Abstract

Hemodialysis (HD) patients have a high rate of poor sleep. The Pittsburgh Sleep Quality Index (PSQI) is the most popular self-reported instrument to measure sleep quality. No study has tested the psychometric efficiency of the Bahasa (Indonesian language) PSQI in hemodialysis (HD) patients, and no cross-cultural adaptation has been done. The purpose of this research was to translate the PSQI into Bahasa and improve its psychometric properties for use in evaluating HD patients in Indonesia. This study used instrument translation, cultural adaptation, content validation, and equivalence to crossculturally validate and adapt an instrument. The hemodialysis center at a West Java medical facility served as a source of potential HD patients throughout the period of October 2022 to February 2023. A content validity index (CVI) was calculated for each item and the entire scale. The factor structure of the Bahasa PSQI has been evaluated by confirmatory factor analysis and internal consistency was calculated using Cronbach's alpha. The online survey received a response rate of 62.8% from a total of 220 patient with HD (about ten responders each item). The level of language clarity was 87.8%, and the level of cultural relevance was 84.5%. The CVI varied from 0.92 to 1.00. Each factor had factor loading ranging from 0.51 to 0.62. that Cronbach alpha for total score was 0.803, and the subscale ranged from 0.771 to 0.863. Our research provides preliminary evidence of the validity and reliability of the translated and adapted tool using data from HD patients in Indonesian hospitals.

Abstract

Latar Belakang : Gagal ginjal kronis, juga dikenal sebagai disfungsi kronis ginjal progresif dan ireversibel, adalah proses yang menyebabkan uremia dan membantu mengontrol metabolisme, keseimbangan kalori, dan keseimbangan elektrolit. Ini menimbulkan bahaya bagi sejumlah penyakit, termasuk obesitas, diabetes, hipertensi, dan kanker ginjal. Tujuan : mengevaluasi efektivitas masih perlu dikonfirmasi effektivitas aerobic exercise dan stretching exercise intradialytic terhadap penurunan restless leg syndrome pada pasien gagal ginjal kronik dengan hemodialisis. Metode : Penelitian ini menggunakan pendekatan kuasi eksperimen kuantitatif yang dibagi kedalam kelompok intervensi dan kontrol, pada Rumah Sakit Haji Jakarta selama 4 minggu. Empat puluh peserta terdaftar di setiap kelompok, sehingga total 80 peserta terdaftar. Purposive sampling digunakan untuk memilih responden. Selain itu, setiap responden diminta melengkapi skala penilaian International Restless Legs Syndrome Study Group (IRSSG). Hasil : Mayoritas responden laki-laki sebanyak 42 responden (52,50 %), Lamanya hemodialysis >12 bulan (85%), dan memiliki tingkat hemoglobin kurang dari 10 mg/dl (80%). Hasil penelitian ini menunjukkan bahwa: Pada kelompok intervensi Aerobic Exercise dan Stretching Exercise Intradialytic mengalami penurunan tingkat RLS dengan skor rata-rata pre test 2.12 menjadi 1.56 saat post test dengan nilai p value (0,001<0,05), sedangkan pada kelompok kontrol tidak terjadi, dimana nilai rata-rata pre test 2.06 menjadi 2.19 saat post test dengan p value (0,164>0,05). Kesimpulan : Terdapat pengaruh Aerobic Exercise dan Stretching Exercise dengan media audio visual terhadap tingkat RLS pada pasien gagal ginjal kronik yang menjalani hemodialisis. Studi berikutnya dapat mempelajari faktor-faktor yang mempengaruhi keberhasilan Aerobic Exercise dan Stretching Exercise dalam menurunkan RLS.

Abstract

Background: Restless legs syndrome (RLS) is common and burdensome among patients undergoing hemodialysis (HD). Intradialytic exercise provides a pragmatic opportunity for symptom management; however, evidence on resistance exercise during HD remains limited. Objective: This pilot study evaluated the effect of intradialytic resistance exercise on RLS severity in adults receiving maintenance HD. Methods: A two-arm, parallel-group pilot randomized controlled trial was conducted in two HD units (January–April 2025). Adults on thrice-weekly HD for ≥3 months with at least moderate RLS (IRLS ≥11) were randomized 1:1 to intradialytic resistance exercise (IRE) or usual care. The IRE group performed supervised elastic-band exercises during the first 2 hours of HD, three times weekly for 8 weeks. Outcomes were assessed at baseline, week 4, and week 8. The primary outcome was the International Restless Legs Syndrome Scale (IRLS) score, analyzed using mixed-effects models. Results: Forty-two participants were enrolled (IRE n=21; control n=21), with 90.5% completing the study. A significant group×time interaction was observed (p<0.001). At week 8, the IRE group demonstrated a clinically meaningful reduction in IRLS compared with controls (mean difference −8.4; 95% CI −11.7 to −5.0; Cohen’s d=1.11). Adherence was high and no serious adverse events occurred. Conclusions: An 8-week intradialytic resistance exercise program was feasible, safe, and effective in significantly reducing RLS severity among HD patients. Larger trials are warranted to confirm long-term benefits and implementation potential.
