Abstract

Medical records are a mandatory and important document in the management of hospital services. Based on the Minister of Health Regulation No. 24 of 2022, it is described that medical records consist of confidential patient identity data containing records of examination, treatment, medical actions, and other services that have been provided according to patient needs. Problems that occur in various departments include incomplete documents in the preparation, coding errors, and the wrong location of files in the filling selection. Inadequate storage space and sub-optimal filling systems also contribute to lost records and inefficient retrieval. This study aims to look at the medical record file management process at the UPTD Special Lung Hospital North Sumatra Province. This research uses descriptive qualitative research methods. The results showed that the UPTD Special Lung Hospital North Sumatra Province still uses hybrid medical record file, and electronic medical records at the coding and indexing stages, and the supporting factors of the medical record file management process include human resources, suitability of Standard Operational Procedures (SOP), facilities and infrastructure, and reporting carried out at the UPTD Special Lung Hospital North Sumatra Province still has obstacles in the medical record file management process, namely the assembling process, coding and indexing, and the medical record file storange process.

Abstract

Kemenkes menyusun sebuah program Indonesia Sehat demi meningkatkan derajat kesehatan masyarakat yang setinggi-tingginya. Oleh karena itu, demi menjalankan program tersebut, Kemenkes memutuskan perlu adanya kebijakan terkait dana demi memberikan pelayanan kesehatan yang berkualitas kepada masyarakat, salah satunya dengan dibentuknya kebijakan Dana BOK di tahun 2010. Penelitian ini bertujuan untuk melihat implementasi dana BOK di Puskesmas X di Kota Medan. Jenis penelitian ini bersifat kualitatif deskriptif. Pengumpulan data menggunakan teknik wawancara mendalam kepada Kepala Puskemas, Bendahara BOK dan Pengelola UKM. Hasil penelitian menunjukkan bahwa tahap perencanaan pengelolaan dana BOK di Puskesmas X di Kota Medan sudah mengikuti sesuai dengan juknis BOK terbaru, Permendagri dan Peraturan Wali Kota Medan. Pada tahap pelaksanaan, Puskesmas X sudah berusaha untuk melakukan kegiatan yang telah disusun sesuai POA dan juga target SPM yang dibuat, serta untuk tahap pencatatan dan pelaporan, masih terdapat beberapa program yang belum mencapai target. Hal ini terkendala karena kurangnya kelengkapan data pasien ketika berobat dan penginputan berbagai kegiatan harus diinput melalui beberapa aplikasi. Kesimpulan dalam penelitian ini adalah Puskesmas X di Kota Medan telah berusaha sebaik mungkin untuk melakukan tahap perencanaan, pelaksanaan, serta pencatatan dan pelaporan, akan tetapi masih terdapat banyak kendala yang harus dihadapi mulai dari target SPM yang tidak tercapai, kurangnya tenaga kesehatan, kurangnya kesadaran masyarakat, serta penginputan data yang menggunakan beberapa aplikasi yang banyak memakan waktu.

Abstract

Training and professional development are crucial factors in improving the quality of human resources (HR) in the healthcare sector. This component is crucial for ensuring that healthcare workers have the skills and knowledge necessary to provide safe and effective healthcare services. This study used a qualitative descriptive approach to understand healthcare workers' perceptions of the quality of training and professional development at Simalingkar Community Health Centers. The study subjects were healthcare workers selected based on their experience participating in training and professional development. The study found that healthcare workers at Simalingkar Community Health Centers have positive perceptions of training and professional development, which includes the use of health applications and curative treatment methods. The planning process involves regular meetings with the government and is supported by the Regional Budget (APBD) and the Regional Budget (BOK). Training has been shown to improve work quality, and mini-workshops are used for evaluation and agreement on next steps. The success of training is influenced by management support, resources, organizational culture, government policies, technology, and collaboration with educational institutions. This study concludes that the training and professional development program at Simalingkar Community Health Center has demonstrated a positive impact on the performance of healthcare workers and the quality of services provided. However, the program's success is influenced by various internal and external factors, including the individual, training methodology, work environment, and training needs.
