Abstract

Background: Pulmonary tuberculosis is a highly contagious disease that most commonly affects the lungs. The Lampung Provincial Health Office estimates that there will be 7,325 cases of pulmonary tuberculosis in 2024, with 5,605 cases treated. Meanwhile, the number of pulmonary tuberculosis cases in Bandar Lampung increased from 2020 to 2022. In 2020, there were 16,753 cases, 21,426 cases in 2021, and 25,403 cases in 2022. The Case Detection Rate (CDR) in Bandar Lampung City was 41% in 2020, 45% in 2021, and 40% in 2022. These figures are still below the national target of 90%. Purpose: To analyze nursing care for pulmonary tuberculosis patients with ineffective airway clearance using chest physiotherapy and effective coughing interventions. Method: A descriptive study using a case study approach to explore the problem of ineffective airway clearance nursing care in pulmonary tuberculosis patients using chest physiotherapy and effective coughing interventions in the community in Pesawaran Regency, Lampung Province. The instruments used were observation sheets, SOPs for chest physiotherapy and effective coughing. The intervention was carried out twice a day (morning and afternoon) for 3 consecutive days. Results: Respondent 1's SpO2 increased from 95% to 98%, while respondent 2's SpO2 remained stable and high, from 99% to 98%. Respondent 1's respiratory rate decreased from 24x/minute to 18x/minute, and respondent 2's respiratory rate decreased from 26x/minute to 20x/minute. Respondents 1 and 2 experienced a decrease in sputum volume to 30 cc. Conclusion: Implementing nursing interventions with chest physiotherapy and effective coughing techniques for three consecutive days effectively addressed the problem of ineffective airway clearance, normalized respiratory frequency, eliminated rhonchi, and kept SpO2 within normal limits, as well as reduced sputum production in patients with pulmonary tuberculosis. Suggestion: Healthcare facilities such as hospitals are expected to increase the routine implementation of chest physiotherapy and effective coughing techniques in the management of patients with pulmonary tuberculosis, especially those experiencing ineffective airway clearance. Keywords: Chest physiotherapy; Effective cough; Nursing care; Pulmonary tuberculosis   Pendahuluan: Penyakit tuberculosis paru merupakan penyakit menular yang sangat berbahaya dan paling sering diserang adalah paru-paru. Dinas Kesehatan Provinsi Lampung mengestimasikan jumlah kasus tuberculosis paru pada tahun 2024, estimasi kasus tuberculosis paru sebanyak 7.325 kasus dan terobati sejumlah 5.605 kasus. Sedangkan jumlah kasus tuberculosis paru di Bandar Lampung mengalami kenaikan dari tahun 2020-2022. Pada tahun 2020 sebesar 16.753 kasus, tahun 2021 sebesar 21.426 kasus dan tahun 2022 sebesar 25.403 dengan angka Case Detection Rate (CDR) di Kota Bandar Lampung tahun 2020 sebesar 41%, tahun 2021 sebesar 45% dan tahun 2022 sebesar 40% angka tersebut masih di bawah target Nasional yaitu 90%. Tujuan: Untuk menganalisis asuhan keperawatan pada pasien tuberculosis paru dengan masalah bersihan jalan napas tidak efektif menggunakan intervensi fisioterapi dada dan batuk efektif. Metode: Penelitian deskriptif dengan pendekatan studi kasus untuk mengeksplorasi masalah asuhan keperawatan bersihan jalan nafas tidak efektif pada pasien tuberculosis paru dengan menggunakan intervensi fisioterapi dada dan batuk efektif pada masyarakat di Kabupaten Pesawaran Provinsi Lampung. Instrumen yang digunakan berupa lembar observasi, SOP fisioterapi dada dan batuk efektif. Intervensi dilakukan 2 kali dalam sehari (Pagi dan sore) selama 3 hari berturut-tururt. Hasil: Menunjukkan bahwa SPO2 pada responden 1 mengalami peningkatan dari 95% menjadi 98%, sedangkan responden 2 mengalami hasil yang stabil dan tinggi dari 99% menjadi 98%. Sedangkan hasil frekuensi pernafasan pada responden 1 turun dari (24x/menit) menjadi (18x/menit) dan responden 2 mengalami penurunan dari (26x/menit) menjadi (20x/menit). Responden 1 dan responden 2 mengalami penurunan volume sputum menjadi 30 cc. Simpulan: Implementasi keperawatan dengan tindakan fisioterapi dada dan teknik batuk efektif selama 3 hari  erturut-turut efektif mampu mengatasi masalah ketidakefektifan bersihan jalan napas, menormalkan frekfensi pernafasan, menghilangkan suara ronchi, SpO2 dalam batas normal dan menurunkan pengeluaran sputum pada pasien tuberculosis paru. Saran: Diharapkan fasilitas pelayanan kesehatan seperti rumah sakit dapat meningkatkan penerapan intervensi fisioterapi dada dan teknik batuk efektif secara rutin dalam penatalaksanaan pasien tuberculosis paru, khususnya yang mengalami masalah bersihan jalan napas tidak efektif

Abstract

Introduction: In Indonesia, projections indicate that the count of individuals suffering from kidney failure is set to rise from 19,612 to 100,000 between 2014 and 2019. The occurrence of those receiving hemodialysis, both seasoned and newcomers, from January to October 2024, reached 255 new patients alongside 352 longstanding patients. However, data concerning the quality of life for chronic kidney failure patients undergoing Hemodialysis therapy is currently nonexistent and has yet to be uncovered.Purpose: It's recognized that a connection exists between hemodialysis (in terms of duration/time, frequency, and length of HD therapy) and the quality of life of patients with chronic kidney failure.Methods: This study employs a quantitative research methodology utilizing an observational design framed within a cross-sectional approach. The population considered for this investigation was gathered from January through October 2024, with a total of 255 patients, out of which the sample comprised 156 newly diagnosed chronic kidney failure patients receiving hemodialysis. The method of sampling applied was accidental sampling. The research instrument utilized is the quality of life assessment (WHOQOL-BREF), which consists of 25 items. The analysis conducted includes both univariate and bivariate analyses, utilizing the Spearman rank test.Results: Findings from the study indicated that the majority of participants underwent hemodialysis for a duration of ≥3 hours per session, totaling 143 respondents (91.7%). The majority received hemodialysis twice a week, comprising 117 respondents (75.0%). Moreover, most of the respondents had been undergoing hemodialysis for a period of 12-24 months, totaling 69 respondents (44.2%), and a significant number reported a favorable quality of life, amounting to 48 respondents (30.8%). Statistical tests utilizing the Spearman rank test for the duration/time of HD in relation to quality of life yielded a p-value of 0.001.
