Abstract

Congenital cataract is an eye disorder in the form of impaired lens transparency due to cloudiness that appears at birth or immediately after birth. Post-congenital cataract surgery requires regular monitoring to prevent complications that can reduce visual function. Therefore, this case report aims to describe the incidence of secondary glaucoma in patients with aphakia after congenital cataract surgery with uncontrolled post-operative monitoring regularly. A 4-year-old boy patient was brought by his parents with complaints of reappearance of white spots in the left eye which had become increasingly widespread in the last 2-3 months. Complaints accompanied by a feeling of glare in bright places. The patient's parents said the patient could not focus on seeing objects around him. The patient had a history of congenital cataract surgery in both eyes 2 years ago and did not have routine follow-up after cataract surgery. Congenital cataract is a lens clouding that can occur unilaterally or bilaterally when the child is born or immediately after birth. If monitoring is not carried out routinely, the most common condition that occurs in aphakic after congenital cataract surgery is secondary glaucoma. Regular monitoring after congenital cataract surgery is necessary to achieve successful management and prevent complications.

Abstract

AbstrakSalah satu upaya untuk meningkatkan derajat kesehatan masyarakat adalah dengan menyediakan pelayanan kesehatan yang terjangkau dan bermutu. Salah satu program yang disediakan puskesmas untuk mengatasi permasalahan pasien adalah Home Visit. Pada pelaksanaan home visit diperoleh berbagai data akurat dan terkini mengenai kondisi kesehatan pasien dan anggota keluarga, kondisi lingkungan di sekitar rumah, serta hubungan antar anggota keluarga. Salah satu instrumen pendukung yang dapat digunakan untuk kegiatan home visit sebagai upaya mengoptimalkan pelayanan kesehatan adalah tersedianya rekam medis dalam bentuk family folder. Kegiatan pengabdian yang dilakukan berupa kegiatan home visit terhadap pasien penderita diabetes mellitus tipe II di wilayah kerja puskesmas Batoh, Kecamatan Lueng Bata, Kota Banda Aceh. Kegiatan home visit hingga penyusunan laporan disupervisi oleh dosen pembimbing dari Departemen Family Medicine Fakultas Kedokteran Universitas Syiah Kuala, Banda Aceh. Home visit dilakukan sebanyak dua kali pada satu keluarga. Tujuannya adalah untuk mengevaluasi pengobatan dan memperoleh informasi lebih banyak mengenai kondisi kesehatan pasien/keluarga, hubungan pasien dengan keluarga, kondisi tempat tinggal hingga kondisi lingkungan sekitar pasien/keluarga yang tinggal satu rumah dengan pasien. Kegiatan home visit dilengkapi dengan pengisian instrumen penilaian keluarga yang terdiri dari genogram, family life cycle, family map, family APGAR, dan Family SCREEM. One of the programs provided by health center to resolve patient problems is the Home Visit. During the home visit, various accurate and up-to-date data will be obtained regarding the health condition of patients and family members, environmental conditions around the house, and relationships between family members. One of the supporting instruments that can be used for home visit as an effort to optimize health services is the availability of medical records in the form of a family folder. Community service activities carried out in the form of home visits to one patient with Type II Diabetes Mellitus in the working area of the Batoh Health Center, Lueng Bata District, Banda Aceh City. Home visit activities to the preparation of reports are supervised by supervisors from the Family Medicine Department of the Faculty of Medicine, Syiah Kuala University, Banda Aceh. Home visits were carried out twice in one family. The aims are to evaluate treatment, obtain more information about the health condition of patient and family, patient-family relationship, living conditions to the environmental conditions around the patient and family members who live with the patient. Home visit activities are complemented by completing the family assessment tools which consists of a genogram, family life cycle, family map, family APGAR, family SCREEM and family life line.
