Abstract

Indonesia merupakan negara Agraris dengan sektor pertanian menjadi salah satu kunci utama. Peningkatan produksi pertanian berkorelasi positif dengan penggunaan pestisida. Jika tidak bijak, pestisida menyebabkan berbagai masalah kesehatan. Untuk mencegah dan mengurangi efek buruk paparan pestisida, penting untuk menggunakan alat pelindung diri (APD) pada petani. Kegiatan ini bertujuan mengidentifikasi perilaku kepatuhan dan memberikan edukasi penggunaan APD untuk menghindari paparan pestisida. Kegiatan dilakukan melalui kerjasama FK Universitas Jember, pemerintah desa dan puskesmas Jenggawah. Kegiatan dilaksanakan di balai Dusun Gayasan B pada 18-19 Juni 2022, pukul 07.30-13.00 WIB. Jumlah sasaran dalam dua hari sebanyak 300 peserta. Tim pelaksana terdiri atas dosen, mahasiswa dan tenaga pendukung kegiatan, yang berjumlah 20 orang. Kegiatan terdiri atas pemeriksaan TTV, antropometri, anamnesis, identifikasi dan konseling mengenai penggunaan APD dilakukan oleh mahasiswa, dan konseling kesehatan dilakukan oleh dosen. Jumlah warga yang hadir dalam 2 hari mencapai 317 orang, dengan jumlah petani sebanyak 134 orang. 62 dari 134 petani menggunakan APD. Jenis APD yang sering digunakan oleh para petani adalah masker, baju lengan panjang dan sarung tangan. Melalui kegiatan ini diharapkan meningkatkan pengetahuan dan kesadaran petani terkait pentingnya APD, sehingga terbentuk masyarakat agrikultur yang sehat dan diharapkan visi Agromedis Fakultas Kedokteran Universitas Jember terimplementasi riil sebagai salah satu tri dharma perguruan tinggi.

Highlight: LEMS is a rare neurological disease with fluctuating symptoms Delays and misdiagnosis of LEMS disease often occur The two main pathophysiologies of LEMS are autoimmune and paraneoplastic.   ABSTRACT A rare condition known as Lambert-Eaton myasthenic syndrome (LEMS) affects the neuromuscular junctions, which are the connections between muscles and nerves. Tumor-associated or autoimmune causes trigger this condition. This mechanism depends on the presence of antibodies that directly attach to voltage-gated calcium channels located on the presynaptic nerve terminals. LEMS disease is divided into non-paraneoplastic or non-tumor LEMS (NT-LEMS) and paraneoplastic LEMS (P-LEMS). NT-LEMS is believed to be caused by an autoimmune process. On the other hand, P-LEMS has an underlying tumor, and LEMS symptoms are paraneoplastic manifestations of the tumor. Clinical signs of LEMS include proximal muscle weakness, autonomic dysfunction, and decreased deep tendon reflexes. The predominant sign of LEMS is weakness of the lower extremities. The defining characteristic of LEMS is a weakness that spreads from caudal to cranial, causing oculobulbar manifestations, and from proximal to distal, potentially involving the feet and hands. The diagnosis of LEMS depends on clinical, electromyographic, and serological findings of anti-VGCC antibodies. Therefore, comprehensive oncologic screening and monitoring should promptly follow a diagnosis of LEMS. The standard approach to treating LEMS symptoms is administering drugs that improve neurotransmission, such as potassium channel blockers and amifampridine. In refractory cases, immunosuppressants or immunomodulator agents, such as a combination of prednisone and azathioprine, are used. If a tumor is detected, oncological therapy should be a priority.

Abstract

Objective: To evaluate the effectiveness and safety of intracavernosal injections of platelet-rich plasma (PRP) as an alternative therapy for the treatment of erectile dysfunction. Material & Methods: We conducted a comprehensive literature search on Pubmed, Cochrane Library, ScienceDirect, Scopus, and Proquest databases for randomized controlled clinical trials (RCTs) until April 2023. The study protocol has been registered in the International Prospective Register of Systematic Reviews (PROSPERO CRD42023424586). Results: Our results showed that PRP could improve MCID in IIEF-EF score (OR: 4.64; 95% CI: 2.20, 9.79; p < 0.0001; I2 = 77%) and PSV (MD: 9.63; 95% CI: 1.17, 18.09; p = 0.03; I2 = 73%). In addition, PRP administration was shown to be relatively safe, and there was no significant difference in post-injection VAS pain scores when compared to controls (MD: -0.09; 95% CI: -0.49, 0.31; p = 0.66; I2 = 84%). Conclusion: This systematic review and meta-analysis showed promising results regarding the effects of PRP administration on erectile dysfunction. However, due to the lack of quality and number of studies, these findings should be interpreted carefully, and further investigations are still needed.
