Abstract

Fracture occurs due to the damage of the shape and function of the bone, and the fractured patients generally experience pain. One of the interventions to reduce pain intensity in fracture patients is cold compress. This study aims to identify the effectiveness of cold compress on pain intensity in fracture patients in H. Adam Malik Hospital Medan. This is a quantitative study with a quasi-experimental design approach with an equivalent control group pretest-posttest design. The number of samples is 70 respondents consisting of 35 people for each intervention and control group, respectively, which was collated by consecutive sampling. Data analysis used paired t-test and independent t-test. The results showed that there was a significant influence before and after cold compress intervention on pain intensity (p value 0.001 (p = <0.05)). There is a significant difference between the intervention with cold compresses and the hospital standard intervention on pain intensity in fracture patients (p value 0.001 (p = <0.05)). It can be concluded that cold compress has a significant effect on the pain intensity in fracture patients. Cold compress is one of the nursing interventions that can help to reduce the intensity of fracture pain.

Abstract

AbstrakLuka maligna dengan tingkat malodor dan jumlah eksudat yang berlebihan dapat menyebabkan masalah ketidaknyamanan dan isolasi sosial sehingga berdampak negatif bagi kualitas hidup pasien. Penelitian ini bertujuan membandingkan efektifitas antara perawatan luka menggunakan madu dengan metronidazole dalam menurunkan tingkat malodor dan mengurangi jumlah eksudat luka maligna. Penelitian. Desain penelitian yang digunakan adalah kuasi eksperimen dengan non equivalent pretest-posttest controlled group design dan non equivalent posttest only controlled group design. Berdasarkan consecutive sampling diambil sampel sebanyak 12 responden, terdiri dari enam responden kelompok kontrol dan enam responden kelompok intervensi, dengan kriteria: luka maligna stadium lanjut, laki-laki dan perempuan berusia 23-59 tahun, luas luka = 4cm². Perawatan luka dengan madu menurunkan tingkat malodor menurut pasien berdasarkan Numeric Rating Scale (NRS) dari 6,0 sebelum intervensi menjadi 2,1 sesudah intervensi hari ke-6, sementara perawatan luka dengan metronidazole tingkat malodor dari 5,6 menjadi 4,6. Hasil uji t menunjukkan nilai p<0,05; pada perubahan tingkat malodor. Perawatan luka dengan madu menunjukkan peningkatan jumlah eksudat dari 66,6gr sesudah intervensi hari ke-3 menjadi 80,8gr hari ke-6, sementara perawatan luka dengan metronidazole menunjukkan peningkatan jumlah eksudat dari 44,5gr menjadi 51,1gr. Hasil uji t menunjukkan nilai p>0,05 pada perubahan jumlah eksudat. Peneliti menyimpulkan perawatan luka dengan madu lebih efektif dibandingkan dengan metronidazole menurunkan tingkat malodor. Sementara perawatan luka dengan madu dan metronidazole belum efektif mengurangi jumlah eksudat luka maligna. Para pengambil kebijakan di institusi pelayanan kesehatan perlu mengeluarkan kebijakan yang dapat mengakomodasi penggunaan madu sebagai agen topical perawatan luka maligna. AbstractMalodor and exudates of wounds in malignancy can cause problems of discomfort & social isolation for patients. Both of them can produce negative impact on their quality of life. The treatment of malignant wounds use the right topical agent is a major factor in reducing malodor and wounds exudates. A comparative study was conducted to evaluate the effectiveness of honey and metronidazole on malodor & exudates malignant wounds. A Quasi experimental with non equivalent pretest-posstest controlled group design and non equivalent posttest only controlled group design were used in this study. Twelve sample was taken by a consecutive sampling, consis of six patients of control group and six patients of intervention group with a final stage of malignancy, 23-59 years old in male and female, size of wound is = 4 cm2. The wounds which we treated with honey demonstrated a reduction in malodor from patient perspectives using a Numeric Rating Scale (NRS), from the mean score of malodor on onset was 6,0 and on the sixth days of the treatment, to 2,1. group, malodor also reduced from 5,6 before treatment and dropped to 4,6 after treatment. at test showed that there are a significant difference between honey and metronidazole in reducing malodor (p<0,05). On the other hand, the wounds treated with honey and metronidazole preduced more drainage. In the honey group, the increase in the amount of wound exudate was noticeable on the third days (66,6 gr) and the sixth days (80,8gr) after the treatment. While in the metronidazole group, the amount of wound exudate was increase on the third days (44,5 gr) and the sixth days (51,1gr) after the treatment. There are not statistically significant (p>0,05). The study concluded that the use of honey in the treatment of wounds in malignancy is more effective than metronidazole in reducing malodor patients perspectives. Base on findings, it is requested for decision makers in the healthcare institution to produce a policy that could accommodate usage of honey as a topical agent in the treatment of malignant wounds.

Abstract

Introduction: The nursing intervention program is a strategy for the rehabilitation process among fracture patients The Levine conceptual model program is a practical nursing theory using energy conservation, energy, structural integrity, personal integrity, and social integrity. The study aims to identify the effect of nursing intervention based on Levine's theory of the rehabilitation process among fracture patients.Methods: A quasi-experimental with equivalent control group design was applied in this study. Sixty-two respondents were selected into the experimental group (n=31) and control group (n=31) by using a consecutive sampling technique. The patients' rehabilitation on fracture included sleep disorder, pain, anxiety, and family support as dependent variables. Researchers used the Sleep Quality Scale (SQS) instrument, the Numeric Rating Scale, the Hamilton Anxiety Rating Scale, and the family support scale. Data were analyzed using a Wilcoxon Signed Rank Test.Results: The results showed a significant effect of patients' recovery on fracture among patients after receiving Levine-based nursing intervention than before receiving the intervention (p <0.05). The results of research on the nursing intervention program are based on Levine's conceptual model of sleep disorders, pain, anxiety, and family support (p <0.05). In conclusion, there was significantly different nursing intervention based on Levine in   energy conservation, energy, structural integrity, personal integrity, and social integrity.Conclusion: The nursing intervention program based on Levine's conceptual model could be part of independent nursing intervention to deal with recovery in fracture patients. Based on this description, the researcher is interested in examining the effect of nursing intervention based on Levine’s conceptual model program on rehabilitation process among fracture patients.

Abstract

Stroke sufferers continue to increase every year and become polemic in various countries. Stroke is the second most common cause of death after heart disease. Stroke sufferers cause disruption of physical mobility, so they need help to carry out daily activities, one of which is the implementation of oral care, which is still minimal. The study aimed to describe the perception of family caregivers in implementing oral care among stroke patients in Medan city areas. A qualitative with phenomenology approach was applied in this study. The study was conducted in the Health Centre of Medan Deli, in April-August 2018. Twelve participants were recruited using purposive sampling. Data collection used focus group discussion (FGD). We analyzed the data by using the thematic analysis. The results of the study found that four themes, namely: 1) Caregiver knowledge in the implementation of oral care for stroke patients at home is not good, 2) The process of implementing oral care by caregiver for stroke patients at home is less optimal, 3) Family obstacles in carrying out oral care for stroke patients at home, 4) Expectations and suggestions in the implementation of oral care for stroke patients at home. Expected of research to developing or making a guide to the implementation of oral care as health education for families as caregivers in treating stroke patients at home, so that problems of oral care for stroke patients can be minimized. Abstract

Fracture is the discontinuity of bone tissues according to their types and areas. Intervention for recovering fractured patients is done by using Levine theory. The objective of the research was to identify the influence of Levine conceptual model based-intervention on anxiety. The research used quasi-experimental method with equivalent control group design. The samples were 52 respondents, 26 of them were in the intervention group ad the other 26 of them were in the control group, taken by using consecutive sampling technique. The data were analyzed by using paired t-test and independent t-test. The result of the research showed that there the influence of Levine conceptual model based-intervention program on anxiety before and after the intervention at p-value=0.000 (p<0.05). There was the difference of this type of intervention from hospital standard nursing intervention at p-value = 0.000 (p<0.5). The conclusion that there was the influence of Levine conceptual model based-intervention program on anxiety. It is recommended that this type of nursing intervention be used as a part of independent nursing intervention to help handle on anxiety of fractured patients.

Abstract

This study aims to identify the effectiveness of abdominal massage in preventing constipation in stroke patients. The method used is a quasi-experimental two-group pretest-posttest. The sampling technique was conducted by consecutive sampling using the Constipation Assessment Scale (CAS) instrument. The results showed an effect of abdominal massage on the prevention of constipation in stroke patients (p = 0.001) and after the procedure (p = 0.001). Abstract

This study aims to identify and evaluate the effect of natural music therapy intervention on hemodialysis patients. This research uses quantitative methods with a quasi-experimental non-equivalent control group pretest-posttest design. Respondents in this study were 96 hemodialysis patients. Data were collected using a State Anxiety Inventory (SAI) questionnaire comprising 20 statement items. Data were analyzed using nonparametric tests (Wilcoxon Signed Rank Test and Mann Whitney U Test). The results showed a difference between the mean value of anxiety in the intervention group and the control group, namely 37.90 in the intervention group and 42.33 in the control group, with a p-value of 0.048. In conclusion, natural music therapy is recommended as an alternative to reduce the level of anxiety in hemodialysis patients to improve the patient's quality of life. Abstract

Patients with health problems who undergo bed rest for a long time can experience various health problems. Problems that can be felt by patients on functional and hemodynamic status. This study aims to identify the effectiveness of progressive mobilization on functional status and hemodynamics in bed rest patients. The research method used a qualitative design with a randomized controlled trial method. The number of samples in this study was 70 people for each group. The sample in this study was taken by simple random sampling method with a total of 70 people in the control group and 70 people in the intervention group. Bivariate statistical test used Friedman and Mann Whitney test, while multivariate test used logistic regression. The results of this study showed that there was a significant effect of progressive mobilization on functional and hemodynamic status (p0.05). The conclusion of this study is that progressive mobilization is effective on the functional and hemodynamic status of the patient so that it can be applied in the ICU of the hospital as a nursing intervention

Abstract

Spiritualitas merupakan factor protektif dalam proses berduka pada keluarga pasien kronis pada pasien post stroke. Bereavement life review  adalah salah satu intervensi dalam penguatan spiritual keluarga pasien penyakit post stroke. Bereavement life review merupakan intervensi yang prosesnya adalah dengan mencari dan menggali makna hidup individu sehingga makna spiritualitas pasien atau keluarga dapat meningkat. Bereavement life review njuga merupakan intervensi yang mudah, mcepat, dan dapat dilakukan oleh perawat yang terlatih. Tujuan penelitian ini adalah mengetahui pengaruh bereavement life review terhadap spiritual pasien post stroke. Desain penelitian ini adalah quasi-eksperimental dengan pretest posttest control  group. Sampel yang digunakan adalah salah satu pasien post  stroke yang menjalani fisiotherapy di rumah sakit. Sehingga didapatkan sampel sebanyak 30 responden dengan 15 kelompok kontrol dan 15 kelompok intervensi. Pengambilan sampel dilakukan dengan menggunakan consecutive sampling. Kelompok intervensi mendapatkan  bereavement life review dengan dua sesi yang dilakukan oleh perawat. Kesejahteraan spiritual  diukur menggunakan instrumen SWBS (spiritual well-being scale). Analisis data menggunakan dependent t-test, Mann Whitney dan Wilcoxon. Hasil penelitian menunjukkan adanya perbedaan skor rerata postest kesejahteraan spiritual pada kelompok kontrol dengan kelompok intervensi (98,71 ± 3,65 dan  106,5 ± 1,83; p = 0,000). Terdapat perbedaan skor rerata kesejahteraan spiritual pada pretest dengan posttest pada kelompok intervensi (99,07 ± 2,95 dan 106,5 ± 1,83; p = 0,001). Proses bereavement life review merupakan proses peningkatan spiritual melalui proses rekontekstualisasi, memaafkan terhadap diri, dan refleksi yang membentuk penguatan koping sehingga muncul pemaknaan  terhadap diri sendiri. Dapat disimpulkan bereavement life review berpengaruh positif terhadap peningkatan kesejahteraan spiritual keluarga pasien stroke. Bereavement life review  dapat digunakan sebagai intervensi perawatan pasien stroke dan keluarga.

Abstract

Indonesia merupakan negara kepulauan yang terletak pada pertemuan tiga lempeng bumi (Eurasia, Indo-Australia dan Pasifik) dan dilalui deretan gunung berapi dan lautan. Keadaan tersebut membuat Indonesia memiliki beragam sumber daya alam dan budaya namun juga beresiko rentan terhadap bencana alam. Begitu juga di Propinsi Sumatera Utara, sebanyak 33 wilayah kabupaten dan kota yang ada di Sumatera Utara masuk sebagai wilayah rawan bencana. Dalam kontek pengurangan risiko bencana, pengabdian masyarakat perguruan tinggi dilaksanakan untuk mendorong terciptanya desa dan masyarakat yang aman dan tangguh terhadap bencana. Desa Sumbul merupakan salah satu desa yang ada di Kecamatan Sinembah Tanjung Muda (STM) Hilir, Kabupaten Deli Serdang, provinsi Sumatra Utara. Masyarakat desa Sumbul masih belum memahami seutuhnya tentang bencana alam, daerah ini merupakan salah satu wilayah yang rawan akan terajadinya bencana alam. Oleh Karena itu tim dari Fakultas Keperawatan Universitas Sumatera Utara memberikan jalan keluar untuk permasalahan ini dengan cara memberikan pelatihan pemberdayaan masyarakat dalam mewujudkan desa tangguh bencana berbasis masyarakat sebagai objek pengabdian pada masyarakat agar desa tersebut menjadi desa tangguh bencana. Tujuan desa tangguh bencana adalah untuk penguatan pengetahuan, keterampilan, dan perilaku pengurangan resiko terhadap bencana alam maupun bencana yang bersifat human error. Kegiatan pengabdian pada masyarakat ini dilaksanakan dalam waktu 1 bulan yaitu bulan Agustus 2019, dengan kegiatan berupa pemberian materi tentang kebencanaan, diskusi kelompok, latihan dan simulasi pemasangan tenda pengungsi dan teknik evakuasi. Pelatihan kesiapsiagaan bencana ini merupakan awal dari usaha membentuk desa tangguh bencana.
