The research design used is descriptive with a case study method. The study took 1 client with a medical diagnosis of breast cancer and problems with skin integrity disorders. Pre-survey data processing was taken in Room VI at Kabanjahe General Hospital. The research results showed that the client said there was a wound on the left breast, with objective data the color of the wound was blackish brown, there was blood mixed with a little pus, and when the bandage was removed the smell was unpleasant. After 3 days of nursing care, the wound can improve with the criteria of the wound being red, no tissue necrosis, no odor, no pus coming out and a little bleeding. The conclusion of the nursing care evaluation shows that the problem of impaired skin integrity is still in the partially resolved stage. Based on the results obtained, further research is still needed, because healing skin integrity disorders due to cancer takes quite a long time, so clients must be patient and comply with all ongoing treatment.

Probiotik adalah mikroorganisme yang memberikan maafaat kesehatan untuk menyeimbangkan saluran pencernaan. Namun, banyak ibu menyusui tidak memahami bahwa ASI memiliki kandungan probiotik yang berperan penting dalam tubuh bayi. PKM ini bertujuan untuk meningkatkan pengetahuan kepada ibu menyusui melalui penyuluhan tentang manfaat probiotik. Kegiatan ini dilakukan dengan melibatkan ibu menyusui yang ada didesa Sumbul kecmatan kabanjahe. Materi yang disampaikan definisi tentang probiotik, manfaat probiotik, dan jenis – jenis probiotik dalam tubuh manusia. Partisipan ibu menyusui memahami bahwa ASI sangat penting bagi kesehatan bayi. Kesimpulannya, penyuluhan manfaat pentingnya probiotik dan sumber – sumber probiotik untuk kesehatan bayi.

This community service aims to enhance the quality of life and independence of residents in the Social Rehabilitation Center for the Disabled (UPT) through appropriate nursing interventions. The community service method involves a participatory approach, where collaboration between the team and relevant stakeholders is employed to design and implement the SP (1-4) strategy. The steps include problem identification, intervention planning, action implementation, and result evaluation. The evaluation results also indicate that the implementation of this strategy can be adopted and adapted to the needs of similar service units in various contexts. In conclusion, the application of the SP (1-4) implementation strategy is effective in improving self-care and the quality of life of residents at the Social Rehabilitation Center for the Disabled. The implications of this community service can serve as a reference for similar institutions and relevant stakeholders to develop intervention models that are suitable for the context and characteristics of the served community.

Music therapy is the use of music for relaxation to accelerate the healing, improve mental function and create a sense of prosperity, music has a biological effect on human behavior by involving specific brain functions such as memory, learning, motivation, and emotion. Pain is an important sensation for the body. The sensation of vision, hearing, smell, taste, touch and pain is the result of sensory receptor stimuli. Pain management as part of the nurse's care of the patient's response will differ between patients, Appendectomy is the appendix uplifted by an endoscopic procedure or approach, Appendixitis is a bacterial infection. The trigger factor is the lumen blockage caused by fecalite, lymphoid hypertrophy, dry barium, seed or intestinal worms.This research is quasi experiment, which is done to know the influence of classical music therapy to the intensity of pain in post appendectomy patients with 32 respondents, the method of data collection by way of way and direct observation of respondents, The analysis technique used is t-test with 95% confidence level.The results of this study indicate that to determine the effect of classical music therapy on the intensity of pain in patients post apendiktomi very real and close. Before performing music therapy the mean value of 2.59 after music therapy the mean value of 1.44 suggest to health workers especially to nurses At Bina Kasih General Hospital Medan to provide (Penkes) Health education to patients who experience pain post Apendiktomi.
