri. Salah satu pendekatan yang ingin diuji adalah pendekatan Mindfulness-Based Interventions (MBIs) terhadap peningkatan penerimaan diri. Tujuan penelitian ini adalah untuk mengetahui efektivitas MBIs terhadap penerimaan diri pasien hemodialisis. Metode penelitian menggunakan quasi experimental dengan metode pretest–posttest control-group design, sebanyak 68 orang pasien yang menjalani hemodialisis regular di RSP Prof. Dr. Chairuddin P Lubis USU. Subjek penelitian dibagi menjadi dua kelompok yaitu kelompok intervensi dan kelompok kontrol dengan masing-masing kelompok berjumlah 34 orang. Penulis memilih Mindfulness-Based Cognitive Therapy (MBCT) sebagai intervensi MBIs. Hasil penelitian menunjukkan nilai skor penerimaan diri pada kelompok intervensi mengalami peningkatan signifikan dengan rerata 102 ± 14,98. Pada kelompok kontrol tidak memperlihatkan peningkatan dengan rerata 109,56 ± 6,25. Uji Mann whitney memperlihatkan perbedaan nilai skor penerimaan diri yang signifikan antara kelompok intervensi dan kelompok kontrol sesudah pemberian intervensi (p=0,040).  Berdasarkan hasil penelitian dapat ditarik kesimpulan MBIs efektif terhadap peningkatan penerimaan diri pada pasien hemodialisis. Diharapkan penelitian ini menjadi dasar bagi peneliti berikutnya untuk merancang intervensi yang lebih komprehensif dan efektif dalam peningkatan penerimaan diri pasien hemodialisis. Kata Kunci: Hemodialisis, Mindfulness, Mbis, MBCT, Penerimaan Diri

Abstract

                    ABSTRAK Mindfulness merupakan salah satu bentuk pelatihan regulasi emosi. Regulasi emosi merupakan proses yang dapat mempengaruhi saat individu dapat mengalami dan mengekspresikan emosi tertentu. Pengaturan emosi yang dimaksud bukanlah melarang individu untuk merasakan emosinya, tetapi mengatur intensitas dan ekspresi emosinya agar tidak merugikan diri sendiri atau orang lain. Agar kesejahteraan psikologis individu meningkat. Metode yang digunakan penulis adalah dengan menggunakan telaah beberapa literature yang berfokus pada mindfulness dan asuhan keperawatan. Tiga basis data dimasukkan dalam penelitian ini, yaitu Proquest, Sciencedirect, dan Google Scholar, yang terdiri dari 6 referensi berupa artikel jurnal bereputasi (3 internasional dan 3 nasional) yang berfokus pada mindfulness dan asuhan keperawatan. Studi literatur ini bertujuan untuk mengetahui manfaat mindfulness untuk meningkatkan kualitas hidup pada pasien hemodialisis. Dari hasil studi literatur ini ditemukan bahwa penerapan mindfulness dapat meningkatkan kualitas hidup pasien hemodialisis. Pelatihan mindfulness dapat memfasilitasi pengaturan emosi dan dianggap sebagai faktor pengaruh mendasar dalam pencapaian kesehatan mental dan kesejahteraan yang positif. Kemudian mindfulness dapat membantu pasien memulihkan dan menjaga keutuhan tubuh, pikiran dan jiwa yang dapat memberikan energi dan kekuatan internal sehingga potensi penyembuhan dapat terwujud. Temuan manajemen stres yang dikaitkan dengan peningkatan kualitas hidup, sangat signifikan. Selanjutnya, penelitian ini sebaiknya diterapkan di berbagai setting dengan penilaian tindak lanjut agar lebih efektif dan dapat diterapkan dalam pengukuran yang optimal. Kata Kunci: Hemodialisis, Kualitas Hidup Pasien, Mindfulness  ABSTRACT Mindfulness is a form of emotional regulation training. Emotion regulation is a process that can affect when individuals can experience and express certain emotions. The regulation of emotions in question is not to prohibit individuals from feeling their emotions, but to regulate the intensity and expression of their emotions so as not to harm themselves or others, so that the individual's psychological well-being will increase. The method used by the author is to use a review of several literatures hat focus on mindfulness and nursing care. Three data bases were included in this study, namely Proquest, Sciencedirect, and Google Scholar, consisting of 6 references in the form of reputable journal articles (3 international and 3 national) focusing on mindfulnessity and nursing care.This literature study aims to determine the benefits of mindfulness to improve the quality of life in hemodialysis patients. From the results of this literature study, it was found that the application of mindfulness can improve the quality of life of hemodialysis patients. Mindfulness training can facilitate emotional regulation and is considered a fundamental influencing factor in the attainment of positive mental health and well-being. Then mindfulness can help patients recover and maintain the integrity of the body, mind and spirit which can provide energy and internal strength so that healing potential can be realized.  The findings of stress management which is associated with an increase in quality of life, is very significant. Furthermore, this research should be applied in various settings with follow-up assessments to be more effective and can be applied in optimal measurement. Keywords: Hemodialysis, Mindfulness, Quality of Life
