Abstract

Background: Diabetes mellitus (DM) is a metabolic disease characterized by chronic hyperglycemia resulting from defects in insulin secretion and/or insulin action. Bone fragility in T2DM, which mirrored bone mineral density and bone mass reduction. Classical diagnosis of osteoporosis in T2DM is performed by dual-energy X-ray absorptiometry (DXA) and the risk of fracture can be assessed using the FRAX tool (Fracture Risk Assessment Tool). The purpose of the study was to assess the risk of osteoporosis using FRAX tool calculation (without BMD) in T2DM patients at the endocrine metabolic clinic of Haji Hospital Medan. Method: Descriptive observational with a cross-sectional study approach using primary and secondary data from the medical records of the Endocrine and Metabolic Clinic of Haji Hospital Medan in September-October 2023.Results: Of the 80 respondents with a risk of major osteoporosis, there were 77 respondents (96.3%) with low risk, 3 respondents (3%) with moderate risk, and no respondents with high risk (0%). The FRAX score for the risk of hip fracture received a low risk of 72 respondents (90%). At the same time, the high-risk was 8 respondents (10%).Conclusion: The majority of T2DM patients at Haji Hospital Medan are at low- risk for major osteoporosis and hip fracture

Abstract

Osteoartritis adalah penyakit degeneratif yang ditandai dengan degradasi matriks kartilago articular yang dipengaruhi oleh beberapa faktor, salah satunya adalah tingkat aktivitas fisik yang dapat diukur dengan international physical activity questionnaire (IPAQ). Tujuan dari penelitian untuk menentukan apakah ada korelasi antara tingkat aktivitas dengan derajat keparahan osteoartritis. Penelitian ini mengaplikasikan metode deskriptif analitik dengan pendekatan Cross Sectional. Sebanyak 67 pasien osteoartritis di poli reumatologi Rumah Sakit Umum (RSU) Haji Medan diambil sebagai sampel dengan menggunakan teknik purposive sampling, kriteria inklusi meliputi pasien yang terdiagnosis osteoartritis lutut berdasarkan kriteria radiografi Kellgren-lawrence dengan hasil pemeriksaan X-ray pada rekam medik, berusia diatas 50 tahun dan bersedia berpartisipasi setelah mengisi lembar informed consent. Pasien dengan kondisi medis lain yang dapat memengaruhi tingkat aktivitas fisik seperti gangguan neurologis dan muskuloskeletal atau yang tidak dapat mengisi kuesioner pada pasien yang mengalami gangguan kognitif akan dieksklusi dari penelitian. Analisa penelitian dilakukan dengan menggunakan uji Spearman. Temuan penelitian menunjukkan bahwa tingkat aktivitas fisik sebagian besar pasien adalah tinggi (44,8%) dan aktivitas tingkat sedang (31,3%). Derajat keparahan osteoartritis paling banyak adalah grade 2 dengan jumlah 30 orang (44,8%) diikuti oleh grade 3 dengan jumlah 21 orang (31,3%). Analisis korelasi spearman didapati nilai (p = 0,000 0,05, r = -0,453). Kesimpulannya bahwa ada hubungan yang signifikan antara tingkat aktivitas fisik dengan derajat keparahan osteoartritis dengan kekuatan korelasi yang cukup dan arah hubungan yang negatif. Indikasinya, bahwa intervensi berbasis aktivitas fisik dapat menjadi komponen penting dalam penatalaksanaan osteoartritis.

Abstract

Maternal Mortality (MMR) is still a global health problem that needs serious attention. In 2022, Deli Serdang Regency was recorded to have the highest AKI in North Sumatra Province, which was 16 per 42,363 live births. One of the efforts that can be made to reduce this number is by using the Maternal and Child Health Book (KIA). This study aims to analyze the relationship between the level of knowledge of pregnant women and the use of KIA books at the Tanjung Rejo Health Center, Deli Serdang Regency. This study uses an analytical observational design with a cross-sectional approach. A total of 100 pregnant women became respondents to the study. Data was collected through questionnaires, then analyzed using the Spearman statistical test. The use of KIA books is assessed through indicators of readability, comprehension, and application of information, while mother's knowledge is measured through the level of mastery of the content of KIA books. The results showed that the majority of pregnant women were at healthy reproductive age (55%), poorly educated (72%), unemployed (89%), and had high parity (56%). Most respondents (73%) fall into the category of poor utilization of KIA books, while 79% have a low level of knowledge. Statistical analysis showed a significant relationship between the level of knowledge of pregnant women and the use of KIA books (p = 0.001). The use of KIA books in pregnant women is significantly influenced by the level of knowledge.
