Abstract

Inisiasi Menyusu Dini (IMD) adalah proses meletakkan bayi baru lahir pada dada atau perut ibu agar bayi secara alami dapat mencari sendiri sumber air susu ibu atau ASI dan mulai menyusu bayi akan mendapatkan kekebalan tubuh. Tujuan penelitian adalah untuk mengetahui faktor-faktor yang berhubungan dengan pelaksanaan Inisiasi Menyusu Dini (IMD) di RSUD Kota Prabumulih. Metode penelitian yang digunakan adalah study analitik dengan pendekatan cross sectional. Populasi pada penelitian ini adalah seluruh ibu bersalin di Rumah Sakit Umum Daerah Kota Prabumulih. Sampel penelitian adalah sebagian ibu bersalin yang melahirkan di RSUD Kota Prabumulih Tahun 2020 dengan jumlah sebanyak 35 orang. Instrumen yang digunakan adalah kuisioner sebagai panduan pengambilan data. Analisa yang digunakan adalah analisa univariat dan bivariat. Hasil penelitian berdasarkan analisa univariat diketahui bahwa dari 35 Responden didapatkan responden yang dilakukan Inisiasi Menyusu Dini sebanyak 20 responden (57.1%) lebih banyak dari yang tidak dilakukan Inisiasi Menyusu Dini sebanyak 15 responden (42.9%), dari 35 responden didapatkan bahwa responden yaang pendidikan tinggi sebanyak 19 responden (54.3%) lebih banyak dari responden yang pendidikan rendah sebanyak 16 responden (45.7%), dari 35 responden didapatkan bahwa responden yang jumlah anak ≥ 3 sebanyak 9 responden (25.7%) lebih sedikit dari responden yang jumlah anak ≤ 2 sebanyak 26 responden (74.3%), dan dari 35 responden didapatkan bahwa yang berpengetahuan baik sebanyak 19 responden (54.3%) lebih banyak dari responden yang berpengetahuan rendaah sebanyak 16 responden (45.7%). Dari analisa bivariat diketahui bahwa ada hubungan bermakna antara pendidikan ibu dengan Inisiasi Menyusu Dini dengan nilai p value = 0,000 ( p< 0,05 ), ada hubungan bermakna antara paritas ibu dengan Inisiasi Menyusu Dini dengan nilai p value = 0,002 ( p< 0,05 ), dan ada hubungan bermakna antara pengetahuan ibu dengan Inisiasi Menyusu Dini dengan nilai p value = 0,000 (p< 0,05). Simpulan: ada hubungan yang bermakna antara pendidikan ibu dengan Inisiasi Menyusu Dini, ada hubungan yang bermakna antara paritas ibu dengan Inisiasi Menyusu Dini, dan ada hubungan yang bermakna antara pengetahuan ibu dengan Inisiasi Menyusu Dini.

Abstract

Background: One contributing factor to the failure of breastfeeding is the rising prevalence of mothers engaged in full-time employment, exemplified by occupations like factory work. This phenomenon is attributed to various impediments, including restricted intervals for rest, inadequacy of workplace amenities and provisions, and the presence of fatigue. Objective: To comprehensively understand the breastfeeding endeavors of mothers employed at PT Heinz ABC factory, delving into their experiences is imperative. Methods: Qualitative The research employed a generic qualitative research approach, utilizing purposive sampling as the sampling technique. The research involved nine informants who were mothers employed in factories and had children aged between 7-24 months. Data collection was conducted through online in-depth interviews conducted via WhatsApp voice calls. The researchers also employed supportive instruments, specifically interview guides that had undergone pilot interviews beforehand. The data analysis was conducted through thematic analysis. Results: This research is segmented into three themes, namely knowledge, self-efficacy, and barriers. Conclusion: The prevalence of breastfeeding practices among mothers employed in factories remains low. This can be attributed to obstacles hindering the sustained implementation of exclusive breastfeeding, including inadequacies in workplace facilities, restricted rest intervals, psychological pressure, and low self-efficacy.

Abstract

Background: Inadequate breast milk production remains a frequent reason for early supplementation and discontinuation of exclusive breastfeeding. Fenugreek (Trigonella foenum-graecum) and moringa (Moringa oleifera) are widely used as natural galactagogues; however, comparative evidence examining their relative effectiveness within the same postpartum population remains limited, particularly in community maternal health settings. Methods: A quantitative quasi-experimental study with a posttest-only control group design was conducted in 2025 in Karawang Regency, Indonesia, and reported in accordance with the TREND statement. A total of 45 postpartum breastfeeding mothers were allocated consecutively by facility to three groups: control (n = 15), fenugreek (n = 15), and moringa (n = 15). No pretest was conducted; therefore, baseline equivalence was not confirmed. The fenugreek group received 500 mg fenugreek seed extract twice daily for 7 days; the moringa group received 250 mg moringa leaf extract twice daily for 7 days — both doses based on published therapeutic evidence. Lactation performance was assessed on day 8 using a validated structured questionnaire and observation sheet (Cronbach’s α = 0.81; CVI = 0.89). Data were analysed using the Kruskal-Wallis test, followed by Mann-Whitney U post hoc comparisons with Bonferroni correction Results: Post-intervention lactation performance scores (mean ± SD) were 5.93 ± 1.03 (control), 8.93 ± 1.03 (fenugreek), and 9.73 ± 0.46 (moringa). The Kruskal-Wallis test showed a significant overall difference (H = 34.21, df = 2, p < 0.001, η² = 0.76). Pairwise comparisons showed significant differences for control vs. fenugreek (U = 4.50, Z = −4.68, p < 0.001, r = 0.85), control vs. moringa (U = 0.00, Z = −5.20, p < 0.001, r = 0.95), and fenugreek vs. moringa (U = 54.00, Z = −2.87, p = 0.004, r = 0.52). Conclusion: Both fenugreek and moringa supplementation significantly improved lactation performance compared with routine care. Moringa demonstrated statistically significantly higher performance than fenugreek. Randomised controlled trials with objective lactation indicators and confirmed baseline equivalence are needed to confirm these findings.
