Abstract

Background : Sepsis is a major disease that affects millions of people worldwide every year. Almost all organs and systems were affected by sepsis. Hemostatic system is also affected. Easily accessible, inexpensive, and widely used laboratory tests that shows sepsis and the severity are important.Purpose : To investigate the relationship of the MPV values with the degree of sepsis and correlation MPV with procalcitonin.Method :  A cross sectional study was conducted. The subject with infection was treated in ward or Intensive Care Unit (ICU) in Adam Malik General Hospital, after fulfilling the inclusion criteria, full blood examination including MPV, procalcitonin, blood cultures was taken. The MPV values was compared between sepsis and non-sepsis, also between sepsis and severe sepsis. Then MPV values was correlated with procalcitonin.Results : Of the 50 patients with sepsis and 50 non-sepsis, found a significant difference in MPV levels of sepsis patients 10.97 (0.63) compared to the non-sepsis 9.17 (± 0.75), (p < 0.001). From the 17 patients sepsis and 33 severe sepsis patients also found a significant difference in MPV levels of severe sepsis patients 11,19 (0.64) compared with groups of sepsis patients 10.52 (0.27), (p < 0.001). And there is a strong correlation between MPV with procalcitonin r=0,702.Conclusion : The value of MPV is higher in patients sepsis and non-sepsis, and there is also a difference value of MPV in patients sepsis and severe sepsis. And also found there is correlation between MPV and procalcitonin.Keywords : sepsis, severe sepsis, mean platelet volume.

Abstract

Background: Coronavirus-19 (COVID-19) disease is caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), first reported from Wuhan, the capital of Hubei province, China, in December 2019. Since the occurrence of COVID-19 pandemic, there have been concerns about the risk of SARS-CoV-2 infection and complications in systemic autoimmune-rheumatic patient. Therefore, we intend to make recommendations for the management of autoimmune rheumatic diseases during this pandemic. Method: Indonesian Rheumatology Association (IRA) held an online meeting on May 17th2020 and formed a task force that was in charge to make recommendation for management of adult autoimmune rheumatic disease patients during COVID-19 pandemic. This task force consisted of 9 IRA members that held their first online meeting on May 18th2020. The draft of the recommendation were reviewed in three online meetings that resulted in final recommendation through consensus from all of task force members. The final draft of recommendation was approved by IRA executive board on July 10th2020. Result: The management of autoimmune-rheumatic diseases during the COVID-19 pandemic must consider several important factors such as underlying rheumatic-autoimmune disease factors, risk factors for COVID-19 exposure in autommune-rheumatic disease patients and the presence of COVID-19 infection symptoms Discussion: Recommendations for pharmacological management of autoimmune rheumatic disease patients during COVID-19 pandemic are according to exposure to COVID-19 and the presence of symptoms. There are four groups of patients that are the focus of this recommendation: 1) autoimmune rheumatic disease patients without symptoms of infection AND without risk of exposure to COVID-19; 2)autoimmune rheumatic disease patients without symptoms of infection AND with close contact (exposure) to COVID-19 probable or confirmed patients; 3) autoimmune rheumatic disease patients with symptoms of COVID-19 infection but the infection status is not yet known (suspected and probable case of COVID-19); and 4) autoimmune rheumatic disease patients with confirmed COVID-19 infection. Each groups have different management approaches. Conclusion: Recommendations for the management of autoimmune rheumatic diseases during this pandemic is needed to guide doctors that treat autoimmune rheumatic diseases.

Abstract

Vaccination is a very important measure for the prevention of various infections worldwide including the recent COVID-19 disease. However, until now the COVID-19 vaccine with various platforms has not been clinically tested on autoimmune inflammatory rheumatic disease (AIIRD) patients, due to caution against possible side effects and unknown efficacy. Several recent studies proved that there is increased risk of SARS-CoV-2 infection in AIIRD patients and moreover, those patients also have worse COVID-19 outcomes. Thus, patients with AIIRD should be prioritized for vaccination because they have an increased burden of infections, including COVID-19. Many studies showed that inactivated/non-live vaccine is safe for AIIRD patients and do not cause disease exacerbations. We conclude that benefits of vaccination greatly outweigh the risks of infection and therefore, COVID-19 vaccines can also be administered safely in stable AIIRD patients.

Abstract

Background. Diabetes is a major health problem worldwide including in Indonesia. There is a close relationship between diabetes and cardiovascular disease, with cardiovascular events being the most prevalent cause of morbidity and mortality in diabetic patients. Therefore, it is important to assess cardiovascular risk in patients with diabetes in order to conduct appropriate preventive strategies. Various methods have been developed to assess the degree of cardiovascular risk, one of which is the Jakarta Cardiovascular Score, a scoring method that has been adapted for the Indonesian community. This study aims to describe the profile of cardiovascular risk in diabetes patients at Universitas Sumatera Utara Hospital. Methods. A descriptive, cross-sectional study was conducted in the internal medicine outpatient clinic at Universitas Sumatera Utara Hospital. All subjects were diabetic patients who had not developed any cardiovascular complications. The risk of cardiovascular disorders was assessed using the Jakarta Cardiovascular Score. Data were collected through medical records, interviews, and physical examinations. Results. There were 64 subjects in this study. The majority of subjects (81.3%) were classified as high risk for cardiovascular disease. Conclusion. Most patients with diabetes have a high degree of risk of cardiovascular disease.

Abstract

Mual dan muntah pascaoperasi, yang dikenal sebagai Post Operative Nausea Vomiting (PONV), merupakan komplikasi umum yang sering dikeluhkan pasien setelah menjalani pembedahan dengan anestesi umum. Penelitian ini bertujuan untuk menganalisis insidensi PONV pada pasien yang menerima anestesi umum di Rumah Sakit Prof. Chairuddin P. Lubis USU, Medan. Desain penelitian bersifat observasional analitik dengan pendekatan cross-sectional. Teknik pengambilan sampel menggunakan purposive sampling dengan jumlah sampel sebanyak 97 pasien. Instrumen yang digunakan adalah lembar penilaian PONV. Hasil penelitian menunjukkan bahwa 40,6% pasien mengalami PONV sebanyak 4–6 kali, 34,4% sebanyak 1–3 kali, dan 25,0% lebih dari 7 kali. Durasi operasi sebagian besar berkisar antara 60–90 menit, dengan anestesi umum intubasi endotrakeal (GA ETT) yang digunakan pada mayoritas kasus. Penggunaan GA ETT pada pembedahan berdurasi panjang berhubungan dengan risiko PONV yang lebih tinggi. Temuan ini menegaskan pentingnya pengembangan strategi pencegahan PONV yang lebih efektif. Anestesi umum dengan GA ETT pada operasi berdurasi panjang berkorelasi dengan risiko PONV yang lebih tinggi, diduga akibat akumulasi efek anestesi dan stimulasi refleks vagal. Temuan ini menegaskan perlunya strategi pencegahan PONV yang terarah.
