Abstract

Diarrhea remains one of the leading causes of morbidity and mortality among children in developing countries, including Indonesia. One of the most common causes of severe acute diarrhea is rotavirus infection, which is highly contagious through the fecal-oral route and can lead to severe dehydration and even death. Efective prevention can be achieved through rotavirus vaccination, which has been proven to reduce incidence, prevent transmission, and decrease disease severity. Rotavirus vaccination in children is known to be benefcial in controlling cases of rotavirus diarrhea, and since 2023, the rotavirus vaccine has been part of Indonesia's national immunization program, provided free of charge to infants according to the Ministry of Health's immunization schedule. Multiple studies have shown that this immunization reduces the risk of serious complications, shortens hospital stays, decreases hospitalization rates, and lessens the economic burden on families due to medical costs and lost productivity. Broad implementation of rotavirus vaccination is therefore a key strategy to improve child health and reduce diarrhea-related mortality in Indonesia.

Abstract

The prevalence of obesity is increasing worldwide and is considered as epidemic. According to the 2018 Riskesdas data, the obesity rate in Indonesia has increased to 35.4% compared to 2007. Obesity is a risk factor for metabolic diseases due to chronic inflammatory conditions. This is associated with increased permeability of the intestinal wall which is characterized by increased levels of zonulin. Probiotics are known to reduce zonulin levels and improve intestinal integrity in obesity.

Abstract

Vitamin D is recognized for its pivotal role in maintaining calcium-phosphorus homeostasis and regulating bone metabolism. The beneficial effects of vitamin D on the protective function of the immune system are seen in the innate immune system. It increases the production of defensin β2 and cathelicidin by macrophages, monocytes, and keratinocytes in response to infection. Cathelicidin is produced by neutrophils, macrophages, and cells lining epithelial surfaces such as skin, respiratory, and digestive tracts. Recent research showed that vitamin D exerts potent immunomodulatory effects by modulating the innate and adaptive immune systems. This led to the recognition of various disease conditions associated with vitamin D deficiency. This narrative review aimed to describe the molecular mechanisms for the vitamin D immunomodulatory effect.
