Abstract

This rapid development of technological knowledge can help humans fulfill the new order of civilization in the era of technology that was created to make daily work easier. Computers are not only used as electronic typewriters but have been used to increase work and business productivity. An information system is a system that provides information for the decision-making process at every level in an organization. Network users control what they do through graphical applications prepared for the user. All data including electronic mail (e-mail), databases are stored on the server. Special servers for the internet or even for certain internet functions are the heart of information on the internet. Getting many benefits from the internet, for example reducing communication costs, improving communication and coordination skills, distributing knowledge and providing facilities for e-business or e-commerce, including reducing communication costs. In this article the author explains the implementation of the internetworking concept (internet and extranet), the benefits of using each and the development that can be carried out from the internetworking concept, components and forms of application of the internetworking.

Abstract

In order to increase customer satisfaction, the hospital is required to maintain and improve the quality of service within the hospital. Efforts to improve the quality of a hospital must be balanced with the implementation of Total Quality Management (TQM) in the optimal health sector by paying attention to all aspects of the hospital. Medical personnel who are agile and skilled in their fields will be very supportive if accompanied by supporting hospital facilities and infrastructure, this can make the hospital known to the wider community because of good service so that the quality of the hospital is not in doubt. For that a hospital must pay attention to quality control management in its own hospital.

Abstract

Paparan asap rokok pasif (secondhand smoke) merupakan faktor risiko lingkungan yang berkontribusi terhadap terjadinya gangguan paru pada dewasa non-perokok, khususnya dalam lingkungan keluarga. Tingginya prevalensi perilaku merokok di dalam rumah menyebabkan paparan kronik terhadap berbagai zat toksik yang berdampak negatif pada sistem pernapasan. Artikel ini bertujuan untuk meninjau bukti ilmiah terkini mengenai hubungan antara paparan asap rokok pasif dan risiko gangguan paru pada dewasa non-perokok dalam lingkungan keluarga. Metode yang digunakan adalah tinjauan pustaka naratif terhadap publikasi nasional dan internasional dalam lima tahun terakhir yang membahas dampak paparan asap rokok pasif terhadap fungsi paru dan penyakit paru kronik pada dewasa. Hasil tinjauan menunjukkan bahwa paparan asap rokok pasif berhubungan dengan penurunan fungsi paru, peningkatan risiko penyakit paru obstruktif kronik, asma dewasa, gangguan respirasi kronik, serta kanker paru pada non-perokok. Lingkungan rumah merupakan sumber utama paparan yang bersifat berulang dan jangka panjang. Oleh karena itu, pendekatan kesehatan keluarga melalui edukasi dan penerapan kebijakan rumah bebas asap rokok menjadi strategi penting dalam upaya pencegahan gangguan paru pada dewasa non-perokok.

Abstract

Background: Patients with chronic obstructive pulmonary disease (COPD) undergoing elective surgery under general anesthesia carry an elevated risk of postoperative pulmonary complications (PPCs). Evidence on optimal intraoperative ventilation strategies in COPD-specific populations remains limited. Methods: A systematic review following PRISMA 2020 guidelines was conducted. PubMed, Scopus, and the Cochrane Library were searched from January 2015 to January 2025. Eligible study designs included randomized controlled trials, prospective cohort studies, and retrospective cohort studies reporting intraoperative ventilation strategies and postoperative pulmonary outcomes in adult COPD patients undergoing elective surgery. Methodological quality was assessed using the Newcastle-Ottawa Scale and Cochrane Risk of Bias 2.0 tool. Results: From 1,101 identified records, 220 duplicates were removed. After screening 881 records by title and abstract, 15 underwent full-text review and 4 studies were included, comprising 709 patients across 1 randomized controlled trial and 3 retrospective cohort studies. Low tidal volume ventilation reduced PPC risk (OR 0.50; p = 0.010). PEEP showed no significant protective effect. Dexmedetomidine reduced ICU admission rates (4% vs 28%; OR 9.33). One-lung ventilation exceeding 2 hours independently increased pulmonary infection risk. Ninety-day mortality was higher in patients who developed PPCs (5.8% vs 1.3%; p = 0.016). Conclusion: Low tidal volume ventilation reduces PPCs in COPD patients undergoing general anesthesia, whereas PEEP confers no significant benefit. Multimodal strategies including sugammadex reversal and intraoperative dexmedetomidine offer complementary risk reduction. Randomized trials in diverse COPD populations are needed.
