Abstract

PT Jasa Marga (Persero) Tbk is a company engaged in toll road management and toll road development. The method used in this study implements the Hazard and Operability method in analyzing the Occupational Safety and Health Management System with this method which can later determine the risk and level / level of the Application of Safety and Health Management Systems to accidents. And get proposed improvements to reduce existing risks. The achievement of SMK3 at PT Jasa Marga (Persero) Tbk was 74.89% in YELLOW category. This value is obtained from an average of 5 (Variables), namely K3 Commitment and Policy (76.15%), K3 Planning (72.77%), K3 Plan Implementation (75.07%), K3 Performance Evaluation and Evaluation (74 , 64%), and Review and Improvement of SMK3 Performance (75.79%). Achievement of Occupational Health and Safety Management System (SMK3) on accidents occupies level 3, namely caution. Proposed improvements to reduce the risk of potential hazards arising is to make special crossings for workers, make drainage channels in special crossing lines for workers, and require every worker to use personal protective equipment.

Abstract

Penelitian ini mengkaji tentang aspek tanggung jawab hukum Direksi dan Komisaris dalam konteks tata kelola perusahaan di Indonesia. Fokus utama penelitian adalah menganalisis cakupan, batasan, dan implementasi tanggung jawab hukum organ perusahaan tersebut berdasarkan peraturan perundang-undangan yang berlaku, khususnya Undang-Undang Nomor 40 Tahun 2007 tentang Perseroan Terbatas. Metodologi yang digunakan adalah penelitian hukum normatif dengan pendekatan perundang-undangan dan konseptual. Hasil penelitian menunjukkan bahwa tanggung jawab hukum Direksi mencakup fiduciary duty yang terdiri dari duty of care, duty of loyalty, dan duty of skill dalam pengurusan perusahaan. Sementara Dewan Komisaris memiliki tanggung jawab pengawasan dan pemberian nasihat kepada Direksi. Kedua organ perusahaan ini dapat dimintai pertanggungjawaban secara pribadi apabila terbukti lalai atau melakukan kesalahan dalam menjalankan tugasnya. Namun, terdapat perlindungan hukum melalui prinsip Business Judgment Rule selama keputusan diambil dengan itikad baik dan untuk kepentingan perusahaan. Penelitian ini juga mengungkapkan pentingnya pemahaman komprehensif tentang batas-batas tanggung jawab hukum untuk mencegah timbulnya kerugian perusahaan dan melindungi kepentingan stakeholders. Rekomendasi yang diajukan meliputi perlunya penguatan mekanisme check and balance, peningkatan kompetensi organ perusahaan, dan implementasi sistem tata kelola perusahaan yang efektif.

Abstract

Stamping press machines rely on reliable clutch brake systems to ensure operational accuracy, productivity, and safety. This study presents an empirical failure analysis of a wet-type multi-disc clutch brake system in a 400-ton stamping press that experienced operational failure following flood exposure. Field observations, visual and dimensional inspections, and hydraulic system evaluations were conducted to identify failure mechanisms and root causes. The results indicate that the clutch brake failure was caused by the combined effects of mechanical interference leading to hydraulic hose rupture, improper installation of sealing components, and aging-related degradation of elastomeric seals. Flood-induced contamination significantly accelerated lubrication failure and material deterioration, resulting in hydraulic leakage and loss of system reliability. Corrective actions, including component replacement, lubrication system restoration, and hydraulic pressure adjustment, were implemented and validated through no-load testing, load testing, and production trials using actual stamped parts. Post-repair testing confirmed stable machine operation and acceptable product quality. The novelty of this study lies in demonstrating how environmental exposure interacts with installation errors and material aging to produce system-level clutch brake failure. The findings provide practical guidance for failure prevention, condition-based maintenance, and reliability improvement of heavy-duty stamping press machines operating in flood-prone industrial environments.
