Abstract

Gastrointestinal dysmotility in systemic sclerosis occurs as the end result of extensive fibrosis of the gastrointestinal tract. The entire length of the tract from the esophagus to the anorectum could be affected, exerting various gastrointestinal symptoms. Clinical manifestations attributed to gastrointestinal dysmotility are associated with significant distress and an increased risk of nutritional impairment, reducing the the quality of life of systemic sclerosis patients. One of the most commonly overlooked gastrointestinal implications in systemic sclerosis is malnutrition. Once malnutrition ensues in the course of systemic sclerosis, the detrimental effects attributed to nutritional decline are difficult to reverse and pose an increased risk of mortality. Adequate management through timely diagnosis of gastrointestinal dysmotility and utilization of malnutrition screening tools for systemic sclerosis patients could prevent the progression to malnutrition and its negative impacts.

Abstract

ABSTRACT Sexually transmitted disease (STD) is still a major public health problem in Bali, as one of the province with high number of cases and disease burden. Syphilis was one of the STD that usually found in Wangaya Regional Hospital as a dermatovenereology referral. This study aims to describe syphilis patient profile on dermatovenereology outpatient polyclinic from January 2023 until December 2025. Our study used a non-experimental, descriptive, cross-sectional, and retrospective design, using a secondary data from medical record (diagnosis, gender, age, education, employment, therapy, VDRL, and address) on all new syphilis patient. Data is processed using SPSS 29. We found 26 cases of syphilis incidence proportion that matches criteria, with 96,2% diagnosed latent syphilis. Half of them had Human Immunodeficiency Virus (HIV) as a comorbid; 6 patient is pregnant (23,1%); and 3 patient had HIV while in pregnancy (11,5%). The majority of demographic profile was male (61,5%), of productive age (18 – 59 years old) (96,2%), with secondary education level (92,3%), and had a job as an entrepreneur (65,4%). A little more than half of the patient achieve seroreversion (61,5%), including all of the pregnant woman. The majority of patients reside in Bali (76,9%), and this proportion is in accordance with increasing case number post COVID-19 pandemic, along with economy and tourism rebound. High number of cases on reproductive age bracket and pregnant women population has to put health care worker and government as a policy maker, on a crucial note. Keywords: Syphilis, Sexually Transmitted Infections, HIV, Pregnancy.  ABSTRAK Infeksi menular seksual (IMS) masih menjadi masalah kesehatan yang perlu didalami di Bali, sebagai salah satu provinsi dengan konsentrasi kasus dan angka beban kasus penyakit yang tinggi. Sifilis sebagai salah satu jenis IMS, sering ditemukan pada Rumah Sakit Umum Daerah Wangaya sebagai rumah sakit rujukan bidang dermatologi venereologi. Penelitian bertujuan untuk mengetahui profil pasien sifilis di poliklinik dermatologi venereologi Rumah Sakit Umum Daerah Wangaya periode Januari 2023 - Desember 2025. Studi ini bersifat non-eksperimental, deskriptif, potong lintang, dan retrospektif, dengan menggunakan data sekunder dari rekam medis (diagnosa, jenis kelamin, usia, pendidikan, pekerjaan, terapi, VDRL, dan alamat) pada seluruh pasien sifilis yang baru ditemukan. Data diproses menggunakan SPSS 29. Proporsi insidensi sifilis sebesar 26 kasus yang memenuhi kriteria, dengan 96,2% pasien terdiagnosa sifilis fase laten. Setengahnya memiliki komorbid Human Immunodeficiency Virus (HIV), sedang hamil (6 pasien, 23,1%), dan pasien sifilis dengan HIV dan sedang hamil sebesar 3 pasien (11,5%). Profil demografi yang mayoritas kami temukan, berjenis kelamin laki-laki (61,5%), berada pada rentang usia produktif (18 – 59 tahun) (96,2%), dengan tingkat pendidikan menengah (92,3%), dan memiliki pekerjaan sebagai wirausaha (65,4%). Sebagian besar pasien dapat seroreversi setelah terapi (61,5%), termasuk seluruh ibu hamil. Mayoritas pasien berdomisili di Bali (76,9%), dan proporsi ini sesuai dengan meningkatnya angka kasus pasca pandemi COVID-19, dengan pulihnya ekonomi dan pariwisata. Tingginya kasus sifilis pada populasi usia reproduktif dan ibu hamil, masih menegaskan bahwa sifilis masih merupakan masalah IMS yang perlu diperhatikan oleh tenaga kesehatan maupun pemerintah sebagai pemangku kebijakan.  Kata Kunci: Sifilis, Infeksi Menular Seksual, HIV, Kehamilan.
