Abstract

Background: Elderly people have poor sleep quality which disrupts their activities during the day. Progressive muscle relaxation (PMR) is a simple relaxation method that goes through two processes, namely tensing and relaxing the muscles of the body and is one of the techniques that can reduce sleep quality in the elderly. Purpose: To determine the effect of progressive muscle relaxation (PMR) on sleep quality in the elderly. Method: Quantitative research type with pre-experimental one group pretest-posttest design. The population of all elderly people at the Labuhan Ratu Health Center was 342 elderly people with a sample size of 20 respondents. Sampling used purposive sampling technique and data analysis used the Wilcoxon test. Results: The average sleep quality of the elderly before the intervention was 10.30 with a standard deviation of 2,080, a range of values ​​7-15, while the average sleep quality of the elderly after the intervention decreased to 5.35 with a standard deviation of 1,599, a range of values ​​4-9. The difference in the average value before and after PMR was 4.95 with a standard deviation difference of 0.481. Conclusion: There was a difference in the average sleep quality of the elderly before and after the intervention PMR with a statistical test result of p-value 0.000 (<0.05), so there was an effect of PMR on the sleep quality of the elderly. Suggestion: PMR can be done independently at home and as an additional therapy that can be used to improve sleep quality.   Keywords: Elderly; Progressive Muscle Relaxation (PMR); Sleep Quality.   Pendahuluan: Lansia memiliki kualitas tidur yang buruk yang menyebabkan terganggunya aktivitas di siang hari. Progressive muscle relaxation (PMR) merupakan salah satu metode relaksasi sederhana yang melalui dua proses yaitu menegangkan dan merelaksasikan otot tubuh dan menjadi salah satu teknik yang dapat menurunkan kualitas tidur lansia. Tujuan: Untuk mengetahui pengaruh progressive muscle relaxation (PMR) terhadap kualitas tidur lansia. Metode: Jenis penelitian kuantitatif dengan pre eksperimental one group pretest-posttest design. Populasi seluruh lanjut usia yang ada di Puskesmas Labuhan Ratu sebanyak 342 lansia dengan jumlah sampel 20 responden. Pengumpulan sampel menggunakan teknik sampling purposive sampling dan analisis data menggunakan uji Wilcoxon. Hasil: Rata-rata kualitas tidur lansia sebelum intervensi adalah 10.30 dengan standar deviasi 2.080 rentang nilai 7-15, sedangkan rata-rata kualitas tidur lansia sesudah diberi intervensi mengalami penurunan menjadi 5.35 dengan standar deviasi 1.599, rentang nilai 4-9. Selisih nilai mean sebelum dan setelah PMR 4.95 dengan selisih standar deviasi 0.481. Simpulan: Terdapat perbedaan rata-rata kualitas tidur lansia sebelum dan sesudah dilakukan intervensi (PMR) dengan hasil uji statistik p-value 0.000 (< 0.05), sehingga adanya pengaruh PMR terhadap kualitas tidur lansia. Saran: PMR dapat dilakukan secara mandiri di rumah dan sebagai terapi tambahan yang bisa digunakan dalam meningkatkan kualitas tidur.   Kata Kunci: Kualitas Tidur; Lansia; Progressive Muscle Relaxation (PMR).

Abstract

Introduction: In Indonesia, projections indicate that the count of individuals suffering from kidney failure is set to rise from 19,612 to 100,000 between 2014 and 2019. The occurrence of those receiving hemodialysis, both seasoned and newcomers, from January to October 2024, reached 255 new patients alongside 352 longstanding patients. However, data concerning the quality of life for chronic kidney failure patients undergoing Hemodialysis therapy is currently nonexistent and has yet to be uncovered.Purpose: It's recognized that a connection exists between hemodialysis (in terms of duration/time, frequency, and length of HD therapy) and the quality of life of patients with chronic kidney failure.Methods: This study employs a quantitative research methodology utilizing an observational design framed within a cross-sectional approach. The population considered for this investigation was gathered from January through October 2024, with a total of 255 patients, out of which the sample comprised 156 newly diagnosed chronic kidney failure patients receiving hemodialysis. The method of sampling applied was accidental sampling. The research instrument utilized is the quality of life assessment (WHOQOL-BREF), which consists of 25 items. The analysis conducted includes both univariate and bivariate analyses, utilizing the Spearman rank test.Results: Findings from the study indicated that the majority of participants underwent hemodialysis for a duration of ≥3 hours per session, totaling 143 respondents (91.7%). The majority received hemodialysis twice a week, comprising 117 respondents (75.0%). Moreover, most of the respondents had been undergoing hemodialysis for a period of 12-24 months, totaling 69 respondents (44.2%), and a significant number reported a favorable quality of life, amounting to 48 respondents (30.8%). Statistical tests utilizing the Spearman rank test for the duration/time of HD in relation to quality of life yielded a p-value of 0.001.
